Associations between parental alcoholism and delinquent behaviours among youth: exploring serial and parallel mediation by posttraumatic stress disorder symptoms, anxiety symptoms and impulsivity.

Background: Despite mounting evidence linking parental alcoholism (PA) with delinquency in youth, little is known about the factors that mediate this link globally. This study, therefore, aimed to explore whether Posttraumatic Stress Disorder (PTSD) symptoms, Generalized Anxiety Disorder (GAD) symptoms and impulsivity serially or parallel mediate the associations between PA and delinquency among youth.Method: A random sample of 341 male participants aged between 17 and 25 years (M = 21.34, SD = 2.23) was selected from the Iwawa Rehabilitation Centre for youth affected by delinquency, alcohol o

Estimating the effect of hormonal contraceptive use on anemia: A cross-sectional comparative analysis of 46 Demographic and Health Surveys.

BACKGROUND: Anemia is a significant health concern for women in low- and middle-income countries. Hormonal contraceptives, which can reduce heavy menstrual bleeding and prevent pregnancy and its resulting blood loss, are an underexplored opportunity for anemia reduction. METHODS: Using Demographic and Health Survey data from 46 countries across four global regions, we examined country prevalence of anemia (i.e., hemoglobin below 12 g/dl via HemoCue testing) among women aged 15-49 years who were not pregnant. For each country, we used multivariable logistic regression to compare odds of anemia

Mother and newborn skin to skin contact in East Africa: Prevalence and predictors (2019-2023).

Skin-to-skin contact (SSC) between newborns and mothers is crucial for reducing hypothermia complications and enhancing mother-baby bonding. This study aimed to examine the prevalence rates and risk factors for SSC in five East African countries. Data from the most recent Demographic and Health Surveys covering five East African countries from 2019 to 2023 were utilized for secondary data analysis. A total of 37,140 newborns were included in the weighted sample sizes. The factors contributing to SSC prevalence were assessed using a multilevel mixed-effects logistic regression model, with signi

Future climate and demographic changes will almost double the risk of schistosomiasis transmission in the Lake Victoria Basin.

BACKGROUND: The Lake Victoria Basin (LVB), supporting millions of people across Uganda, Kenya, Tanzania, Rwanda, and Burundi, is a critical freshwater ecosystem. However, it faces significant One Health challenges, notably urogenital and intestinal schistosomiasis, diseases transmitted by Bulinus and Biomphalaria snails, respectively. Climate, topography, environmental and demographic drivers influence snail habitat suitability and distribution, potentially increasing schistosomiasis risks for communities dependent on agriculture, fishing, and water-related livelihoods. METHODS: This study ap

Health promoting palliative care interventions in African low-income countries: a scoping review.

BACKGROUND: Palliative care (PC) has advanced rapidly since 2005, when the World Health Assembly posited it as an urgent humanitarian need. Over the same period, the principles of the Ottawa Charter for Health Promotion have been promoted to implement sustainable PC. It is not known whether the Ottawa Charter principles have been integrated into existing PC efforts in low-income African countries. The purpose of this scoping review is to determine the state of knowledge on the consequences of health promoting PC (HPPC) interventions in African low-income countries. METHODS: We searched for li

Feasibility and acceptability of frequent vaginal self-sampling at home by Rwandan women at high risk of urogenital tract infections.

OBJECTIVES: To establish temporal links between vaginal microbiota (VMB) data and incident clinical events, frequent longitudinal vaginal sampling is required. Self-collection of swabs at the participant's home may be useful to avoid overburdening research clinics and participants. One-off vaginal self-sampling for STI or cervical cancer screening programmes has been shown to be feasible and acceptable to women in multiple studies, including in sub-Saharan Africa, but the feasibility and acceptability of frequent longitudinal vaginal sampling in the context of VMB sequencing studies is unknown

Child vaccination in sub-Saharan Africa: Increasing coverage addresses inequalities.

BACKGROUND: Vaccines have substantially contributed to reducing morbidity and mortality among children, but inequality in coverage continues to persist. In this study, we aimed to examine inequalities in child vaccination coverage in sub-Saharan Africa. METHODS: We analysed Demographic and Health Survey data in 25 sub-Saharan African countries. We defined full vaccination coverage as a child who received one dose of bacille Calmette-Guérin vaccine (BCG), three doses of diphtheria, pertussis, and tetanus vaccine (DTP 3), three oral polio vaccine doses (OPV 3), and one dose of measles vaccine.

Cross-Sectional Analysis of Selected Genital Tract Immunological Markers and Molecular Vaginal Microbiota in Sub-Saharan African Women, with Relevance to HIV Risk and Prevention.

Data on immune mediators in the genital tract and the factors that modulate them in sub-Saharan women are limited. Cervicovaginal lavage (CVL) samples from 430 sexually active women from Kenya, South Africa, and Rwanda were analyzed for 12 soluble immune mediators using Bio-Plex and Meso Scale Discovery multiplex platforms, as well as single enzyme-linked immunosorbent assays. Ten bacterial species were quantified in vaginal swab samples. Bacterial vaginosis (BV) was defined by Nugent scoring. CVL samples from HIV-infected women showed a clear-cut proinflammatory profile. Pregnant women, adole

Twelve-Month Antiretroviral Therapy Suppresses Plasma and Genital Viral Loads but Fails to Alter Genital Levels of Cytokines, in a Cohort of HIV-Infected Rwandan Women.

BACKGROUND: Genital viral load (GVL) is the main determinant of sexual transmission of human immune-deficiency virus (HIV). The effect of antiretroviral therapy (ART) on local cervico-vaginal immunological factors associated with GVL is poorly described. We aimed to identify the risk factors of detectable GVL, and the impact of ART on HIV genital shedding and its correlates in a cohort of HIV-infected women, attending HIV care in Kigali, Rwanda. MATERIALS AND METHODS: All participants were evaluated for GVL, plasma viral load (PVL), CD4 count, various sexually-transmitted infections (STIs) at

The role of genetic polymorphisms in cytochrome P450 and effects of tuberculosis co-treatment on the predictive value of CYP2B6 SNPs and on efavirenz plasma levels in adult HIV patients.

Efavirenz (EFV) exhibits interindividual pharmacokinetic variability caused by differences in cytochrome P450 (CYP) expression. Most tuberculosis (TB) drugs interact with the CYP metabolizing enzymes, while the clinical validity of genotyping in predicting EFV plasma levels in Rwandan subjects is not known. We investigated in patients co-infected with human immunodeficiency virus (HIV) and TB recruited in Rwanda the effects of 10 SNPs in five drug-metabolizing enzymes on EFV plasma levels and treatment response when patients are treated with EFV-containing therapy alone (n=28) and when combine

Assessment of the performance of primary healthcare in eight African countries: based on the Composite Fuzzy Index.

BACKGROUND: African countries face a dual burden of infectious and non-communicable diseases, placing substantial pressure on primary healthcare (PHC) systems. Existing indicator-weighting approaches are limited in addressing redundancy and non-linear relationships. In addition, conventional composite indices often rely on linear aggregation assumptions that may not adequately reflect complex relationships among PHC performance indicators. This study proposes a novel weighting method and composite index to improve facility-level assessment of PHC performance. METHODS: We analysed nationally r

Analysing government health expenditure in Africa: Trends, determinants and policy options.

BACKGROUND: In April 2001, African Union (AU) Heads of State adopted the Abuja Declaration, pledging to allocate at least 15% of government expenditure to health. More than two decades later, many African health systems remain underfunded, with external aid increasingly unsustainable as official development assistance declines. AIM: This article examines progress towards the Abuja target, the role of political economy in shaping government health expenditure (GHE), and policy options for sustainable domestic financing. SETTING: The authors examined the Global Health Expenditure across 53 AU

National and Sub-National Delivery of Balanced Energy and Protein (BEP) Supplements to Pregnant and Lactating Women in LMICs: Lessons from Multi-Country Implementation Case Studies.

UNLABELLED: The World Health Organization recommends the use of balanced energy protein (BEP) supplements during pregnancy in settings with a ≥ 20% prevalence of underweight women of reproductive age to reduce the risk of adverse health outcomes. Several countries are implementing BEP supplementation in varied formats. However, the implementation and monitoring of outcomes remain poor across countries. This qualitative study explores the experiences, opportunities, and challenges related to implementing national and sub-national BEP supplementation programs in nine countries (12 countries orig

Geospatial variation of bottle feeding practice among children aged 0-23 months in East Africa: Geographically weighted regression analysis.

INTRODUCTION: Bottle-feeding a baby aged 0-23 months is the act of giving any liquid or semi-solid food from a bottle with a nipple. Bottle feeding can increase their risk of excessive weight gain, malnutrition, iron deficiency, and reduced birth spacing. However, no study has been conducted on geographically weighted regression (GWR) analysis and the factors associated with bottle-feeding practice. OBJECTIVE: To assess the geospatial variation and determinants of bottle-feeding practice among children aged 0-23 months in East Africa using GWR. METHODS: A total sample of 28,562 children aged

Allergic reactions associated with tsetse fly bites.

BACKGROUND: Allergic reactions in response to arthropod bites are a growing concern worldwide. Alpha-Gal syndrome (AGS) is a red meat allergy associated with IgE antibodies against galactose-alpha-1,3-galactose (alpha-Gal), typically linked to tick bites but other arthropods may also play a role. The objective of this study was to investigate the potential association between tsetse fly (Glossina spp.) bites and AGS by analyzing clinical cases, citizen science data, and the possible production of alpha-Gal by tsetse flies. METHODS: Data from five case series involving tsetse fly bites include
