Isolated sphenoid inflammatory diseases associated with visual impairment: 15-year experience at a single institution.

We present a retrospective series of 11 patients treated for isolated sphenoid inflammatory disease (ISID) with visual impairment in the period between 1994 and 2008. The series included 7 females and 4 males. All patients, preoperatively assessed by CT or MR, underwent endoscopic sinus surgery and broad-spectrum antibiotic therapy. The procedure was always performed in an emergency setting, with an interval from the onset of visual impairment ranging between 1 and 40 days (mean 9 days). The possibility of recovery was related to both the modality of onset and severity of the deficit. All pati

Prospective evaluation of aerosol delivery by a powered nasal nebulizer in the cadaver model.

BACKGROUND: The objective of this study was to compare the distribution of aerosol delivered via a powered nasal nebulizer device in 5 fresh frozen-cadaver heads (10 total sides). METHODS: Nasoneb® (Medinvent, St. Paul, MN) was used to deliver a total volume of 10 mL (9 mL saline and 1 mL of 10% fluorescein). Aerosol distribution was assessed in 3 trials: (1) unoperated nose; (2) post-functional endoscopic sinus surgery (FESS); and (3) post-FESS with endoscopic modified Lothrop procedure (EML). Two independent observers rated the distribution of the fluorescein-dyed saline in the anterior nas

Endonasal approaches to the sellar and parasellar regions: closure techniques using biomaterials.

PURPOSE: We reviewed the clinical outcomes resulting from various closure techniques used following endoscopic endonasal surgery for lesions in the sellar and parasellar regions. We compared our current closure technique, which uses a biological matrix of native equine collagen (TissuDura) fixed with fibrin sealant (Tisseel), with the technique we employed previously, using autologous materials, in order to assess the comparative efficacy and tolerability of both methods over the medium- to long-term. METHODS: A review was conducted of all cases of endonasal endoscopic intervention carried ou

Transnasal surgery for infradiaphragmatic craniopharyngiomas in pediatric patients.

OBJECTIVE: Transnasal surgery has been performed in only a small number of cases of pediatric craniopharyngiomas, and its efficacy is still a matter of debate. METHODS: We analyzed the results of transnasal surgery performed in 11 pediatric patients (6 male and 5 female patients) with infradiaphragmatic craniopharyngiomas who were younger than 15 years at the time of surgery (age range, 7.7-14.9 yr) and who were treated between 1985 and 1996, when more refined diagnostic and surgical techniques were available. Two of the patients had undergone primary surgery elsewhere. The transnasal approac

Diagnostic accuracy and influencing factors of preoperative biopsies in sinonasal malignancy.

OBJECTIVES: The nasal cavity's cytological diversity necessitates pathological confirmation for treatment planning, especially in malignancies. However, the accuracy of preoperative biopsies and their influencing factors are still unclear. This study aimed to evaluate histopathologic concordance between preoperative biopsy and permanent pathology by technique and identify factors contributing to misdiagnosis. METHODS: We identified 652 patients diagnosed with sinonasal solid malignancy between 1991 and 2023. Excluding those without surgical treatment or a confirmed cancer diagnosis, 376 patie

Transcranial penetrating skull base trauma with metal rod: Endoscopic repair of CSF leak.

BACKGROUND: Penetrating skull base injuries are complex clinical scenarios requiring multidisciplinary management to address both immediate life-threatening conditions and long-term complications.Anterior skull base fractures account for 21% of skull fractures from which 4% were caused by head trauma [1]. Post-traumatic cerebrospinal fluid (CSF) leaks may arise, becoming a major source of morbidity; these can lead to the development of severe intracranial infections [2]. Endoscopic Endonasal Approach is a conservative technique that uses a multilayer coverage including a nasoseptal flap to cre

Relationship of Lesion Location to Postoperative Steroid Use in Eosinophilic Chronic Rhinosinusitis.

OBJECTIVES: Eosinophilic chronic rhinosinusitis (ECRS) is known to recur after surgery. The treatment choice for recurrent ECRS, such as oral steroids or biological agents, must be chosen carefully, and identifying the lesion location may be useful. This study aimed to evaluate the postoperative course of ECRS patients and assess the relationship between endoscopic lesion location and postoperative oral steroid use. METHODS: Patients with chronic rhinosinusitis who underwent bilateral endoscopic sinus surgery from April 2018 to March 2020 were divided into two groups based on the presence or

Clinical Presentation and Outcome of Sinonasal Extraosseous Plasmacytoma in Denmark: A Nationwide Cohort From 1980 to 2017.

OBJECTIVES: Extraosseous plasmacytoma (EOP) is a rare plasma cell neoplasm that tends to convert to plasma cell myeloma (PCM) in about 11% to 35% of cases. It has a predilection for the upper respiratory tract, prototypically affecting the nasal cavity and paranasal sinuses. Contemporary first-line treatment is radiotherapy, with more recent studies showing an added benefit of combining radiation with surgery. In this cohort study, we aimed to examine clinical presentation, treatment, and prognosis for all patients nationwide from 1980 through 2017. Furthermore, we determined the size and exte

Late-Onset Nonhereditary Cherubism: First Reported Case in Ghana With Review of Diagnostic and Management Challenges.

BACKGROUND: Cherubism is a rare genetic disorder characterised by multilocular cystic lesions in the mandible and/or maxilla, which result in the typical cherub-like face. Two forms of cherubism exist-hereditary (familial) and nonhereditary (nonfamilial)-and it usually occurs amongst children aged 2-7 years. The disorder is caused by a mutation in the SH3BP2 gene on Chromosome 4p16.3, essential for jaw development. The prognosis of cherubism shows that lesions increase in size and plateau at puberty, after which the lesions begin to regress and become undetectable. We present the first case of

[First clinical experiences with an endoscope manipulator system in endo- and transnasal surgery].

BACKGROUND: Endo- and transnasal surgery needs optical support. The use of a microscope allows bimanual manipulation. More often the endoscopic technique is used which needs one hand for endoscope guidance "loosing" it for manipulation or demanding an assistant for endoscope guidance. In this work the use of a miniature endoscope manipulator system for endonasal and transnasal surgery was evaluated. MATERIAL AND METHODS: 31 FESS with manipulator-assisted endoscope guidance were performed. The used endoscope positions, the number of position changes and conditional interruptions were documente
