Timing of seasonal influenza epidemics for 25 countries in Africa during 2010-19: a retrospective analysis.

BACKGROUND: Using country-specific surveillance data to describe influenza epidemic activity could inform decisions on the timing of influenza vaccination. We analysed surveillance data from African countries to characterise the timing of seasonal influenza epidemics to inform national vaccination strategies. METHODS: We used publicly available sentinel data from African countries reporting to the WHO Global Influenza Surveillance and Response FluNet platform that had 3-10 years of data collected during 2010-19. We calculated a 3-week moving proportion of samples positive for influenza virus

Novel FujiLAM assay to detect tuberculosis in HIV-positive ambulatory patients in four African countries: a diagnostic accuracy study.

BACKGROUND: Development of rapid biomarker-based tests that can diagnose tuberculosis using non-sputum samples is a priority for tuberculosis control. We aimed to compare the diagnostic accuracy of the novel Fujifilm SILVAMP TB LAM (FujiLAM) assay with the WHO-recommended Alere Determine TB-LAM Ag test (AlereLAM) using urine samples from HIV-positive patients. METHODS: We did a diagnostic accuracy study at five outpatient public health facilities in Uganda, Kenya, Mozambique, and South Africa. Eligible patients were ambulatory HIV-positive individuals (aged ≥15 years) with symptoms of tubercu

Genetic variability in HPV 33 and 35 E6 and E7 genes from South African and Mozambican women with different cervical cytology status.

BACKGROUND: Among the high-risk human Papillomavirus (hr-HPV) genotypes related to cervical cancer (CC) cases, HPV16 and 18 are the most studied worldwide. However, several studies have identified HPV 33 and HPV 35 as some of the most common genotypes in sub-Saharan African regions. This study aims to investigate the genetic variability and lineages of HPV 33 and 35 based on the HPV E6 and E7 genes in isolates from South African and Mozambican women with different cervical cytology statuses. METHODS: The study analysed 26 HPV 33 and 46 HPV 35 DNA samples collected previously from South Africa

Materno-fetal transmission of pregnancy malaria: an immunoparasitological study on 202 parturients in Maputo.

A total of 202 delivering mothers and their newborns were studied with the intention to follow the materno-fetal transmission of malaria. Malaria infection was encountered in 35 cases (17.3%) in which Plasmodium falciparum predominated in peripheral blood while P. malariae predominated on the placental surface. In cord blood P. falciparum was encountered in 1.5% of the cases. There was slightly more maternal infection in rural (23%) than in suburban areas (19%). Less malaria infection was encountered in primiparas than in multiparas and there was similar antibody response in both mothers and t

Sex-related inequalities in current cigarette smoking among adolescents in Africa.

INTRODUCTION: Risky behaviours, including tobacco use, are highly prevalent among adolescents worldwide. Although these behaviours are largely influenced by various sociodemographic factors, including sex, there is a paucity of regionally representative literature on the sex-related inequalities in cigarette smoking among adolescents in Africa. This study examined the sex-based disparities in current cigarette smoking among adolescents aged 13-15 years in Africa. METHODS: The present study employed a secondary analysis of nationally representative data on 45 African countries obtained from th

Child deaths caused by Klebsiella pneumoniae in sub-Saharan Africa and south Asia: a secondary analysis of Child Health and Mortality Prevention Surveillance (CHAMPS) data.

BACKGROUND: Klebsiella pneumoniae is an important cause of nosocomial and community-acquired pneumonia and sepsis in children, and antibiotic-resistant K pneumoniae is a growing public health threat. We aimed to characterise child mortality associated with this pathogen in seven high-mortality settings. METHODS: We analysed Child Health and Mortality Prevention Surveillance (CHAMPS) data on the causes of deaths in children younger than 5 years and stillbirths in sites located in seven countries across sub-Saharan Africa (Ethiopia, Kenya, Mali, Mozambique, Sierra Leone, and South Africa) and s

Gastro-duodenal disease in Africa: Literature review and clinical data from Accra, Ghana.

Gastroduodenal disease (GDD) was initially thought to be uncommon in Africa. Amongst others, lack of access to optimal health infrastructure and suspicion of conventional medicine resulted in the reported prevalence of GDD being significantly lower than that in other areas of the world. Following the increasing availability of flexible upper gastro-intestinal endoscopy, it has now become apparent that GDD, especially peptic ulcer disease (PUD), is prevalent across the continent of Africa. Recognised risk factors for gastric cancer (GCA) include Helicobater pylori (H. pylori), diet, Epstein-Bar

Spatial distribution and factors associated with co-occurrence of anemia and undernutrition among children aged 6-59 months in East Africa.

BACKGROUND: Anemia and undernutrition are major public health concerns affecting child growth, development, and survival, particularly in low-resource settings like East Africa. These conditions often occur together and exacerbate the health and developmental challenges of young children. Despite their high prevalence, no research has explored their co-occurrence and the spatial variations across East African countries. Hence, this study aimed to analyze the spatial distribution and identify predictors of the co-existence of anemia and under-nutrition among children aged 6-59 months in East Af

Post-mortem characterisation of pathogen-specific causes of infection-related deaths in African and south Asian neonates: a prospective, observational, multicentre study.

BACKGROUND: Post-mortem biological investigation of causes of deaths is uncommon in low-income and middle-income countries (LMICs). The primary objective of Child Health and Mortality Prevention Surveillance (CHAMPS) is to determine the underlying and contributing causes of death for stillbirths and children younger than 5 years, including identification of specific pathogens and risk factors associated with mortality. We report on the role of infections, including pathogen-specific causes, in the overall causal pathway to death in neonates (aged <28 days) across CHAMPS sites in Ethiopia, Keny

Respiratory syncytial virus infection among children younger than 2 years admitted to a paediatric intensive care unit with extended severe acute respiratory infection in ten Gavi-eligible countries: the RSV GOLD-ICU Network study.

BACKGROUND: Patient-level data on life-threatening respiratory syncytial virus (RSV) infection in children in low-income and lower-middle-income countries (LMICs) are scarce, and this scarcity might limit demand for RSV interventions in LMICs who rely on support from Gavi, the Vaccine Alliance. We aimed to describe the characteristics of RSV-positive children younger than 2 years who were admitted to paediatric intensive care units (PICUs) with extended severe acute respiratory infection (eSARI) in Gavi-eligible countries. METHODS: The RSV GOLD-ICU Network study is a 2-year prospective, multi

Global, regional, and national estimates of the impact of a maternal Klebsiella pneumoniae vaccine: A Bayesian modeling analysis.

BACKGROUND: Despite significant global progress in reducing neonatal mortality, bacterial sepsis remains a major cause of neonatal deaths. Klebsiella pneumoniae (K. pneumoniae) is the leading pathogen globally underlying cases of neonatal sepsis and is frequently resistant to antibiotic treatment regimens recommended by the World Health Organization (WHO), including first-line therapy with ampicillin and gentamicin, second-line therapy with amikacin and ceftazidime, and meropenem. Maternal vaccination to prevent neonatal infection could reduce the burden of K. pneumoniae neonatal sepsis in low

Phylogenetic Diversity of Vibrio cholerae Associated with Endemic Cholera in Mexico from 1991 to 2008.

UNLABELLED: An outbreak of cholera occurred in 1991 in Mexico, where it had not been reported for more than a century and is now endemic. Vibrio cholerae O1 prototype El Tor and classical strains coexist with altered El Tor strains (1991 to 1997). Nontoxigenic (CTX(-)) V. cholerae El Tor dominated toxigenic (CTX(+)) strains (2001 to 2003), but V. cholerae CTX(+) variant El Tor was isolated during 2004 to 2008, outcompeting CTX(-) V. cholerae. Genomes of six Mexican V. cholerae O1 strains isolated during 1991 to 2008 were sequenced and compared with both contemporary and archived strains of V.

Concurrent coverage and determinants of vitamin A supplementation and deworming among children aged 12-59 months in 15 Sub-Saharan African countries.

BACKGROUND: Vitamin A supplementation (VAS) and deworming (DW) are proven, cost-effective interventions that protect children against preventable morbidity, mortality, and the burden of micronutrient deficiencies and parasitic infections. Nevertheless, many children fail to receive both interventions simultaneously, limiting the potential health gains. Assessing co-coverage and its determinants is crucial for guiding integrated child health strategies and closing persistent gaps across Sub-Saharan Africa. METHODS: We analyzed DHS data from 15 Sub-Saharan African countries, including 107,725 c

Cholera outbreaks in Africa.

During the current seventh cholera pandemic, Africa bore the major brunt of global disease burden. More than 40 years after its resurgence in Africa in 1970, cholera remains a grave public health problem, characterized by large disease burden, frequent outbreaks, persistent endemicity, and high CFRs, particularly in the region of the central African Great Lakes which might act as reservoirs for cholera. There, cases occur year round with a rise in incidence during the rainy season. Elsewhere in sub-Saharan Africa, cholera occurs mostly in outbreaks of varying size with a constant threat of wid
