Totally laparoscopic resection of right-sided colon cancer using transvaginal specimen extraction with a 10-mm-long abdominal incision.

BACKGROUND: Natural orifice specimen extraction (NOSE) has been developed as a means of decreasing the incidence of surgical wound complications. We refined the procedure for totally laparoscopic colectomy with transvaginal specimen extraction using the reduced port surgery technique with the ultimate goal of attenuating damage to the abdominal wall. We herein report this innovative technique and its short- and long-term outcomes. METHODS: We prospectively collected data on seven patients who underwent totally laparoscopic colectomy using transvaginal specimen extraction with a 10-mm-long abd

Video-assisted thoracoscopic lobectomy in children: safety, efficacy, and risk factors for conversion to thoracotomy.

BACKGROUND: Video-assisted thoracoscopic lobectomy in small children has not been widely performed because of difficulties in single-lung ventilation and surgical technique. This study assessed the feasibility, outcomes, and risk factors for conversion to thoracotomy of thoracoscopic lobectomy in children. METHODS: From 2005 to 2011, thoracoscopic lobectomy was tried in 50 consecutive pediatric patients. The median age was 3.2 years and the median body weight was 16 kg. Congenital cystic adenomatoid malformation (CCAM) (78%) and pulmonary sequestration (18%) were the most common diagnoses. Pr

Pancreatic fistula after distal pancreatectomy: predictive risk factors and value of conservative treatment.

HYPOTHESIS: Predictive factors of pancreatic fistula (PF) and the value of conservative management of PF following distal pancreatectomy (DP) are poorly known. DESIGN: Case series. SETTING: A university hospital referral center. PATIENTS: From 1991 to 2003, 175 patients underwent DP with routine drainage of the pancreatic stump and postoperative repeated measures in drainage fluid. Pancreatic fistula was defined as an amylase level in surgical drainage fluid more than 5-fold the serum level after postoperative day 5, or amylase-rich fluid collection. Computed tomographic scan was only done

An outermost-layer-oriented medial approach to facilitate margin-negative distal transection in gastric cancer with duodenal invasion: a single-centre retrospective cohort study.

BACKGROUND: Gastric cancer with duodenal invasion (Duo+) presents a distinct surgical challenge because adequate distal-margin clearance must be achieved without compromising duodenal stump safety. Standardised laparoscopic strategies for this setting remain poorly defined. METHODS: In this single-centre retrospective cohort study, we evaluated the technical feasibility and observed outcomes of an integrated extended-mobilisation and margin-control strategy in 500 patients undergoing laparoscopic distal gastrectomy with D2 lymphadenectomy for stage II-III gastric adenocarcinoma. The cohort in

Single-stapled versus double-stapled colorectal anastomosis after anterior resection: Systematic review and meta-analysis of short-term and functional outcomes.

BACKGROUND: The double-stapled anastomosis technique is widely considered the standard approach for colorectal anastomosis after anterior rectal resection. Recently, the single-stapled anastomosis technique has gained attention because it may reduce potential weak points that could contribute to anastomotic leakage by avoiding stapler line intersections. This systematic review and meta-analysis aim to compare surgical and functional outcomes between single-stapled anastomosis and double-stapled anastomosis. METHODS: A systematic literature search of PubMed, EMBASE-Medline, and the Cochrane Li

Robotic secondary cytoreduction in recurrent ovarian cancer: a tailored approach for kidney transplant recipients.

BACKGROUND: The rate of kidney transplantation has been steadily increasing worldwide, accompanied by significant improvements in post-transplant survival rates. However, transplant recipients have a higher incidence of malignancies compared with the general population, and their oncological management often poses unique challenges. In recent years, major advances in the treatment of ovarian cancer (OC) have expanded the therapeutic options available for recurrent disease. Two randomised trials have underscored the role of surgery in platinum-sensitive recurrent OC while minimally invasive app

Evaluation of the nonstapling bullectomy by manual suturing for young men with primary spontaneous pneumothorax.

BACKGROUND: Currently, surgery for primary spontaneous pneumothorax (PSP) is performed by bullectomy using a stapler with complete video-assisted thoracic surgery (cVATS). However, the postoperative recurrence rate (RR) of PSP is high in young men. The factors of postoperative PSP recurrence are the formation of postoperative bulla neogenesis (POBN) around the staple line, pleural injury caused by forceps for VATS, and bulla overlooked with a thoracoscope. We attempted nonstapling bullectomy with one-port-one-window (1p-1w) by using hybrid VATS (hVATS) to reduce postoperative RR. This study ai

Minimally invasive pancreaticoduodenectomy for circumportal pancreas: literature review and report of two type IIIA cases.

BACKGROUND: Circumportal pancreas is a rare morphological variant with clinical significance due to the high risk of postoperative pancreatic fistula in patients undergoing pancreaticoduodenectomy. Type IIIA (suprasplenic anteportal) is the most common type of circumportal pancreas. We present two cases of type IIIA treated with minimally invasive pancreaticoduodenectomy, and review the literature on patients with circumportal pancreas who underwent pancreatic surgery. Unilateral single-port thoracoscopic surgery for bilateral pneumothorax or pulmonary bullae.

BACKGROUND: Rapid rehabilitation surgery has become a widely accepted approach. Thoracic surgeons have attempted in many ways to make surgery less invasive. We combined tubeless technology, single-port technology and mediastinum approach for the treatment of simultaneous bilateral primary spontaneous pneumothorax(PSP)or pulmonary bullae. And we evaluated its therapeutic effect. This study aimed to investigate if tubeless single-port video-assisted thoracic surgery (Tubeless-SPVATS) via anterior mediastinum can be used as an alternative surgical treatment for bilateral lung diseases, especially

Intracorporeal vs laparoscopic-assisted resection for uncomplicated diverticulitis of the sigmoid.

BACKGROUND: Minimally invasive surgery for uncomplicated diverticulitis of the sigmoid (UDS) may be performed either as an intracorporeal procedure (LICR) or as laparoscopically assisted colon resection (LACR). METHODS: Prospectively collected data of 40 selected patients who had undergone LICR for UDS between 1992 and 1994 were compared retrospectively with those of 34 diagnosis-matched LACR controls operated on at the same hospital between 1995 and 1996 to assess the short-term outcome. RESULTS: There were no mortalities. LICR and LACR patients were well matched for age, gender, weight, Am

A Case of Esophageal Retention Cyst with High Fluorodeoxyglucose Uptake on PET/CT Scan.

INTRODUCTION: Esophageal retention cysts are rare, benign lesions that can mimic submucosal tumors. Their clinical presentation and imaging characteristics may lead to diagnostic challenges, particularly when fluorodeoxyglucose-positron emission tomography/CT (FDG-PET/CT) shows increased uptake, raising suspicion of malignancy. CASE PRESENTATION: A 77-year-old man presented with epigastric pain. Upper gastrointestinal endoscopy revealed an esophageal mass, prompting referral to our hospital. Endoscopic ultrasonography (EUS) identified a hypoechoic submucosal tumor with multiple cystic compone

Combined resection of lung cancer and thoracic aortic wall with simultaneous thoracic aortic endografting: a case report.

BACKGROUND: Combined resection of lung cancer and the thoracic aortic wall with thoracic aortic endografting has been reported. However, whether the resection and endografting should be performed simultaneously or in two steps remains controversial. CASE PRESENTATION: A 68-year-old man was referred to our hospital because of left chest pain. Chest contrast-enhanced computed tomography revealed a huge tumor of the left lower lung lobe, and invasion to the aortic wall was suspected. Bronchoscopic examination was performed, revealing squamous cell carcinoma with a programmed death ligand 1 expre

Laparoscopic Purse-String Suture Technique for Total Intracorporeal Rectosigmoid End-to-End Anastomosis After Segmental Bowel Resection.

OBJECTIVE: Bowel endometriosis affects 8% to 12% of women with infiltrating endometriosis, mostly involving the rectum and sigmoid [1]. Surgery is preferred when medical therapy fails or is contraindicated. Although segmental resection has shown good outcomes, it carries significant risks of perioperative complications [1-3], partially due to the mini-laparotomy required for specimen retrieval and bowel anastomosis (postoperative pain, wound-related issues, blood loss, hernias). Total intracorporeal laparoscopic anastomosis may reduce them [4,5]. While promising, experience with this technique

Estrangulated obturatory hernia. Case report of a challenging diagnosis of obstructive syndrome.

INTRODUCTION AND IMPORTANCE: Obturator hernia accounts for less than 1 % of all documented cases of pelvic hernias. It most commonly presents as an obstructive syndrome in elderly, multiparous, slim women, characterized by a wider pelvis that facilitates the passage of the hernia sac through the obturator foramen alongside the obturator nerve. In this case, adhering to the SCARE (Updating Consensus Surgical CAse REport) checklist criteria, we present a typical scenario involving an elderly woman who was initially misdiagnosed with a fecaloma, concealing an obturator hernia. CASE REPORT: An 85
