Airway Management in the Offshore Rescue Helicopter: Evaluation of Tracheal Intubation in Alternative Positions.

OBJECTIVE: This study focuses on airway management in offshore rescue helicopters, where patients often require initial and prolonged medical care during long flights. Here, the patient's atypical, sideway position in the helicopter cabin prevents intubation in the standard way. The aim of this study is therefore to investigate optimal strategies for airway management from the side. The results can also be transferred to other emergency situations with atypical patient position and confined spaces. METHODS: In a randomized trial, 440 intubations were performed on a manikin in 7 different sett

An exploratory study of volatile and semi-volatile organic compounds in PM2.5 atmospheric particles from an outdoor environment in Brazil.

The development of methods for determining volatile and semi-volatile organic compounds in public spaces has become necessary to identify potential health and environmental risks. This study presents a practical methodology for sampling, extracting, detecting, and identifying these compounds in a vehicular traffic region in Belo Horizonte, Brazil. The methodology uses direct-immersion solid phase microextraction (DI-SPME) and static headspace (SHS) to extract SVOCs/VOCs. Comprehensive time-of-flight gas chromatography mass spectrometry (GC×GC/Q-TOFMS) and gas chromatography coupled to mass spe

Carcinogens in spacecraft air.

The health effects of long-term exposure to ionizing radiation during spaceflight are a major concern to NASA, especially for missions beyond low-Earth orbit. Experiments involving astronauts will be conducted during the next few years to improve the risk assessment for exposures to ionizing radiation during flights; however, concomitant exposure to certain airborne chemical carcinogens during these experiments could confound the results of radiation experiments. Carcinogens can reach the spacecraft atmosphere from leaking thrusters, from off-gassing materials, from chemical experiments and fr

Expression of hypoxia-inducible genes is suppressed in altered gravity due to impaired nuclear HIF1α accumulation.

Extravehicular activities, the backbone of manned space exploration programs, set astronauts into mild hypoxia. Unfortunately, microgravity aggravates threatening symptoms of hypoxia such as vision impairment and brain edema. Hypoxia-inducible factors (HIFs) sense cellular hypoxia and, subsequently, change the cells' expression profile instantaneously by rapidly translocating-most likely cytoskeleton-dependently-into the nucleus and subsequently forming transcription complexes with other proteins. We tested the hypothesis that this fundamental process could be altered by sudden changes in grav

Time-resolved contrast-enhanced magnetic resonance angiography in the investigation of suspected intracranial dural arteriovenous fistula.

Cerebral angiography is widely regarded as the gold standard for the evaluation and diagnosis of neurovascular abnormalities. However, recent improvements in the spatial and temporal resolution of time-resolved magnetic resonance angiography (MRA) offer clinicians a non-invasive alternative to cerebral angiography. We explored the utility of this technique in an elderly female patient with a suspected intracranial dural arteriovenous fistula (dAVF). A product pulse sequence available from the scanner's manufacturer (time-resolved imaging of contrast kinetics, TRICKS; GE Healthcare, Milwaukee,

Simulating human space physiology with bed rest.

In a recent review on bed rest studies of the past 20 years, it was concluded that head-down bed rest has proved its usefulness as a reliable simulation model for most physiological effects of spaceflight. Much of this research has been conducted to find countermeasures against the negative effects, which are associated with gravitational unloading. There have been partial successes in the prevention of, for example muscle wasting, cardio-vascular deconditioning, adverse metabolic changes, and bone demineralization. Reviews refer to bone-related measurements of the U.S. and Russian space progr

Vestibulo-spinal response modification as determined with the H-reflex during the Spacelab-1 flight.

Our laboratory at the Johnson Space Center has employed the H-reflex recorded from the soleus muscle as a method of monosynaptic reflex testing in conjunction with vertical linear acceleration to assess modification of utriculo-saccular function induced through prolonged exposure to microgravity. It was hypothesized that exposure to free fall would reduce the necessity for postural reflexes in the major leg muscles, and that postural modification would reflect a change, not in the peripheral vestibular organs, but more centrally. This postural adjustment would reflect a sensory motor rearrange

Is spaceflight-induced immune dysfunction linked to systemic changes in metabolism?

The Space Shuttle Atlantis launched on its final mission (STS-135) on July 8, 2011. After just under 13 days, the shuttle landed safely at Kennedy Space Center (KSC) for the last time. Female C57BL/6J mice flew as part of the Commercial Biomedical Testing Module-3 (CBTM-3) payload. Ground controls were maintained at the KSC facility. Subsets of these mice were made available to investigators as part of NASA's Bio-specimen Sharing Program (BSP). Our group characterized cell phenotype distributions and phagocytic function in the spleen, catecholamine and corticosterone levels in the adrenal glan

Centrifugation protocol for the NASA Artificial Gravity-Bed Rest Pilot Study.

We have implemented a 41-day ground-based study to investigate the effects of daily artificial gravity loading on bed rest deconditioned human subjects. Each subject underwent 21 days of 6 degree head-down bed rest. Treatment subjects received 60 min daily doses of inertial mechanical loading (2.5 G at the feet decreasing to 1 G at the heart) produced by a short radius centrifuge. During rotation, the subject's cardiovascular responses were monitored via ECG, blood pressure and pulse oximetry, and subjective assessment of motion sickness and overall health were periodically requested. Semantic-geography integrated spatiotemporal safety mapping for urban low-altitude airspace environments.

Unmanned aerial vehicle (UAV) safety is becoming increasingly critical as UAV operations are integrated into urban service and transportation systems, creating an urgent need for risk representations that can support refined low-altitude airspace management. Existing urban safety maps are still mainly expressed as two-dimensional or height stratified representations, which cannot capture the continuous variation of risk with altitude and therefore remain inadequate for continuous low-altitude flight scenarios. This paper develops a continuous three-dimensional spatiotemporal safety mapping fra

An explicit analytical model for passive twist and systematic optimization of membrane flapping wings.

The hovering performance of membrane flapping-wing micro air vehicles (FWMAVs) is governed by numerous coupled parameters, making rapid forward optimization difficult via traditional empirical trials or localized fluid-structure interaction (FSI) analysis. This paper proposes a systematic design framework based on a decoupled parametric configuration and an explicit twist formula. First, a parametric wing design method is developed, in which the wing planform and dominant vein constraints are reduced to a small set of geometric variables, thereby simplifying the design space while preserving t

Astrochemistry prefers the biomolecule: isomer-selective production of ethanolamine (HOCH2CH2NH2) in interstellar model ices.

A crucial molecule in the chemistry of life, ethanolamine (HOCH2CH2NH2) is among the most prevalent biomolecules. It serves as a ubiquitous hydrophilic head group for the molecules comprising cellular membranes and it functions as both precursor and intermediate in the biological synthesis of neurotransmitters among other metabolites, protein folding, and post-translational modification. Because of the out-sized biological importance of this molecule ethanolamine is expected to serve as a signature for prebiotic chemistry in space, highlighting the importance of its recent discovery. Here, we

Establishing Sterility Assurance for Bacillus canaveralius 29669 Spores Under High Heat Exposure.

The ever-increasing complexity in critical spacecraft hardware and materials has led to the development of new microbial reduction procedures as well as to changes in established processes such as heat microbial reduction (HMR). In the space biology field of Planetary Protection, 500°C for 0.5 s is the current HMR recommendation to reduce microorganisms from flight hardware. However, more studies are needed to effectively determine the microbial reduction capability of high-temperature (more than 200°C), short-duration (under 30 s) heat exposures. One of the many recent microbial reduction bio

Discrimination and Identification of Aroma Profiles and Characterized Odorants in Citrus Blend Black Tea with Different Citrus Species.

Citrus blend black teas are popular worldwide, due to its unique flavor and remarkable health benefits. However, the aroma characteristics, aroma profiles and key odorants of it remain to be distinguished and cognized. In this study, the aroma profiles of 12 representative samples with three different cultivars including citrus (Citrus reticulata), bergamot (Citrus bergamia), and lemon (Citrus limon) were determined by a novel approach combined head space-solid phase microextraction (HS-SPME) with comprehensive two-dimensional gas chromatography-time-of-flight mass spectrometry (GC×GC-TOFMS).

Klotho-deficient mice are resistant to bone loss induced by unloading due to sciatic neurectomy.

Unloading induces bone loss as seen in experimental animals as well as in space flight or in bed-ridden conditions; however, the mechanisms involved in this phenomenon are not fully understood. Klotho mutant mice exhibit osteopetrosis in the metaphyseal regions indicating that the klotho gene product is involved in the regulation of bone metabolism. To examine whether the klotho gene product is involved in the unloading-induced bone loss, the response of the osteopetrotic cancellous bones in these mice was investigated. Sciatic nerve resection was conducted using klotho mutant (kl/kl) and cont

Intracranial Fluid Redistribution But No White Matter Microstructural Changes During a Spaceflight Analog.

The neural correlates of spaceflight-induced sensorimotor impairments are unknown. Head down-tilt bed rest (HDBR) serves as a microgravity analog because it mimics the headward fluid shift and axial body unloading of spaceflight. We investigated focal brain white matter (WM) changes and fluid shifts during 70 days of 6° HDBR in 16 subjects who were assessed pre (2x), during (3x), and post-HDBR (2x). Changes over time were compared to those in control subjects (n = 12) assessed four times over 90 days. Diffusion MRI was used to assess WM microstructure and fluid shifts. Free-Water Imaging was u

Space sickness and fluid shifts: a hypothesis.

In a sample of 64 first-time Space Shuttle crew members, 9 preflight variables related to fluid, electrolyte, and cardiovascular status were previously found to be significantly related to space sickness. The nine variables are serum uric acid, red cell count, environmental temperature at the launch site, serum phosphate, urine osmolality, serum thyroxine, sitting systolic blood pressure, calculated blood volume, and serum chloride. Using discriminant analysis, these preflight variables were used to correctly classify the 64 astronauts according to their space sickness incidence (NOTSICK or SI

Baroreflex control of heart rate at rest and during exercise in microgravity conditions.

One of the most important features of prolonged weightlessness is a progressive impairment of muscular function. Physiological modifications related to exposure to microgravity, e.g. altered blood volume distribution, impaired myocardial function, and a decrease in stroke volume, in addition to the alterations in muscular function, contribute to the reduced physical exercise capacity commonly reported after space flight. The interplay between muscular, hemodynamic and body fluid modifications occurring in microgravity could alter also the neural mechanisms of cardiovascular regulation both in
