Predictors of deep-vein thrombosis in subarachnoid hemorrhage: a retrospective analysis.

BACKGROUND: Subarachnoid hemorrhage is a severe subtype of hemorrhagic stroke, and deep-vein thrombosis is a frequent complication detected in these patients. In addition to other well-established risk factors, the early activation of coagulation systems present in patients with subarachnoid hemorrhage could potentially play a role in the incidence of deep-vein thrombosis. This study aims to identify possible predictors for deep-vein thrombosis related to subarachnoid hemorrhage. METHODS: We conducted a retrospective cohort study on patients with a diagnosis of subarachnoid hemorrhage who pre

Therapy in pediatric stroke.

The aim of this review is to summarize the existing literature on therapy and management of cerebrovascular insults in children and adolescents. As data sources, studies were identified by MEDLINE, PubMed, Cochrane Library, and relevant bibliographies for the topic "pediatric stroke." We also reviewed guidelines for "stroke in adults." As a result, pediatric stroke is underestimated. The annual incidence for all stroke entities (cerebral venous thrombosis and hemorrhagic and arterial ischemic stroke) is as high as for pediatric brain tumors, 3-15/100.000 children per year. A distinct etiology

Solitary hemorrhagic cerebellar metastasis from occult papillary thyroid microcarcinoma.

BACKGROUND: Cerebellar metastasis (CM) from papillary thyroid carcinoma (PTC) is exceptional with only 12 reported cases and usually carries a very poor prognosis. In the two previously reported patients in whom CM was detected before PTC, other distant or local metastases were already present by the time of PTC diagnosis. We report a patient found to have papillary thyroid microcarcinoma after surgical resection and histopathological study of a large solitary hemorrhagic CM, who showed no evidence of other metastatic sites and survived 7 years after initial diagnosis. SUMMARY: A 65-year-old

Treatment of raised intracranial pressure following traumatic brain injury.

Appropriate treatment of raised intracranial pressure (ICP) after traumatic brain injury (TBI) remains a controversial topic in neurotraumatology. Several techniques are employed clinically, which include vasopressors, osmotically active substances, hyperventilation, and decompressive surgery. This article reviews six recent papers that have examined alternative methods of treating elevated ICP. The first two papers consider a new and controversial alternative to cerebral perfusion pressure (CPP) management, which involves mild hypotension coupled with pre-capillary vasoconstriction using dihy

[Extravasation of contrast media in an acute stage of middle cerebral artery occlusion-in relation to haemorrhagic infarction (author's transl)].

Following an embolic occlusion of a major cerebral artery with peripheral migration of emboli in the early stage, cerebral haemorrhage from the recanalized perforators may occur in the infarcted zone. The following is a report of such a case. This 66-year-old-man with normotension suffered from sudden unconsciousness and left-sided paresis. On admission to our clinic five hours after onset, the patient was confused with urinary incontinence, left hemiplegia and irregular pulse. The first CT examination, which was performed immediately after admission, showed a small-sized haematoma surrounded

Minimally Invasive Evacuation for Spontaneous Supratentorial Intracerebral Hemorrhage After Contemporary Randomized Trials: Patient Selection, Timing, and Platform Choice.

Spontaneous intracerebral hemorrhage (ICH) accounts for only 10-15% of all strokes but contributes disproportionately to stroke-related death and long-term disability. For decades, surgical evacuation of supratentorial ICH failed to show consistent functional benefit in randomized trials. The Early Minimally Invasive Removal of Intracerebral Hemorrhage (ENRICH) trial altered this trajectory by providing the first positive randomized evidence that an early, protocolized minimally invasive parafascicular approach can improve utility-weighted 180-day outcome in a predominantly lobar population. I

Cranioplasty as a therapeutic intervention for refractory postdecompressive craniectomy seizures in combat-traumatic brain injury: A report of two cases.

BACKGROUND: Combat-traumatic brain injury from penetrating and blast mechanisms frequently necessitates decompressive craniectomy (DC), leaving patients with large skull defects and retained metallic fragments that substantially elevate seizure risk. Syndrome of the trephined (SoT) and sinking skin flap syndrome (SSFS) are severe neurological complications attributed to disturbed cerebral hemodynamics, atmospheric pressure effects, and altered brain architecture following DC. Patients may develop refractory seizures, altered consciousness, and progressive neurological deficits despite maximal

Questioning the Question Mark for Decompressive Hemicraniectomy: A Modified Scalp Incision to Enhance Cranioplasty Outcomes-the Smart-C Incision.

BACKGROUND AND OBJECTIVES: The traditional reverse question mark is the most used incision marking for decompressive craniectomy (DC) worldwide. Motivated by reasons spanning from scalp blood supply, wound dehiscence, surgical access, to scar results, several authors have proposed alternative incision techniques. None of the reported incision techniques considered anticipated cranioplasty planning at the time of DC. This study is a consecutive case series reporting our outcomes in patients who underwent DC using a new incision marking, the smart-C, and subsequently underwent cranioplasty at ou

Untangling the debate: a meta-analysis comparing clipping versus coiling in ruptured middle cerebral artery aneurysms.

The optimal treatment strategy for ruptured middle cerebral artery (MCA) aneurysms remains debated, with microsurgical clipping offering superior occlusion and stroke rates in exchange for a more invasive experience. Although several landmark trials showed an overall benefit for coiling as compared to clipping, MCA aneurysms were underrepresented, and more granular data regarding the impact of the MCA morphology and risk factors have complicated interpretation of preceding data. We sought to perform a comprehensive contemporary meta-analysis comparing clinical and radiographic outcomes after c

Valproate-induced hyperammonemic encephalopathy: the role of clinical pharmacists in medication safety-a case report.

BACKGROUND: Sodium valproate, a commonly prescribed antiepileptic drug in clinical practice, has been occasionally linked to hyperammonemia, although the precise mechanisms underlying this adverse effect remain poorly understood. This article reports a case of hyperammonemic encephalopathy induced by sodium valproate in a patient with epilepsy. Through a comprehensive analysis and discussion of appropriate interventions, we aim to offer valuable insights for optimizing antiepileptic drug therapy in clinical settings, thereby preventing adverse drug reactions and ensuring optimal treatment outc

Abdominal and pelvic surgery in traumatic brain injury patients; a registry-based retrospective analysis.

BACKGROUND: Concurrent injuries with traumatic brain injury (TBI) can significantly impact patient prognosis. Abdominal and pelvic injuries accompanying TBI are infrequent and less discussed. We aimed to investigate the characteristics and outcomes of TBI patients who underwent abdominopelvic surgeries. METHODS: This cross-sectional study examined TBI patients aged 14 and older admitted to a level-I trauma center between 2016 and 2018. The independent variables were the routinely recorded information for trauma patients by physicians and nurses in the emergency department. The outcome measure
