Vascular Complications in Microvascular Decompression: A Survey of 4000 Operations.

BACKGROUND: Vascular complications in posterior fossa surgery, even in microvascular decompression (MVD) involving a small cranial opening, can have catastrophic consequences. We analyzed these complications to determine the incidence, risk factors, prognosis, and preventive measures involved. METHODS: Between April 1997 and March 2018, 4000 consecutive patients with neurovascular compression syndrome were admitted and underwent MVD. We reviewed the medical records of patients who developed vascular complications after MVD, focusing on their past medical history, perioperative laboratory find

Pipeline-assisted coil embolization of a large middle cerebral artery pseudoaneurysm in a 9-month-old infant: experience from the youngest flow diversion case.

Intracranial aneurysms in the pediatric population are rare entities. The authors recently treated a 9-month-old infant with a 19-mm recurrent, previously ruptured, and coil-embolized left middle cerebral artery (MCA) pseudoaneurysm, which was treated definitively with single-stage Pipeline-assisted coil embolization. The patient was 5 months old when she underwent resection of a left temporal Grade 1 desmoplastic infantile ganglioglioma at an outside institution, which was complicated by left MCA injury with a resultant 9-mm left M1 pseudoaneurysm. Within a month, the patient had two aneurysm

Cerebellar Tonsillar Cysts Associated with Chiari Malformation in Adults: A Short Series.

BACKGROUND: Authors analyzed surgical patients with Chiari malformation (CM) associated with cerebellar tonsillar cyst. This association is quite rarely encountered in the literature. CASE DESCRIPTION: We retrospectively reviewed 60 surgical adult patients with MRI evaluation who underwent posterior fossa decompression for treatment of CM type I. All surgeries were performed between January 1995 and July 2015. From 60 patients with CM, four cases presented cerebellar tonsillar cysts (male=2). Basilar invagination (BI) (n=4) and syringomyelia (n=1) were associated. There were pyramidal signs (

Microvascular Decompression for Trigeminal Neuralgia: Technical Refinement for Complication Avoidance.

BACKGROUND: Microvascular decompression (MVD) represents the most effective and safe surgical option for the treatment of trigeminal neuralgia since it was first popularized by Jannetta 50 years ago. Despite several advances, complications such as cerebellar and vascular injury, hearing loss, muscular atrophy, cerebrospinal fluid (CSF) leak, postoperative cutaneous pain, and sensory disturbances still occur and may negatively affect the outcome. We propose some technical nuances of the surgical procedure that were used in our recent series. METHODS: We used a novel hockey stick-shaped retroma

Outcome of microvascular decompression for trigeminal neuralgia using autologous muscle graft: A five-year prospective study.

INTRODUCTION: Trigeminal Neuralgia (TGN) is a syndrome characterized by Paroxysmal, shock like hemifacial pain. Among the various treatment options micro vascular decompression (MVD) has gained popularity in the recent years. MATERIALS AND METHODS: 182 patients underwent MVD, between 1995-2007 out of 530 patients treated for Trigeminal Neuralgia at our service. All were operated by retro auricular sub occipital craniectomy by a single surgeon using autologous muscle graft. They were assessed for pain relief, complications and the data was analysed. RESULTS: Males were 84 (61.3%) females 53 (

Monitoring papilledema in a patient with pineal pilocytic astrocytoma by using Heidelberg Retina Tomograph II. Case report.

The Heidelberg Retina Tomograph (HRT) II is a confocal scanning laser device used to acquire images of the optic nerve head. In this paper the authors report a novel use of this device in the assessment of papilledema in a patient with pilocytic astrocytoma of the pineal region. They also present a new technique to objectively produce 3D data of the optic nerve head. An 11-year-old girl presented with headache and bilateral papilledema; magnetic resonance imaging revealed a lesion in the pineal region. A suboccipital craniectomy was performed, and HRT images were obtained both before (baseline

[Chiari type 1 malformation and magnetic resonance imaging].

The Chiari type 1 malformation is common. Unlike the Chiari type 2 and 3 malformations, it may remain latent for a long time, becoming symptomatic only in adulthood. The introduction of MRI has resulted in an increased number of diagnoses of this malformation in pediatric patients. It appears to be related to underdevelopment of the posterior cranial fossa. It must be differentiated from acquired tonsillar herniation, particularly when herniation results from intracranial hypotension; these cases are sometimes reported as acquired Chiari I malformation with spontaneous resolution. Tonsillar ec

Trigeminal Neuralgia.

The initial treatment for trigeminal neuralgia is medical. Carbamazepine is the drug of choice. If the patient proves to be intolerant of carbamazepine, a number of second-line drugs are available, though data on their relative efficacy are nonexistent. Phenytoin, baclofen, clonazepam, and sodium valproate are all worthy of consideration. Oxcarbazepine may be as effective as carbamazepine, but its availability is limited. Newer agents being tried in this condition include lamotrigine and gabapentin. Their comparative value has not been established. For patients resistant to or intolerant of dr

Early radical treatment of pancraniofacial synostosis.

Premature fusion of all major cranial and facial sutures, termed pancraniofacial synostosis by us, was first described as "Kleeblattschädel deformity," but can also be found in anomalies such as Crouzon's disease, Apert's syndrome, Pfeiffer's syndrome, and severe forms of plagiocephaly. This rare craniofacial anomaly often presents an immediate threat to life right after birth. Early subtotal craniectomy frequently combined with frontoorbitofacial advancement must be performed to decrease intracranial pressure, preserve vision, and open up the blocked upper airway. Concomitant hydrocephaly is

Effects of cranioplasty with customized 3D titanium mesh plates on postoperative complications and neurological outcomes following traumatic brain injury or stroke: a single-center retrospective study.

BACKGROUND AND OBJECTIVES: Persistent controversy surrounds the identification of risk factors contributing to postoperative complications and unfavorable neurological prognoses following cranioplasty (CP) after decompressive craniectomy (DC). This study sought to assess these postoperative outcomes in individuals undergoing 3D titanium mesh CP due to traumatic brain injury (TBI) or hemorrhagic stroke, as well as to determine predictors linked to postoperative complications and suboptimal neurological recovery. METHODS: A retrospective, single-center analysis was performed on patients undergo

The cranioplasty equation: the influence of biomaterials, surgical timing, and frailty on optimising success.

Cranioplasty outcomes are influenced by multiple perioperative factors, yet the interplay between frailty, surgical timing, and material selection remains incompletely understood. This study aimed to characterise the independent and combined effects of these variables on postoperative complications. We conducted a retrospective study of 315 patients undergoing cranioplasty following decompressive craniectomy between January 2014 and June 2024. Patients were stratified by preoperative frailty using the modified frailty index (mFI: not frail [0], pre-frail [1-2], frail [≥ 3]), timing of reconstr

Cerebral Myiasis Associated with Artificial Cranioplasty Flap: A Case Report.

BACKGROUND: Cranioplasty is a commonly performed procedure for the repair of cranial defects. Various materials have been used for this procedure and have a good safety profile. Human cerebral myiasis is an exceedingly rare condition. It involves the invasion of live or dead human tissues by larvae of the insect species dipterous. CASE DESCRIPTION: We describe the first case of cerebral myiasis associated with an artificial cranioplasty bone flap. There was delayed cerebral cortex infestation of the species dipterous after cranioplasty with polymethyl methacrylate bone flap. The patient initi

Brain infarction as the initial presentation of primary posterior mediastinal seminoma.

A 40-year-old man was admitted with acute onset of slurred speech and right hemiparesis. Cerebral infarction was diagnosed from magnetic resonance (MR) images. Chest X-ray incidentally revealed a posterior mediastinal tumour, which proved to be seminoma on CT-guided biopsy. The patient then received chemotherapy. Five months later, right upper limb weakness and slurred speech occurred again. Brain MR images revealed two metastatic lesions in the prior infarct areas, which on histology proved to be metastatic seminoma.

Pathophysiology of persistent syringomyelia after decompressive craniocervical surgery. Clinical article.

OBJECT: Craniocervical decompression for Chiari malformation Type I (CM-I) and syringomyelia has been reported to fail in 10%-40% of patients. The present prospective clinical study was designed to test the hypothesis that in cases in which syringomyelia persists after surgery, craniocervical decompression relieves neither the physiological block at the foramen magnum nor the mechanism of syringomyelia progression. METHODS: The authors prospectively evaluated and treated 16 patients with CM-I who had persistent syringomyelia despite previous craniocervical decompression. Testing before surger

Quality of life after decompressive surgery for severe cerebral venous thrombosis.

BACKGROUND: Decompressive surgery can be lifesaving in patients with severe cerebral venous thrombosis (CVT) and impending brain herniation. However, data on health-related quality of life (HRQoL) after surgery are limited. METHODS: DECOMPRESS-2 was a prospective cohort study including adult patients with CVT from 15 centers in 10 countries who underwent decompressive surgery (2011-2019). HRQoL was assessed using EQ5D-3 L utility scores, subdomains, and a visual analogue scale (VAS) at 6 and 12 months post-surgery.
