Ocular and Systemic Phenotyping of Bardet-Biedel Syndrome Type 7 (BBS7) in a Palestinian Male: Case Report and Literature Review.

INTRODUCTION: Bardet-Biedl syndrome (BBS) is a rare autosomal recessive ciliopathy with at least 26 identified causative genes. BBS7 accounts for approximately 1.5% of cases. The syndrome is characterized by retinal degeneration, obesity, polydactyly, intellectual disability, and renal anomalies. BBS7 encodes a core subunit of the BBSome complex, which is essential for ciliary function and intracellular signaling. CASE PRESENTATION: We report a 9-year-old Palestinian boy with BBS7, confirmed by whole exome sequencing identifying a homozygous pathogenic variant in the BBS7 gene [c.712_715del];

Horizontal extraocular muscle insertion locations in children undergoing extraocular muscle surgery.

BACKGROUND: The location of extraocular muscle (EOM) insertions is clinically relevant in ophthalmologic surgery. The spiral of Tillaux has been a reference for normal EOM insertion since the nineteenth century. Research on EOM insertions is limited and has focused on adult cadaveric eyes. In this study we investigated the location of EOM insertions in children undergoing EOM surgery. METHODS: Children who underwent EOM surgery for strabismus or anomalous head position at a large tertiary care center were consecutively identified from October 2022 to December 2023. Intraoperative measurements

Incomitant strabismus associated with instability of rectus pulleys.

PURPOSE: Connective tissue pulleys serve as functional mechanical origins of the extraocular muscles (EOMs) and are normally stable relative to the orbit during gaze shifts. This study evaluated pulley stability in incomitant strabismus. METHODS: Contiguous 2- or 3-mm thick magnetic resonance images (MRIs) perpendicular to the orbital axis spanned the anteroposterior extents of 12 orbits of six patients with incomitant strabismus. Imaging was performed in central gaze, supraduction, infraduction, abduction, and adduction. Rectus EOM paths were defined by their area centroids and plotted in a

[Complicated oculomotor disorders evaluated by the medical expert].

The association of sensory and motor defects may lead the medical expert to a false estimation of a patient's impairment, especially (1) if he simply adds to the sensory impairment of one eye the motor impairment of the other, without thinking of the eyes as a paired organ; (2) if he underestimates a cumulation of sensory and motor defects in the case of partial adaptation, and (3) if he judges from dyskinetic behavioral characteristics that the patient is blind. Several clinical cases illustrating these problems and allowing the following recommendations are described: (1) Prior to simply sum

Resting cerebral blood flow alterations specific to the comitant exophoria patients revealed by arterial spin labeling perfusion magnetic resonance imaging.

PURPOSE: It has been shown in many previous studies that there were significant changes of the brain anatomy and function in strabismus. However, the significance of the alterations of resting cerebral blood flow (CBF) in comitant exophoria (CE) remains obscure. Arterial spin labeling (ASL) MRI, which is a noninvasive method, could be applied to detect the cerebral blood flow quantitatively. Our study aimed to compare the resting CBF between the comitant exophoria and health controls using pseudo-continuous arterial spin labeling (pCASL) perfusion MRI method. METHODS: 32 patients (25 males an

Eye deviation in patients with one-and-a-half syndrome.

To understand malalignments of the visual axes in one-and-a-half syndrome, we measured eye positions in 4 patients with this syndrome under two conditions: with Frenzel goggles to prevent eye fixation and without Frenzel goggles. When fixation was prevented with the Frenzel goggles, all patients showed mild outward deviation in both eyes. Removal of the Frenzel goggles elicited adduction of the eye ipsilateral to the side of the lesion for fixation, with greater outward deviation of the contralateral eye (acute stage), or adduction of both eyes to midposition for biocular fixation (convalescen

Effects of strabismus on responsivity, spatial resolution, and contrast sensitivity of cat lateral geniculate neurons.

Rearing cats with esotropia is known to cause a number of deficits in visual behavior tested through the deviated eye. These include a loss of orienting response to stimuli presented in the nasal visual field of the deviated eye, a reduction in visual acuity, and a general reduction in contrast sensitivity at all spatial frequencies. To assess the involvement of the lateral geniculate nucleus (LGN) in these deficits, we measured the following: 1) the visual responsiveness of lamina A1 cells with peripheral (more than 10 degrees from area centralis) receptive fields in three esotropic and three

Chronic oculomotor nerve palsy secondary to a sphenoid-cavernous sinus lesion in an adolescent: a case report and literature review.

BACKGROUND: Oculomotor nerve (cranial nerve III) palsy in children and adolescents is uncommon and typically suggests an underlying structural lesion rather than microvascular ischemia. Sphenoid sinus disease represents a rare cause of ocular cranial neuropathies and most frequently affects the abducens nerve. Consequently, isolated chronic oculomotor nerve palsy secondary to sphenoid sinus pathology is an unusual clinical presentation. CASE PRESENTATION: We report a 16-year-old male who presented with long-standing right ocular deviation and progressive limitation of eye movements beginning
