INTRODUCTION: Fourth ventricular hamartomas (FVHs) are extremely rare lesions, and their association with refractory epilepsy is often underrecognized. We present a pediatric case of cerebellar-origin epilepsy with complete seizure remission after surgical resection, followed by a literature-based discussion of diagnostic and therapeutic considerations.
CASE REPORT: We describe a 9-year-old female with drug-resistant epilepsy since infancy. She was diagnosed with a fourth ventricular roof hamartoma (FVRH), and after two previous surgeries with no improvement, a third procedure at our institut

BACKGROUND: Currently, there are no definitive guidelines in the investigation and management of atypical facial palsies (AFPs). Our aim was to determine the etiology of AFPs presenting to a tertiary facial palsy center and to review the current spectrum of diagnostic and management approaches to these conditions.
METHODS: A retrospective cohort analysis of attendees at the Queen Victoria Hospital multidisciplinary facial palsy clinic over a 5-year period from 2016 to 2020 was conducted. Demographic data were collated from the QVH Research and Governance team. Those presenting with classic Be

PURPOSE: Crotoxin is the main neurotoxin of South American rattlesnake Crotalus durissus terrificus. The neurotoxic action is characterized by a presynaptic blockade. The purpose of this research is to assess the ability of crotoxin to induce temporary paralysis of extraocular and facial muscles in humans.
METHODS: Doses of crotoxin used ranged from 2 to 5 units (U), each unit corresponding to one LD50. We first applied 2U of crotoxin in one of the extraocular muscles of 3 amaurotic individuals to be submitted to ocular evisceration. In the second stage, we applied crotoxin in 12 extraocular

The authors describe an acute facial and acoustic neuropathy following gamma knife surgery (GKS) for vestibular schwannoma (VS). This 39-year-old woman presenting with tinnitus underwent GKS for a small right-sided intracanalicular VS, receiving a maximal dose of 26 Gy and a tumor margin dose of 13 Gy to the 50% isodose line. Thirty-six hours following treatment she presented with nausea, vomiting, vertigo, diminished hearing, and a House-Brackmann Grade III facial palsy. She was started on intravenous glucocorticosteroid agents, and over the course of 2 weeks her facial function returned to H

BACKGROUND: Botulinum toxin type A (BoNT-A) is widely employed for cosmetic purposes and in the treatment of certain diseases such as strabismus, hemifacial spasm and focal dystonia among others. BoNT-A effect mainly acts at the muscular level by inhibiting the release of acetylcholine at presynaptic levels consequently blocking the action potential in the neuromuscular junction. Despite the great progress in approval and pharmaceutical usage, improvement in displacing BoNT-A to other pathologies has remained very limited. Patients under diagnosis of several types of cancer experience pain in

BACKGROUND: Hyperactive dysfunction syndrome (HDS) of the cranial nerves, such as trigeminal neuralgia (TN), hemifacial spasm (HFS), and glossopharyngeal neuralgia (GPN), is commonly managed by microsurgical treatment. However, certain patients may present a combination of these syndromes in the neurosurgery department. Here, we aimed to retrospectively assess patients with combined HDS from a single center.
METHOD: Of 1275 consecutive patients with HDS treated at our center between 2007 and 2017, 37 patients with combined HDS were enrolled, and their medical and surgical records were analyze

OBJECTIVE: Opening of mastoid air cells (MACs), a complication of lateral suboccipital craniotomy for microvascular decompression (MVD), might cause cerebrospinal fluid (CSF) leakage and meningitis. Only a few studies have investigated the association of degree of MAC development and actual opening of MACs. The present study focused on preoperative risk assessment for predicting the opening of MACs.
PATIENTS AND METHODS: The study included 204 consecutive patients who underwent MVD at our hospital between January 2014 and December 2018. We retrospectively collected clinical and neuroimaging d

We present the surgical techniques and results of cross-facial nerve grafting that have been developed in the repair of ocular-oral synkinesis after facial paralysis. Eleven patients with ocular-oral synkinesis after facial paralysis underwent the cross-facial nerve grafting with facial nerve transposition at a tertiary academic hospital between 2003 and 2009. The patient selection for the study was based on the degree of disfigurement and facial function parameter rating using the Toronto Facial Grading System. The procedures used were surgeries done in two stages. All cases were followed up

Long-term results of radiosurgery for epidermoid tumors are reported. There are 7 cases including 2 males and 5 females, ages ranging from 6 to 46 (mean: 33.3 years). At radiosurgery whole tumor was covered in 4 cases and partially covered in 3 cases in attempting to relieve cranial nerve signs like trigeminal neuralgia and facial spasm. The mean maximum and marginal doses were 25.6 Gy and 14.6 Gy respectively. In the mean follow-up of 52.7 months, all the tumors showed good tumor control without any progression and tumor shrinkage has been confirmed in 2 out of the 7 cases. Symptomatic trigem

BACKGROUND AND OBJECTIVES: Assessing facial nerve injury in hemifacial spasm (HFS) remains clinically challenging; although delayed facial palsy (DFP) frequently resolves without treatment, anticipating its occurrence is important for patient counseling. However, reliable predictors remain incompletely established, and early recognition of corneal complications secondary to facial weakness with timely referral for appropriate ophthalmological care is essential. The aim of this study was to investigate the utility of lateral spread response (LSR) waveform morphology and compound muscle action p

BACKGROUND: Peripheral facial palsy (PFP) exhibits highly variable etiology and clinical course. While some patients recover spontaneously within weeks, others develop persistent sequelae including synkinesis and crocodile tears that substantially impair quality of life. Management strategies differ considerably across clinical centers worldwide, reflecting ongoing uncertainty regarding optimal therapeutic approaches.
METHODS: This narrative review examines PFP pathophysiology, pharmacotherapy, surgical interventions, PRF neuromodulation, and rehabilitation technologies (125). A literature se

Background Microvascular decompression (MVD) is the preferred treatment for refractory trigeminal neuralgia (TN) and hemifacial spasm (HFS). MVD provides long-lasting results for these conditions with a relatively low risk of postoperative complications. However, reoperation rates are reported up to 11%, an unacceptably high rate for an elective procedure. We determined what factors may increase the risk of reoperation among patients undergoing MVD for TN or HFS. Methods Patient data from 2015 to 2020 were obtained from the American College of Surgeons-National Surgical Quality Improvement P

Postoperative neurocritical intensive care unit (NICU) admission of patients who underwent craniotomy for close observation is common practice. In this study, we performed a comparative analysis to determine if there is a real need for NICU admission after microvascular decompression (MVD) for cranial nerve disorders or whether it may be abandoned. The present study evaluates a consecutive series of 236 MVD surgeries performed for treatment of trigeminal neuralgia (213), hemifacial spasm (17), vagoglossopharyngeal neuralgia (2), paroxysmal vertigo (2), and pulsatile tinnitus (2). All patients

OBJECTIVE: The aim of this study was to identify the etiology of hemifacial spasm (HFS) and trigeminal neuralgia (TN) in patients aged 30 years or younger and to examine the efficacy of microvascular decompression (MVD).
PATIENTS AND METHODS: Between 1996 and 2012, 228 HFS and 190 TN patients underwent MVD at Atsuchi Neurosurgical Hospital. Of these, 7 patients were 30 years of age or younger at the time of treatment (HFS: n = 6, TN: n = 1). Assessments were based on their medical history and on magnetic resonance imaging, magnetic resonance angiography, surgical, and follow-up findings.

Botulinum neurotoxin type A (BTX-A) weakens voluntary muscle strength and is an effective therapy for focal dystonia, including cervical dystonia (CD) and benign essential blepharospasm (BEB). It is also known to relieve hemifacial spasm and focal spasticity in children and adults. In addition, BTX-A has been shown to be effective in a wide range of other indications, such as gastrointestinal disorders, hyperhidrosis and cosmetic wrinkle correction (e.g. glabellar frown lines). A new formulation of BTX-A, NT 201 (XEOMIN((R))) has been developed. NT 201 is a formulation of pure BTX-A free of co

OBJECTIVE: Stereotactic radiosurgery is an effective alternative to surgical resection for the majority of patients with vestibular schwannomas (VS). However, after radiosurgery, the imaging characteristics of VSs are variable, and correct interpretation is critical to prevent unnecessary surgery for these patients.
METHODS: A retrospective study of 208 consecutive patients with unilateral VS having radiosurgery between March 1990 and December 2001. Thirty (14%) patients had tumors that enlarged at least 2 mm after radiosurgery. The median follow-up after radiosurgery was 56 months (range 24-
