Cartilage lesions in patellofemoral dislocations: incidents/locations/when to treat.

Patellofemoral (PF) dislocations are frequently associated with chondral injury. Chondral and osteochondral lesions are often associated with traumatic (high-energy) PF dislocations, whereas atraumatic (low-energy) PF dislocations in patients with significant PF risk factors have a much lower incidence of osteochondral damage. This article provides a historical overview and delineates the current state of radiographic and clinical outcomes of osteochondral lesions after PF dislocation. The importance of understanding risk factors of redislocation is emphasized, and the current treatment option

[Two siblings with bilateral congenital knee dislocations: case report].

Congenital dislocation of the knee and congenital permanent dislocation of the patella are rare disorders usually associated with complex syndromes. Two cases of siblings, girl and boy, who had the same clinical phenotype of this disorder are presented. The diagnosis of Desbuquois dysplasia was made and its autosomal recessive heredity was confirmed by genetic analysis. DNA samples were sent for a molecular genetic analysis of the skeletal dysplasia. The girl was surgically treated for a complete (grade 3) anterior dislocation of the tibia on the femur in the first year of life. Redressing cas

Graft choice for isolated MPFL reconstruction: gracilis versus semitendinosus.

INTRODUCTION: After the first patellar dislocation, most patients report damage of the medio-patellofemoral ligament (MPFL) and surgical reconstruction is required. The purpose of this study is to systematically review current evidence and to clarify the role of the gracilis and semitendinosus tendons as graft for isolated MPFL reconstruction. MATERIALS AND METHODS: The present systematic review was conducted according to the PRISMA guidelines. The literature search was conducted in October 2019. All clinical trials using the semitendinosus and/or gracilis tendon grafts for isolated MPFL reco

UCI total knee replacement. A follow-up study.

Using the UCI (University of California at Irvine) total knee prosthesis, an arthroplasty was performed in ninety-seven patients (121 knees) from 1972 through 1977. I examined eighty of these patients (100 knees) at three to eight years after the operation and it was necessary to either perform or recommend further surgery in twenty-five of them (twenty-seven knees). These results were designated as failures. The knees in valgus angulation that failed typically did so within the first year because of medical instability and patellar dislocation. The knees in varus angulation that failed typica

Clinical outcomes of medial patellofemoral ligament reconstruction in patients with an increased tibial tuberosity-trochlear groove distance.

PURPOSE: Medial patellofemoral ligament (MPFL) reconstruction is performed to treat recurrent patellar dislocation (RPD). However, the effectiveness of MPFL reconstruction in patients with a severely lateralised tibial tuberosity remains unknown. In this study, the clinical outcomes of MPFL reconstruction in patients with an increased tibial tuberosity-trochlear groove (TT-TG) distance were examined. METHODS: A total of thirty-four patients who underwent MPFL reconstruction for RPD were retrospectively examined. Nineteen patients with a TT-TG distance of >20 mm (increased TT-TG distance group

Gracilis tendon transfer associated with distal alignment for patella alta with recurrent dislocations: an original surgical technique.

Many surgical techniques for the medial patellofemoral ligament have recently been suggested, all of which included problems identifying the femoral anchorage point and determining the proper extent of knee flexion for the transplant. P. Burdin proposed a different and original approach consisting in performing a gracilis muscle transfer to the medial edge of the patella, thus obtaining progressive tension of the transfer during knee flexion by means of the myotatic reflex. We report the results herein. We retrospectively assessed 17 knees treated for patellofemoral instability using this tech

Clinical results of isolated reconstruction of the medial patellofemoral ligament for recurrent dislocation and subluxation of the patella.

Complications and results of medial patellofemoral ligament (MPFL) reconstruction, performed between 2000 and 2003, were studied retrospectively in 24 randomised patients (24 knees). All procedures were performed for recurrent dislocation or subluxation of the patella, using an autogenous semitendinosus tendon. Two different methods of anchoring of the transplant were compared. After a 2-year follow-up, patellar stability was found to be similar, when the transplant was sutured to the fibrous tissue and periosteum overlying the patella or when it was fixed in a bone tunnel through the patella.

MRI of Hoffa's fat pad.

The infrapatellar fat pad of Hoffa is commonly injured but rarely discussed in the radiological literature. Abnormalities within it most commonly are the consequences of trauma and degeneration, but inflammatory and neoplastic diseases of the synovium can be confined to the fat pad. The commonest traumatic lesions follow arthroscopy, but intrinsic signal abnormalities can also be due to posterior and superior impingements syndromes and following patellar dislocation. Infrapatellar plica syndrome may also be traumatic in aetiology. The precise aetiology of ganglion cysts is not understood; the

Vibration arthrometry. A preview.

Throughout the ages, physicians have listened to sounds and felt vibrations from human joints in their attempts to diagnose pathology. They have used a wide vocabulary to describe these phenomena, but technology has been slow to provide recording and analytic equipment. Lately, accelerometers have been used with considerable success in a new noninvasive method now known as vibration arthrometry (formerly "arthrography"). The technique has been used in early detection of congenital dislocation of the hip and also in diagnosis of meniscal pathology. More recently, patellar vibration has been use

Patellofemoral disorders: physical and radiographic evaluation. Part II: Radiographic examination.

Evaluation of the patient with patellofemoral complaints requires a comprehensive physical and radiologic investigation. The tangential view is most helpful in the evaluation of such patellofemoral disorders as chondromalacia, lateral patellar compression syndrome, and recurrent subluxation or dislocation. Commonly utilized tangential views include those described by Hughston, Merchant, Laurin, and Ficat. Multiple calculations available for quantitation of abnormal tangential views include the sulcus angle, congruence angle, lateral patellofemoral angle, patella index, and patellofemoral index

Iliotibial band traction syndrome in guided motion TKA. A new clinical entity after TKA.

This study aimed at systematic documentation of lateral knee pain in a consecutive series of 1102 cruciate-substituting, guided motion total knee arthroplasties (TKA) (Journey, Smith and Nephew, Memphis, TN, USA) performed in 1085 patients; 1070 knees were available for review. Follow-up time ranged from one to five years, with a mean of 2.5 years. Symptoms mimicking the well known iliotibial band (ITB) friction syndrome were observed in 77 knees (7.2%). Initial conservative treatment consisted of anti-inflammatory medication (77 knees) and local steroid injection (35 knees). The pain persiste

Osteochondral fracture of the patella without soft tissue injury and with no dislocation: A case report.

INTRODUCTION: Osteochondral fracture of the patella is a fairly common pathology, but almost always associated with a spectrum of soft tissue injuries including anterior cruciate ligament (ACL) rupture. We present a rare case of an osteochondral fracture of the patella in the absence of ligament or soft tissue injuries and with no dislocation of the patella in a pediatric patient. PRESENTATION OF CASE: An 11-year-old male presented to the orthopedic clinic on crutches following a football injury. The patient had pain in his left knee with flexion deformity. Plain film radiography of the left

Tibial Tubercle Osteotomy With Medial Patellofemoral Ligament Reconstruction Utilizing Autograft Hamstring Tendon and Knotless Suture Anchor Fixation.

BACKGROUND: Recurrent patellar instability is defined as ≥2 dislocation events or recurrence after surgical treatment. Surgical intervention is aimed at correcting pathoanatomic abnormalities. A tibial tubercle osteotomy (TTO) is an intervention that corrects patients with high tibial tuberosity-trochlear groove (TT-TG) distances and/or patella alta as measured by increased Caton-Deschamps index (CDI). Anteromedialization osteotomies improve abnormal TT-TG distances and offload the patellofemoral joint in patients with chondral lesions. Distalization can be utilized to correct patella alta and

Medial Patellofemoral Ligament Reconstruction Improves Patella Alta.

BACKGROUND: Patellofemoral instability (PFI) is a common problem with various anatomic risk factors identified, including patella alta as one of the most powerful predictors. Given that the medial patellofemoral ligament (MPFL) is the primary soft tissue stabilizer of the patella, a common procedure to treat recurrent patellar instability is MPFL reconstruction. The aim of this study is to examine the relationship between MPFL reconstruction and patellar height. We hypothesize that after reconstruction, patellar instability patients would have demonstrable reduction in patella height and patel

Patellar Instability after Total Knee Arthroplasty: A Surgical Case Report.

INTRODUCTION: Patella instability and maltracking is a serious complication following total knee replacement that can lead to poor outcomes for the patient and contribute to early failure. The incidence of patella maltracking ranges from 1% to 20% after total knee arthroplasty (TKA), with post-operative anterior knee pain being an important indicator. There are many different surgical approaches for the management of post-operative patella maltracking with very good outcomes. However, the most crucial step is to identify the underlying etiology.
