Incidence, risk factors, and consequences of new-onset atrial fibrillation following epicardial ablation for ventricular tachycardia.

INTRODUCTION: We sought to determine the incidence, predictors, and consequences of new-onset atrial fibrillation (AF) following epicardial ventricular tachycardia (VT) ablation. METHODS AND RESULTS: A total of 41 patients with no prior history of AF underwent epicardial VT ablation via a percutaneous subxiphoid approach. All patients were monitored continuously for 3 days following ablation and then via implantable cardiac defibrillator (ICD) or Holter monitoring. Mean age was 70.0 ± 11.3 years and mean ejection fraction was 30.3 ± 16.6%. In seven (17%) patients, the right ventricle (RV) was

Termination of sustained atrial flutter and fibrillation using low-voltage multiple-shock therapy.

BACKGROUND: Defibrillation therapy for atrial fibrillation (AF) and flutter (AFl) is limited by pain induced by high-energy shocks. Thus, lowering the defibrillation energy for AFl/AF is desirable. OBJECTIVE: In this study we applied low-voltage multiple-shock defibrillation therapy in a rabbit model of atrial tachyarrhythmias comparing its efficacy to single shocks and antitachycardia pacing (ATP). METHODS: Optical mapping was performed in Langendorff-perfused rabbit hearts (n = 18). Acetylcholine (7 ± 5 to 17 ± 16 μM) was administered to promote sustained AFl and AF, respectively. Single a

Diagnosis, clinical features, management, and post-natal follow-up of fetal tachycardias.

OBJECTIVE: To evaluate the diagnosis, clinical features, management and post-natal follow-up in consecutive fetuses identified with tachycardia. METHODS: We reviewed consecutive fetuses with tachycardia identified in a single tertiary institution between January, 2001, and December, 2008. We considered several options for management, including no treatment but close surveillance, trans-placental antiarrhythmic therapy in fetuses presenting prior to 36 weeks of gestation, and delivery and treatment as a neonate for fetuses presenting after 36 weeks of gestation. Data was gathered by a review o

Predictors of atrial tachyarrhythmias in subjects with type 1 ECG pattern of Brugada syndrome.

BACKGROUND: Previous studies have demonstrated a high incidence of atrial tachyarrhythmias (ATs) in patients with Brugada syndrome (BS). The present study aimed to investigate whether various 12-lead electrocardiogram (ECG) and electrophysiological parameters may help to differentiate subjects with a high probability to develop ATs. METHODS AND RESULTS: The clinical records of 38 individuals (31 males, age 44.4 +/- 13.9) with spontaneous (n = 15) or ajmaline-induced (n = 23) type 1 ECG pattern of BS were analyzed. During a mean follow-up period of 4.6 +/- 2.2 years, nine subjects suffered ATs

Effect of nifekalant for acute conversion of atrial flutter: the possible termination mechanism of typical atrial flutter.

BACKGROUND: Nifekalant is a class III antiarrhythmic drug, which is usually used for suppression of ventricular tachycardia (VT) and fibrillation. We studied the efficacy of nifekalant for acute conversion of atrial flutter (AFL) in a prospective, open label study in the intensive care unit (ICU) of cardiovascular medicine. METHODS: This study consisted of 31 patients. Twenty-six patients (84%) suffered from structural heart diseases. AFL was developed in 15 patients (48%) while on antiarrhythmic therapy with class IA or IC drugs (I-AFL group) for suppressing atrial fibrillation (AF) and in t

[Intravenous ablation of the atrio-ventricular junction in patients with supraventricular tachyarrhythmias].

Since the first successful therapeutic DC ablation of the AV junction in 1986, we have treated 20 symptomatic patients with drug-refractory supraventricular tachyarrhythmias (average of 6 antiarrhythmic drugs prior to the ablation attempt). The primary rhythm disturbances necessitating ablation were: AV nodal reentrant tachycardia (50% of pts), atrial flutter or fibrillation, with an uncontrolled rapid ventricular response (40%), atrioventricular reentrant tachycardia using an accessory pathway (20%), atrial tachycardia (10%), and junctional reciprocating tachycardia (5%). Percutaneous cathete

Research Advances in Timing of Anticoagulant Therapy Following Cardioembolic Stroke.

Cardioembolic stroke primarily results from thromboembolism caused by cardiac conditions such as atrial fibrillation, characterised by high recurrence rates, high rates of disability, and high mortality. Anticoagulation therapy is a key measure for the prevention of stroke recurrence, though the timing of its initiation remains debated, requiring a balance between the risk of thromboembolic recurrence and the risk of bleeding. The widely adopted "1-3-6-12-day rule" is based on expert consensus but lacks support from high-level evidence. In recent years, multiple randomized controlled trials (s

Glucagon-like Peptide-1 Receptor Agonists in Liver Transplant Recipients: A Retrospective Cohort Study.

BACKGROUND: Liver transplant recipients face high risks of cardiometabolic events after transplant, driven by posttransplant weight gain, diabetes, hypertension, as well as immunosuppression-related side effects. Glucagon-like peptide-1 receptor agonists (GLP1RAs) improve metabolic and cardiorenal outcomes in nontransplant populations, but their role in liver transplant recipients remains understudied. METHODS: This retrospective cohort study used TriNetX data (January 2010-December 2023) to compare outcomes in liver transplant recipients prescribed GLP1RAs (semaglutide, dulaglutide, liraglut

Prognostic role of myocardial storage, hypertrophy, and inflammation/fibrosis in Fabry cardiomyopathy: a new predictive score including T1 values.

AIMS: Limited data exist about the prognostic significance of the processes underlying Fabry Disease (FD) cardiomyopathy (storage, hypertrophy, inflammation/fibrosis) as detected by cardiac magnetic resonance (CMR). Notably, T1 mapping has had limited prognostic application primarily due to poor reproducibility across different sequences and vendors. This CMR study aimed to identify the prognostic weight of the different mechanisms of FD cardiomyopathy and to develop a predictive score of adverse cardiovascular (CV) events. METHODS AND RESULTS: This prospective, observational study enrolled 1

Cardiovascular protection by SGLT2 inhibitors: an integrative review of mechanistic networks, clinical evidence, and safety considerations.

Sodium-glucose cotransporter 2 (SGLT2) inhibitors were originally developed as glucose-lowering agents for type 2 diabetes mellitus, but accumulating evidence has demonstrated their broad cardiovascular and cardiorenal benefits beyond glycemic control. In particular, empagliflozin and dapagliflozin have shown robust prognostic benefits in heart failure with reduced ejection fraction (HFrEF) and expanding therapeutic value in heart failure with preserved ejection fraction (HFpEF). However, their cardiovascular protection cannot be fully explained by glucose lowering alone and is likely mediated

Left Atrial Volume Index, Left Ventricular Outflow Tract Gradient, and Incident Atrial Fibrillation in Hypertrophic Cardiomyopathy.

BACKGROUND: Atrial fibrillation (AF) is a common complication of hypertrophic cardiomyopathy (HCM). Whether left atrial volume index (LAVI) or resting left ventricular outflow tract (LVOT) gradient better identifies patients who develop AF is unresolved. OBJECTIVES: To compare baseline LAVI and resting LVOT gradient as predictors of incident AF in HCM. METHODS: We retrospectively studied 777 patients with HCM identified by ICD-10 code I42.1 (obstructive hypertrophic cardiomyopathy). Patients with pre-existing AF or atrial flutter were excluded, leaving 547 patients. The primary exposure was

Prognostic impact of atrial fibrillation and atrial flutter in patients with non-ST-segment elevation acute coronary syndrome.

INTRODUCTION AND OBJECTIVES: Arrhythmias such as atrial fibrillation and atrial flutter (AF/AFl) complicate the management of patients with non-ST-segment elevation acute coronary syndrome (NSTEACS). The aim of this study was to analyze the prognostic impact of AF/AFl in this clinical setting. METHODS: The IMPACT-TIMING-GO is a prospective, multicenter registry that enrolled patients with NSTEACS undergoing invasive management. In this subanalysis, patients were categorized based on the presence of AF/AFl (either pre-existing or occurring during admission). Clinical management and complicatio

Heart rhythm management optimisation of pacemaker recipients using remote monitoring: the HERO registry.

INTRODUCTION: Current expert consensus recommends remote monitoring (RM) for cardiac implantable electronic devices (CIEDs). As this recommendation is primarily based on studies involving implantable cardioverter defibrillators (ICDs), the HERO (HEart Rhythm management Optimisation of pacemaker recipients using remote monitoring) study aimed to reinforce this recommendation for pacemaker recipients. METHODS: The exploratory, retrospective, single-centre HERO study included 203 patients with an increased stroke risk (CHA2DS2-VASc score ≥ 2) but without a history of atrial fibrillation or flutt

Outcomes and Safety of Catheter Ablation in the Elderly.

Catheter ablation has emerged as a first-line therapy for many arrhythmias. However, data on the safety and outcomes of catheter ablation in the elderly population remain limited. Here, we aimed to study the outcomes of catheter ablation in octogenarians. The data used in this study were obtained from the National Inpatient Sample database through years 2016-2019. We identified patients ≥80 years old who were diagnosed with atrial fibrillation (AF), atrial flutter (AFL), supraventricular tachycardia (SVT), or ventricular tachycardia (VT) as primary diagnoses. The patients' characteristics and
