OBJECTIVE: Many recent studies have reported on intratympanic gentamicin therapy for the treatment of intractable Ménière's disease. Intratympanic administration of steroids has also been used to treat sudden sensorineural hearing loss. These intratympanic drug therapies are based on the assumption that the drug administered intratympanically enters the inner ear through the round window membrane. We used magnetic resonance imaging (MRI) to evaluate whether and how intratympanically administered gadolinium (Gd) enters the inner ear.
METHODS: GD hydrate was injected intratympanically through t

OBJECTIVE: The p-ADH level in cases of juvenile unilateral profound deafness (JUPD) and the timecourse of the level were examined to investigate whether or not an increase of p-ADH is involved in the development of delayed endolymphatic hydrops (DEH) in JUPD.
MATERIALS AND METHODS: In 90 consecutive patients with unilateral profound or total sensorineural deafness with the onset in early childhood, pure-tone audiometric examination and the measurement of p-ADH and plasma osmolality (p-OSM) were followed up once or twice a year as far as possible. At every testing, we performed careful history

OBJECTIVES/HYPOTHESIS: In 1991, the translabyrinthine approach was modified by sealing the vestibule with bone wax, which allowed preservation of the hearing function in one patient. The present study aimed specifically at evaluating the effectiveness of the modified translabyrinthine approach in preserving hearing function in a group of patients with vestibular schwannoma that involved the internal auditory canal.
STUDY DESIGN: Prospective study.
METHODS: The series consisted of 12 patients with vestibular schwannoma (average age, 49.7 y). The schwannoma was smaller than 2 cm in all patient

Histopathologic study of the human temporal bone entails microscopic examination and analysis of a series of histologic sections. This is currently the most effective method for observing the pathologic conditions of MD by examining the entire inner ear. Complete temporal bone histopathology cannot be replaced by either clinical pathologic study of small biopsy specimens obtained during surgery, or experimental animal studies that can create endolymphatic hydrops but not create MD. We believe that the histopathologic findings together with clinical information on MD is valuable in enhancing ou

Different means of identifying the presence of cerebello-pontine angle (CPA) tumours using brainstem auditory-evoked potentials (BAEP) were evaluated. The results from 33 ears affected by tumours were compared with BAEP from 50 normal ears and 23 ears affected by Menière's disease. Measurement of inter-aural latency differences correctly identified 97% of tumours from the normal group, but only 81% of tumours from the Menière group. Comparison of different interwave interval measures showed that the NI to NV interval was the best criterion and correctly identified 90% of the tumours from both

Dizziness is a frequent complaint after head trauma. Among patients who suffer a concussion (mild traumatic brain injury or mTBI), dizziness is second only to headache in symptom frequency. The differential diagnosis of post-concussive dizziness (PCD) can be divided into non-vestibular, central vestibular and peripheral vestibular causes with growing recognition that patients frequently exhibit both central and peripheral findings on vestibular testing. Symptoms that traditionally have been ascribed to central vestibular dysfunction may be due to peripheral dysfunction. Further, our ability to

HYPOTHESIS/BACKGROUND: We hypothesize that following head trauma there is a difference in temporal bone (TB) pathology in cases with and without skull fracture. Although conductive, sensorineural, mixed hearing loss, and TB pathology following head trauma have been reported, to our knowledge, there are no studies that have compared the pathology of the TB in cases with and without skull fracture.
METHODS: We analyzed 34 TBs from donors who had a history of head trauma (20 with skull fracture and 14 without fracture), and 25 age-matched controls without clinical or histological evidence of oto

PURPOSE: There is still a clinical-radiologic discrepancy in patients with Menière's disease (MD). Therefore, the purpose of this study was to investigate the reliability of current MRI endolymphatic hydrops (EH) criteria according to Baráth in a larger study population and the clinical utility of new imaging signs such as a supplementary fourth low-grade vestibular EH and the degree of perilymphatic enhancement (PE) in patients with Menière's disease (MD).
METHODS: This retrospective study included 148 patients with probable or definite MD according to the 2015 American Academy of Otolaryngo

Over a period of 4 years, 420 patients were observed with fullness in the ear, episodic vertigo, fluctuating hearing, and tinnitus. Of them, 182 patients showed normal hearing levels, a mild eustachian tube dysfunction, normal SP/AP ratios, mydriasis on the side of the affected ear, and a functional disorder of the upper cervical spine. These patients responded to conservative management, particularly physiotherapy. Additionally, 51 patients showed normal hearing levels, a mild eustachian tube dysfunction, an elevated SP/AP ratio suggesting endolymphatic hydrops, mydriasis on the side of the a

OBJECTIVE: The unilateral, low-frequency hearing loss typical of Menière's disease (MD) is often accompanied by a sensation of pressure in the ear. The pathophysiological mechanism remains unclear. The aim of this exploratory study was to investigate whether unilateral low-frequency hearing loss leads to the onset of a sensation of pressure and thus constitutes a possible mechanism for this symptom.
MATERIAL AND METHODS: To simulate Menière's disease-typical hearing loss, 20 normal hearing subjects were presented binaurally with sentences of the Oldenburg Sentence Test (OLSA) with unilateral

OBJECTIVE: While cochlear ossification is a common sequalae of meningitic labyrinthitis, less is known about the effects of meningitis on peripheral vestibular end organs. Herein, we investigate histopathologic changes in the peripheral vestibular system and cochlea in patients with a history of meningitic labyrinthitis.
METHODS: Temporal bone (TB) specimens from patients with a history of meningitis were evaluated and compared to age-matched controls. Specimens were evaluated by light microscopy and assessed for qualitative changes, including the presence of vestibular and/or cochlear endoly

OBJECTIVES/HYPOTHESIS: To assess the utility of imaging in planning intratympanic (IT) gentamicin (Gent) treatment in Ménière's disease (MD), we compared the dosage and outcomes of ITGent with the severity and extent of endolymphatic hydrops (EH), as evaluated by three-dimensional fluid-attenuated inversion recovery (3D-FLAIR) sequence in a 3-T magnetic resonance imaging (MRI) unit, after IT gadolinium administration.
STUDY DESIGN: Retrospective review.
METHODS: A total of 18 patients (10 males and 8 females; age, 28-78 years; median age, 53.2 years) with definite MD participated in the inve

We have cyclically suppressed the 2f1-f2 distortion product otoacoustic emission (DPOAE) with low-frequency tones (17-97 Hz) as a way of differentially diagnosing the endolymphatic hydrops assumed to be associated with Ménière's syndrome. Round-window electrocochleography (ECochG) was performed in subjects with sensorineural hearing loss (SNHL) on the day of DPOAE testing, and from which the amplitude of the summating potential (SP) was measured, to support the diagnosis of Ménière's syndrome based on symptoms. To summarize and compare the cyclic patterns of DPOAE modulation in these groups we

Low-frequency hearing impairment (LFHI) is mainly attributed to endolymphatic hydrops and has a great variety of possible outcomes. At present, no conservative therapeutic regimen has proven to be 'gold-standard', and information about the prognostic indicators of LFHI is scarce. In a retrospective investigation, we evaluated the records of 90 patients who had been treated with infusions improving blood perfusion. In patients lacking complete remission, dehydration infusion therapy was added. We also undertook audiometric follow up. We calculated the outcomes after infusion therapy, dehydratio

BACKGROUND: Menière's disease is defined as the presence of recurrent, spontaneous episodic vertigo, hearing loss (HL), aural fullness, and tinnitus. The occurrence of attacks is unpredictable. The etiology is still unknown, but the disease has a pathologic correlate in hydropic distension of the endolymphatic system. Earlier studies have shown increased incidence of stress on the same day as vertigo attacks, but it has not been determined whether stress occurring on the day of the vertiginous episode came before or after the onset of the vertigo.
METHODS: A case-crossover study including 46

Diagnosis of Menière's Disease is based upon the wellknown labyrinthic syndrome (hypoacusia, tinnitus and dizziness) which manifests with the typical abscessual, recurrent and unforeseeable course. Many Menière patients also report panic attacks, agoraphobia and anxiety which make evaluation of labyrinthic symptoms difficult, affect the onset and maintenance of vestibular compensation and require specific treatments which are very different from those for the control of endolymphatic hydrops. In order to analyse the nature and the meaning of the association of the vestibular and psychiatric sy
