Return to Sport after Disc Herniation: A Comprehensive Review of Conservative and Surgical Treatment Approaches.

Back pain (BP) is a globally prevalent musculoskeletal disorder, affecting up to 85% of individuals at some point in their lives. It significantly affects quality of life and productivity, with a particularly high prevalence in athletes due to the high physical demands of their activity. This review examines the comparative effectiveness of conservative and surgical treatments for patients with disc herniation (DH), focusing on their return-to-sport (RTS) outcomes.A systematic review was conducted of studies evaluating RTS following conservative or surgical treatment of patients with DH. Evolution of the throwing shoulder: why apes don't throw well and how that applies to throwing athletes.

BACKGROUND: Humans have unique characteristics making us the only primate that can throw well while most other primates throw predominately underhand with poor speed and accuracy. The purpose of this study is to illuminate the uniquely human characteristics that allow us to throw so well. When treating an injury such as a labral tear or capsule tear, this study hopes the reader can gain a better understanding of the issues that lead to the tear and those that may determine the success of treatment besides the actual repair. METHODS: In addition to a review of scientific and medical literature

Surgical Management of Osteochondritis Dissecans of the Glenoid via Autologous Bone Marrow Aspirate Concentrate and Extracellular Matrix Graft in an Eighteen-Year-Old College Pitcher.

BACKGROUND: Osteochondritis dissecans (OCD) affects the shoulder in only 0.6% of patients aged 2 to 19 years with disease most commonly in the humeral head. When the glenoid is affected, it is often in male overhead throwing athletes and treated with fixation via autologous osteochondral plugs following bone marrow aspirate. INDICATIONS: The primary indication for surgical management of OCD is failure of conservative management, often with imaging showing disruption of the glenoid subchondral plate. This patient is an 18-year-old male pitcher with over 2 years of chronic, deep-seated shoulder

The development and validation of a functional assessment tool for the upper extremity in the overhead athlete.

BACKGROUND: There are no validated upper extremity instruments designed specifically to evaluate the performance and function of overhead athletes. Current shoulder and elbow scoring systems may not be sensitive to subtle changes in performance in this high-demand population. HYPOTHESIS: The scoring system developed in this study will be valid, reliable, and responsive in the evaluation of overhead athletes. STUDY DESIGN: Cross-sectional study; Level of evidence, 3. METHODS: Based on the results of a pilot questionnaire administered to 21 overhead athletes, a final 10-item questionnaire was

Arthroscopic findings in the overhand throwing athlete: evidence for posterior internal impingement of the rotator cuff.

The purpose of this article is to describe the outcome of an arthroscopic examination and the pathology in symptomatic shoulders of 41 professional overhand throwing athletes. With the arm in the position of the relocation test, 100% of the subjects had either contact between the rotator cuff undersurface and the posterosuperior glenoid rim or osteochondral lesions. Other key findings included undersurface cuff fraying in 93%, posterosuperior labral fraying in 88%, and anterior labral fraying in 36% of the subjects. This study supports the concept of impingement of the posterior cuff undersurf

Skilled throwers use physics to time ball release to the nearest millisecond.

Skilled throwers achieve accuracy in overarm throwing by releasing the ball on the handpath with a timing precision as low as 1 ms. It is generally believed that this remarkable ability results from a precisely timed command from the brain that opens the fingers. Alternatively, precise timing of ball release could result from a backforce from the ball that pushes the fingers open. The objective was to test these hypotheses in skilled throwers. Angular positions of the hand and phalanges of the middle finger were recorded with the search-coil technique. In support of the backforce hypothesis, w

Shoulder Internal and External Rotator Strength Reliability and Validity in Healthy Adolescent Athletes.

BACKGROUND: Handheld dynamometry (HHD) has been recommended in return-to-play (RTP) assessments following shoulder injuries; however, variations in testing methods limits interpretation of results. In contrast, isokinetic dynamometry is considered gold-standard for assessment of strength but is less accessible than HHD. Further assessment is needed to identify which shoulder HHD testing positions best reflect isokinetic dynamometry values and can be used to guide RTP practices in adolescent athletes. # Purpose To compare HHD strength values of shoulder internal rotators (IR) and external rotat

Non-Contact, Mechanical Fatigue-Related ACL Injury Prevention Through Extracellular Matrix Crosslink Preservation: A Narrative Review.

Background: Anterior cruciate ligament (ACL) injuries are increasing in young athletes and many are related to non-contact, spontaneous mechanical fatigue-related ruptures. The objective of this narrative review is to identify and synthesize the anatomical, histological, physiological, and biomechanical basis of extracellular matrix (ECM) factors that contribute to ACL injuries and suggest ways to decrease their occurrence. Methods: The primary investigator searched PubMed, Web of Science, and Google Scholar database titles and abstracts using search phrases with Boolean operators: "anterior c

Graphite shafts reduce forearm muscle activity in golf - a prospective case series of 40 right-handed amateur and professional golfers.

BACKGROUND: Lateral and medial epicondylitis are common overuse injuries in golf affecting both amateur and professional players. Treatment options include medical but also non-medical interventions like optimizing golf equipment. While several studies have investigated the impact of grip size and style as well as the use of a golf glove on forearm muscle activity, the impact of shaft material remains unclear. Thus, aims of this study were (1) to assess the impact of different shaft material on the forearm muscle activity across the 5 phases of the golf swing, (2) to evaluate the effect on the

The Epidemiology of Stress Fractures in Collegiate Student-Athletes, 2004-2005 Through 2013-2014 Academic Years.

CONTEXT: Stress fractures are injuries caused by cumulative, repetitive stress that leads to abnormal bone remodeling. Specific populations, including female athletes and endurance athletes, are at higher risk than the general athletic population. Whereas more than 460 000 individuals participate in collegiate athletics in the United States, no large study has been conducted to determine the incidence of stress fractures in collegiate athletes. OBJECTIVE: To assess the incidence of stress fractures in National Collegiate Athletic Association (NCAA) athletes and investigate rates and patterns

Changes to injury profile (and recommended cricket injury definitions) based on the increased frequency of Twenty20 cricket matches.

This study analyzes injuries occurring prospectively in Australian men's cricket at the state and national levels over 11 seasons (concluding in season 2008-09). In the last four of these seasons, there was more cricket played, with most of the growth being a new form of the game - Twenty20 cricket. Since the introduction of a regular Twenty20 program, injury incidence rates in each form of cricket have been fairly steady. Because of the short match duration, Twenty20 cricket exhibits a high match injury incidence, expressed as injuries per 10,000 hours of play. Expressed as injuries per days

Body dysmorphic disorder: treating an underrecognized disorder.

“Mr. H,” a 33-year-old single white male, presented with preoccupations focused on his “thinning” hair, facial “acne,” and “short” fingers. He began to worry excessively about his appearance at age 15, focusing at that time on his “pale” skin and “uneven” cheekbones. Mr. H described his appearance preoccupations as “severely upsetting,” but he was too embarrassed to reveal them to family or friends. Even though he looked normal to others, Mr. H was “100% convinced” that these body areas appeared “abnormal and deformed,” although in the past he had sometimes thought that “maybe I don't look so
