Chondrolysis in slipped upper femoral epiphysis. Long-term study of the aetiology and natural history.

Of 44 patients (55 hips) with slipped upper femoral epiphysis treated from 1963 to 1989, 13 (14 hips) developed chondrolysis. Eight hips had chondrolysis at the time of presentation, all in female patients who were either coloured or black and who had moderate or severe slips. The other six hips had persistent pin penetration of the joint; in five of these the pin penetrated the anterosuperior quadrant of the head. Removal of penetrating pins resulted in improvement in pain in all six hips and in the range of movement in four. Chondrolysis did not develop in any of 11 hips with transient intra

Diseases of the developing hip joint.

The best treatment for diseases of the hip can begin if one diagnoses the disorder as soon as possible after onset. In many cases, successful management of the disease will prevent permanent damage to the hip joint. If injury occurs to the developing hip joint, major hip surgery may be necessary in adult life.

[Special factors governing x-ray examination of the hip joint in children (author's transl)].

The special features governing roentgen examinations of the hip joint in children in respect of positioning, x-ray technique and radiation protection, are explained. Particular attention is paid to the criteria of an accurate focussing of the pelvis in infants. Furthermore, standard focussing procedures and x-ray symptoms are described for the following diseases and malpositionings: changes of the centre-collum-diaphysis angle (coxa vara, coxa valga), torsional hip, epiphysiolysis capitis femuris, aseptic necrosis of the femoral head (M. Perthes), and coxitis in babies, infants, and juveniles.

Bony adaptation signs are predictive of anterior head-neck offset remodeling after internal fixation for slipped capital femoral epiphysis: a multicenter study on 217 patients (228 hips) with follow-up until end of growth.

BACKGROUND AND PURPOSE: We aimed to evaluate the prognostic value of radiographic factors in predicting femoral head-neck remodeling, cam deformity, and local growth disturbances in slipped capital femoral epiphysis (SCFE) treated with internal fixation (IF) with or without simultaneous closed reduction. METHODS: A retrospective multicenter study on 217 SCFE patients (228 hips) treated with IF in 10 institutions was performed. Hip morphology was assessed using preoperative, postoperative, and end of growth (EG) radiographs and radial MRI scans. Evaluated parameters included: epi-metaphyseal d

Is Subcapital Realignment for Slipped Capital Femoral Epiphysis a Reproducible Technique? Midterm Results.

BACKGROUND: The modified Dunn procedure, which is based on the development of an extended retinacular flap containing the blood supply for the femoral head, allows anatomic reestablishment in patients with moderate to severe slipped capital femoral epiphysis (SCFE). Some controversy exists regarding the short-term to midterm risk of avascular necrosis (AVN) and other complications resulting from the surgical technique. QUESTIONS/PURPOSES: (1) What percentage of patients treated with an extended retinacular flap during the modified Dunn procedure for SCFE with a moderate (slip angle from 30° t

Slipped Capital Femoral Epiphysis in Children without Obesity.

OBJECTIVE: To evaluate rates and characteristics of slipped capital femoral epiphysis (SCFE) in children who are not obese to prevent missed diagnoses and subsequent complications. STUDY DESIGN: A multicenter, retrospective review identified all patients with SCFE from January 1, 2003 to December 31, 2012. Patients were excluded if they received previous surgery at an outside institution, had no recorded height and weight, or had medical co-morbidity associated with increased risk of SCFE. Body mass index (BMI) percentile for age was calculated and categorized for each patient (patients witho

Clinical and radiological outcomes of chronic severe slipped capital femoral epiphysis patients treated by surgical dislocation and modified Dunn osteotomy: Case series.

OBJECTIVES: This study aims to evaluate the clinical and radiological outcomes of patients with chronic severe slipped capital femoral epiphysis (SCFE) treated by surgical dislocation and modified Dunn osteotomy (MDO). PATIENTS AND METHODS: This retrospective study, conducted between January 2010 and December 2017, included nine patients (8 males, 1 female; mean age 12.6 years; range, 7 to 16 years) with nine hips. The degree of corrections in Southwick angle and alpha angle were measured on frog-leg views. Range of motion (ROM) of the hips was measured at each follow-up visit. Heyman and Her

Applications of musculoskeletal ultrasonography in pediatric patients.

Ultrasonography is an excellent adjunct to other musculoskeletal imaging tools utilized in the pediatric population and in some instances offers advantages over CT and MRI. It permits dynamic examination of anatomic structures and assists in guiding minimally invasive procedures. In the lower extremity, ultrasonography assists in screening for such disorders as developmental dysplasia of the hip and in detecting slipped capital femoral epiphysis and femoral acetabular impingement. In the neonatal spine, ultrasonography can identify unossified vertebral arches. Among other applications in the u

Slipped capital femoral epiphysis following fracture of the femoral neck: a case report.

Reports on cases of femoral neck fracture complicated by a slipped capital femoral epiphysis associated with avascular necrosis and coxa vara deformity in children are extremely rare. In this case report, we describe a patient who had complications of a slipped capital femoral epiphysis with avascular necrosis and coxa vara deformity after a Delbet type III left femoral neck fracture. We also describe the surgical treatment to overcome these complications.

Total hip arthroplasty in young adults, with focus on Perthes' disease and slipped capital femoral epiphysis: follow-up of 540 subjects reported to the Norwegian Arthroplasty Register during 1987-2007.

BACKGROUND AND PURPOSE: Pediatric hip diseases account for 9% of all primary hip arthroplasties in the Norwegian Arthroplasty Register. We wanted to validate the diagnosis as reported to the register and to assess the quality of life of these patients after hip replacement. PATIENTS AND METHODS: 540 patients accepted to participate in this follow-up study (634 hips). All were less than 40 years of age and had been reported to the Norwegian Arthroplasty Register as having undergone a primary total hip arthroplasty (THA) between 1987 and 2007. The underlying diagnosis, age at diagnosis, and typ

Femoral morphology due to impingement influences the range of motion in slipped capital femoral epiphysis.

Femoroacetabular impingement due to metaphyseal prominence is associated with the slippage in patients with slipped capital femoral epiphysis (SCFE), but it is unclear whether the changes in femoral metaphysis morphology are associated with range of motion (ROM) changes or type of impingement. We asked whether the femoral head-neck junction morphology influences ROM analysis and type of impingement in addition to the slip angle and the acetabular version. We analyzed in 31 patients with SCFE the relationship between the proximal femoral morphology and limitation in ROM due to impingement based

[Profile of radiologic-orthopedic requirements in pediatric hip dysplasia, coxitis and epiphyseolysis capitis femoris].

DDH, septic coxarthritis and slipped capital epiphysis should be diagnosed as soon as possible. Hip sonography is the goal of DDH examination technique, anatomical identification and 3 landmarks check up are there most important points of the method. With the progress of ossification and loss of the 3 landmarks consecutively, x-rays become increasingly important. Septic coxarthritis is an orthopaedic emergency case, sonography is the most important tool in primary diagnosis. Slipped capital epiphysis requires an x-ray in 2 planes, MRI oder CT scans respectively makes sense in special cases to

[Post-traumatic problem cases involving the elbow in children].

UNLABELLED: In the last 20 years 49 children with gross posttraumatic elbow deformities have been treated in our hospital: 19 patients with an overlooked radial head dislocation, 12 children with a radial condyle deformity and 19 patients with a severe radial head deformity. SECONDARY TREATMENT: In the majority of cases secondary surgical procedures led to unsatisfying results. Only 4 patients with a pseudarthrosis of the radial condyle were treated secondarily. Surgical fixation led to good functional results but was not able to remove the joint deformity. Overlooked radial head dislocations

Results of slipped capital femoral epiphysis in the black population.

This retrospective study, based on a chart and radiographic review of blacks treated for slipped capital femoral epiphysis (SCFE), showed a 93.2% incidence of excellent or good results, and an incidence of only 2.3% of major complications with poor results. Blacks can generally be expected to have a good result after treatment of SCFE in both males and females with a low incidence of avascular necrosis (AVN) or acute cartilage necrosis (ACN).

Transient loss of femoral head Tc-99m diphosphonate uptake with prolonged maintenance of femoral head architecture.

Nine pediatric patients presented with synovitis and suspected Legg-Perthes disease, hemarthrosis, acute slipped capital femoral epiphysis, prolonged traumatic hip dislocation, or transcervical femoral fracture. All had hip scintigraphs that showed loss of Tc-99m diphosphonate uptake in the affected femoral head. Radiographic changes associated with aseptic necrosis failed to appear during the initial follow-up period of three to 14 months. Repeat scintigraphy showed complete revascularization in six cases and partial revascularization in two. Reambulation at 1.5 to eight months after presenta
