Youth homelessness in San Francisco: a life cycle approach.

HIV risk behaviors and seroprevalence are particularly high among street youth. Though many programs have been designed to serve them, street youth have low rates of service utilization. The aim of this street-based, ethnographic project was to study the social and cultural context of street life in this population. Data were collected by participant observation, exploratory interviews and semi-structured interviews. Twenty street youth (15-23 years old; six female), recruited from street sites in San Francisco, participated in the interviews. Field notes and transcriptions were analyzed using

Long-term therapy and retreatment of hepatorenal syndrome type 1 with ornipressin and dopamine.

Peripheral vasodilation is considered an important factor in the pathophysiology of the hepatorenal syndrome (HRS). Therefore, the aim of this study was to evaluate the therapeutic potential of the vasoconstrictor ornipressin plus dopamine in the treatment of the most severe form of HRS, namely HRS type 1. Seven cirrhotic patients (creatinine clearance 15 +/- 1 mL/min, UNaV 7 +/- 2 mmol/24 h) with HRS type 1 were included in the study after normalization of central venous pressure with intravenous albumin and low-dose dopamine had failed to prevent further deterioration of renal function. Nashville experience with liver transplantation in Veterans Administration patients.

In this study, we compared results of 28 liver transplants performed in 25 patients referred through the Veterans Administration with 82 transplants performed in 75 nonveteran patients. The evaluation and follow-up care was provided by the same team of physicians and nurses and the transplant procedure performed in the same hospital for both patient groups. There was a significantly greater incidence of hepatitis C and/or previous alcohol abuse in veteran compared with non-VA patients [23/25 (92%) vs 29/75 (39%); P < 0.05] and a greater incidence of native portal vein thrombosis [6/25 (24%) vs

Black girls and referrals: racial and gender disparities in self-reported referral to substance use disorder assessment among justice-involved children.

BACKGROUND: There is a higher prevalence of substance use disorder (SUD) among justice-involved children (JIC). It is critical to ensure that JIC who report current use are referred for SUD assessment and potentially life-saving treatment services. Prior research suggests that certain minoritized groups may be less likely to have ever been referred for screening, and research on intersectionality suggests that these disparities may be exacerbated for racially minoritized females. METHODS: Multivariate logistic regression and interaction effects were employed to analyze longitudinal data from

The Accessibility of Opioid Agonist Treatment and Its Forced Discontinuation in Swiss Prisons-Attitudes, Perceptions and Experiences of Defense Lawyers in Dealing With Detained Persons Using Opioids.

BACKGROUND: Opioid agonist treatment (OAT) is an important pillar in the treatment of individuals using opioids and its continuation during imprisonment is recommended. Despite this knowledge access to and continuation of OAT is still limited in many countries. The forced discontinuation during pre-trial detention can cause severe withdrawal symptoms, which in turn may significantly impair the defendant's ability to exercise granted procedural participation rights. Furthermore, it can be argued that forced discontinuation of a desired treatment represents a form of a compulsory intervention.

Medical nutrition therapy in Canadian federal correctional facilities.

BACKGROUND: Under- and over nutrition as well as nutrition risk factors such as communicable and non-communicable diseases are a common and major cause of morbidity and mortality in correctional facilities. Consequently, medical nutrition therapy (MNT), a spectrum of nutrition services aimed at optimizing individual well-being, is being recognized as integral to the health of people who experience incarceration. However, there is a paucity of research that explores the delivery of MNT in correctional facilities. METHODS: A scoping review combined with secondary analysis of qualitative data (f

Understanding the context of alcohol impaired driving for fatal crash-involved drivers: A descriptive case analysis.

OBJECTIVE: The objective of this study was to understand the social context and circumstances surrounding alcohol-impaired driving prior to fatal crash involvement for drivers with an illegal blood alcohol concentration (BAC ≥ 0.05 g/100 ml or 0.00 g/100 ml for restricted license holders). METHODS: Coroners' case reports investigating fatal crashes in South Australia over a 3-year period (2008-2010) were examined. The personal and crash characteristics of drivers with an illegal BAC were compared with those who had a legal BAC. For each driver with an illegal BAC, information was recorded inc

The role of primary health care services to better meet the needs of Aboriginal Australians transitioning from prison to the community.

BACKGROUND: Aboriginal Australians are more likely than other Australians to cycle in and out of prison on remand or by serving multiple short sentences-a form of serial incarceration and institutionalisation. This cycle contributes to the over-representation of Aboriginal Australians in prison and higher rates of recidivism. Our research examined how primary health care can better meet the health care and social support needs of Aboriginal Australians transitioning from prison to the community. METHODS: Purposive sampling was used to identify 30 interviewees. Twelve interviews were with Abor

A randomized controlled trial of brief motivational interviewing in impaired driving recidivists: a 5-year follow-up of traffic offenses and crashes.

BACKGROUND: In a previously published randomized controlled trial (Brown et al. Alcohol Clin Exp Res 2010; 34, 292-301), our research team showed that a 30-minute brief motivational interviewing (BMI) session was more effective in reducing percentages of risky drinking days in drunk driving recidivists than a control information-advice intervention at 12-month follow-up. In this sequel to the initial study, 2 main hypotheses were tested: (i) exposure to BMI increases the time to further arrests and crashes compared with exposure to the control intervention (CTL) and (ii) characteristics, such

The link between maltreatment and juvenile firesetting: correlates and underlying mechanisms.

OBJECTIVE: Despite the widely held belief that abuse is a risk factor for childhood firesetting, the role of maltreatment in firesetting is largely unexplored. This study reports on a sample of children and adolescents referred to a brief assessment and intervention program for juvenile firesetters. Firesetting histories of maltreated youth were compared to a group of firesetting youth with no maltreatment history. METHODS: Participants included 205 children and youth aged 4-17 years and their caregivers. Assessments were completed with a standardized protocol. Forty-eight percent of the samp

HMDF-Net: a transfer learning-based heterogeneous multimodal dynamic fusion network for depression detection among inmates in correctional facilities.

OBJECTIVE: In correctional institutions, inmates frequently suffer from mental health issues (such as depression), which can easily lead to emergencies like suicide, self-harm, and violent conflicts, posing severe challenges to security management and recidivism prevention. Traditional assessment methods have limitations such as subjectivity, lag, and low efficiency, which may result in misjudgment, omission, and delayed judgment. To address these issues, this study aims to develop an automated depression screening model suitable for the complex scenarios in correctional institutions to achiev

Association of Early Steroid and Antibiotic Therapy With Airway Outcomes in Adult Epiglottitis: A 10-Year Multicenter Retrospective Cohort Study.

BACKGROUND: Adult epiglottitis is an uncommon but potentially life-threatening condition requiring rapid recognition and airway-focused management, yet evidence regarding early medical therapy is limited. We evaluated temporal trends in adult epiglottitis and examined associations between early corticosteroid or antibiotic therapy and clinically important outcomes. METHODS: We conducted a retrospective cohort study of adults (≥ 18 years) presenting to U.S. emergency departments with acute epiglottitis between 2014 and 2024 using the TriNetX Research Network. Prevalence of alcohol and substance use relapse in forensic inpatients with substance use disorders in Germany.

BACKGROUND: Offenders who pose a risk of harm and whose convictions are linked to substance use can be mandated to undergo treatment in forensic psychiatric hospitals under Section 64 StGB of the German Penal Code (Strafsgesetzbuch; StGB), if there are reasonable prospects that treatment might be successful. Relapses during treatment is a common occurrence in patients with substance use disorders but little is known about the frequencies of such events in a forensic setting. PURPOSE: This study aimed to determine the prevalence of relapse among patients who undergo treatment under § 64 StGB,

"Wing leaders" in recovery residences: staff key approaches supporting criminal legal system-involved residents receiving medication for opioid use disorder.

INTRODUCTION: Recovery homes (also known as recovery residences or sober living homes) are an effective approach to addressing substance use disorder in the United States and have been classified into four levels of care by the National Association of Recovery Residences (NARR). These homes may be particularly successful in supporting recovery and reintegration for individuals in the criminal legal system (CLS) through increased stability and better employment outcomes, reduced recidivism, and bolstered confidence in remaining substance-free. Despite promising findings, more research across th

Drug use among people in prison: A global review of epidemiology, harms and interventions.

BACKGROUND AND AIMS: People who use drugs are overrepresented in the criminal justice system. We aimed to provide a broad synthesis of the epidemiology, harms and interventions related to drug use and drug use disorders among incarcerated adults worldwide, and highlight gaps in evidence and practice. METHODS: In this structured review, we searched PubMed and Web of Science for articles published between 2015 and August 2025. Systematic reviews, meta-analyses and large primary studies with unselected samples were given preference for inclusion. RESULTS: Four in ten adults who enter prison mee

Reducing risk factors for child maltreatment: The Parenting-STAIR open pilot study.

BACKGROUND: Despite a large body of evidence linking the impact of trauma, parenting, and child maltreatment recidivism, current child welfare services often do not target maternal trauma and post-traumatic stress disorder (PTSD). Moreover, there is little evidence that traditional family preservation services (FPS) lower the rates of repeat incidences of child abuse and neglect. The novel intervention, Parenting-STAIR (P-STAIR), seeks to address maternal mental health and parenting skills in order to reduce punitive parenting behaviors. OBJECTIVE: This study analyzes the effects of P-STAIR o
