Clinical outcome after microvascular decompression for trigeminal neuralgia: a systematic review and meta-analysis.

Microvascular decompression (MVD) is considered an effective treatment for trigeminal neuralgia (TN). However, the anatomical and clinical variables associated with a better outcome are not fully examined. The authors performed a systematic review and meta-analysis of the literature investigating the immediate and long-term clinical results of MVD for TN, and the impact of the anatomical features of the neurovascular conflict on the outcome. The systematic search of three databases was performed for studies published between January 1990 and November 2021. PRISMA guidelines were followed. Rand

Direct mechanical stimulation of brainstem modulates cardiac rhythm and repolarization in humans.

Natural mechanical stimulation of the brainstem area by the blood pressure waves propagating in the adjacent arteries plays an important role in the homeostasis of the brainstem centers of cardiovascular control. However, effects of direct mechanical stimulation of this area on the cardiac elecrophysiology have never been studied in humans. In 12 patients (age: 54 +/- 13 years, 5 females) undergoing microvascular decompression, the left (9 patients) or the right (3 patients) side of the ventro-lateral surface of the medulla oblongata was exposed during the surgery, and a mechanical stimulation

Emergent aneurysm clipping without angiography in the moribund patient with intracerebral hemorrhage: the use of infusion computed tomography scans.

The authors report their experience with 25 patients (mean age, 44.3 +/- 12.1 years) with an intracerebral hematoma (ICH) from a ruptured aneurysm who were emergently operated on without angiography. Instead, preoperative high-resolution infusion computed tomography (CT) scans were used to identify the aneurysm causing the hemorrhage. In all patients, the preoperative Glasgow Coma Scale score was < 5 and brain stem compression was evident. ICH was present in the frontal or temporal lobe and was often associated with intraventricular hemorrhage (n = 17) and significant (> 1 cm) midline shift (n

Factors That May Affect Delayed Relief Of Trigeminal Neuralgia After Microneurosurgery And The Long-Term Outcomes Associated With Delayed Relief.

OBJECTIVE: Microvascular decompression (MVD) combined with partial sensory rhizotomy (PSR) with the retrosigmoid approach has become the most effective surgical treatment for trigeminal neuralgia (TN). There is variability in the pain relief processes observed in postoperative patients. The purpose of this study was to investigate delayed relief (DR) and its predictors after MVD and/or PSR for the treatment of TN and study the long-term effects associated with DR. METHODS: Patients with primary TN who underwent MVD and/or PSR by the same surgeon at the China-Japan Friendship Hospital from Mar

Vascular contact with the fifth cranial nerve at the pons in patients with trigeminal neuralgia: detection with 3D FISP imaging.

OBJECTIVE: Vascular contact with the trigeminal nerve at the pons is known to cause trigeminal neuralgia; however, this finding also is present in some asymptomatic subjects. We evaluated the usefulness of high-resolution MR imaging and MR angiography of the posterior fossa to determine the presence or absence of vascular contact with the fifth cranial nerve at the pons in patients with trigeminal neuralgia and in control subjects. SUBJECTS AND METHODS: The trigeminal nerves in 40 symptomatic patients and 114 asymptomatic control subjects were examined for the presence or absence of vascular

The Facial Nerve in Contemporary Surgery: Anatomical Variability, Pathology-Induced Distortion, and Functional Preservation.

Objectives: The facial nerve (FN) possesses one of the most intricate anatomical courses in the head and neck, traversing the brainstem, temporal bone, and parotid gland before terminating within the muscles of facial expression. Owing to its complex anatomy, marked anatomical variability, and frequent distortion by adjacent pathology, preservation of FN integrity remains a fundamental challenge in skull base, otologic, and head and neck surgery. This review aims to provide a comprehensive synthesis of the contemporary literature regarding the clinical anatomy of the FN and to examine how anat

Adverse central nervous system sequelae after selective transforaminal block: the role of corticosteroids.

BACKGROUND CONTEXT: Selective transforaminal epidural injections are frequently employed in the treatment of pain emanating from the spine. Complication rates are typically low and include paresthesia, hematoma, epidural abscess, meningitis, arachnoiditis and inadvertent subdural or subarachnoid injection. Persistent paraplegia after lumbar transforaminal block has been recently reported. Undetected intra-arterial injection has been implicated as a possible cause. PURPOSE: We present a case of massive cerebellar infarction after uneventful selective cervical transforaminal block. Intra-arteri

Granisetron for Postoperative Nausea and Vomiting Prophylaxis in Microvascular Decompression Despite Prominent Non-serotonergic Emetic Pathways.

Background Microvascular decompression (MVD) is associated with a high incidence of postoperative nausea and vomiting (PONV). Unlike in general surgery, where PONV is predominantly serotonin-mediated, MVD additionally activates non-serotonergic emetic mechanisms, including vestibular stimulation and direct brainstem perturbation. Whether selective 5-HT₃ receptor antagonists can effectively prevent PONV in MVD remains unclear. Materials and methods This retrospective, exploratory, quasi-experimental before-after study included 40 consecutive MVD patients at a single institution, divided into a

Bilateral calcaneal insufficiency fractures due to chronic carbamazepine use for trigeminal neuralgia: A case report.

Stress fractures of calcaneus are uncommon cause of heel pain. Stress fractures could be seen in risc groups such as metabolic diseases/medications causing poor bone quality and exposing repetitive microtrauma. Anti-epileptic drug (AED) use is related with poor bone quality and increased fracture risc. Although carbamazepine-induced stress fracture is a well-known entity and there are case reports in other bones such as the femoral neck, bilateral calcaneal insufficiency fractures is an extraordinary location. To the best of our knowledge, this is the first case reporting an insufficiency frac

Prospective and retrospective study of videoconference telemedicine follow-up after elective neurosurgery: results of a pilot program.

Existing literature suggests that use of telemedicine during postoperative appointments can increase access to care and is valued by patients and providers alike. While research examining the clinical equivalency of telemedicine visits for postoperative care has been growing, few studies have reported on telemedicine follow-up after neurosurgery. This study examined if a videoconferencing visit could substitute for an in-person clinic visit for elective neurosurgical cases in the USA. This was a single-center prospective study of patients who underwent elective neurosurgical procedures (aneury

Long-term outcomes of Gamma Knife radiosurgery in treating glossopharyngeal neuralgia.

Glossopharyngeal neuralgia (GPN) is an unusual disorder causing severe, brief pain episodes in the areas supplied by the glossopharyngeal nerve. Initial treatment involves medications like carbamazepine, but if these are ineffective or cause side effects, interventional pain management techniques or surgery may be considered. Gamma Knife radiosurgery is becoming popular in managing GPN due to its lower risk of complications than surgical interventions like microvascular decompression or rhizotomy. In this retrospective case series, we examined the outcomes of Gamma Knife radiosurgery in eight

[On the occasion of my retirement as head of the Neurochirurgische Universitätsklinik Zürich--changing aspects of treatment modality in modern neurosurgery and of neuroscience research. Presentation of our experience and historical backgrounds].

On the occasion of my retirement as head of the Neurochirurgische Universitätsklinik Zürich on 31. May 2007, the changing aspects of neurosurgery in diseases dealt with and in their treatments during the last 70 years (from the foundation of the department by Prof. Krayenbuehl in 1937 successed by Prof. Yaşargil and later on by myself.) were presented. Number of surgical procedures were 1,000 times per year during the interval and the busiest era was that of Prof. Krayenbühl, in which it amounted to as much as 1,500 at the maximum. Lumbar disc surgery was the most frequent one (around 250/year

Mechanism underlying cranial nerve rhizopathy.

The cranial nerve rhizophathy, commonly presented with trigeminal neuralgia (TN) or hemifacial spasm (HFS), is a sort of hyperexcitability disorders with higher incidence in senior Asian. In this paper, a novel hypothesis on the pathogenesis is proposed and with which some clinical phenomena are explained. In those with crowded cerebellopontine angle in anatomy, the cranial nerve root and surrounding vessel are getting closer and closer to each other with aging and finally the neurovascular conflict happens. As the interfacial friction associated with pulse, the nerve incurs demyelination. Sin
