OBJECTIVE: Few studies regarding the epidemiology of essential tremor (ET) and its non-motor symptoms (NMS) have been conducted to date. We thus conducted a door-to-door survey to investigate the ET and its NMS and their clinical manifestations in a rural area of Shanghai, China.
METHODS: We recruited 19,614 residents (≥ 50 years old), living in Malu town, Shanghai. Investigations were divided into two steps. In first step, trained physicians conducted door-to-door surveys and reported probable ET subjects to movement disorder specialists. In the second step, participants with suspected ET ha

BACKGROUND: Parkinson's disease (PD) is the second most common neurodegenerative disorder affecting quality of life. The onset of PD is thought to result from a multifaceted convergence of aging, genetic predisposition, and environmental exposure. The hemoglobin, albumin, lymphocyte, and platelet (HALP) score, a specific measure of inflammation and nutrition, has been identified in the literature. No study has determined whether there are differences in inflammation and nutrition between sporadic and familial forms of PD.
METHODS: A cross-sectional study was conducted involving 1036 participa

Parkinson's disease (PD) is a neurodegenerative disorder whose aetiology remains unclear: degeneration involves several neurotransmission systems, resulting in a heterogeneous disease characterized by motor and non-motor symptoms. PD causes progressive disability that responds only to symptomatic therapies. Future advances include neuroprotective strategies for use in at-risk populations before the clinical onset of disease, hence the continuing need to identify reliable biomarkers that can facilitate the clinical diagnosis of PD. In this evaluative review, we summarize information on potentia

PURPOSE: The ventral tegmental area (VTA), which is rich in dopaminergic neurons, may play a role in influencing clinical symptoms in Parkinson's Disease (PD). However, the degeneration dynamics of the VTA during the early and prodromal stages of PD remain unclear. This study aims to explore microstructural changes in the VTA among prodromal PD patients with idiopathic REM sleep behavior disorder (iRBD) and early-stage PD patients using free water imaging (FWI) to assess free water (FW) and its correlation with clinical symptoms.
METHOD: Diffusion tensor imaging data from 238 participants, in

BACKGROUND: Synucleinopathies include a spectrum of disorders varying in features and severity, including idiopathic/isolated REM sleep behaviour disorder (iRBD), Parkinson's disease (PD), and dementia with Lewy bodies (DLB). Distinct brain atrophy patterns may already be seen in iRBD; however, how brain atrophy begins and progresses remains unclear.
METHODS: A multicentric cohort of 1276 participants (451 polysomnography-confirmed iRBD, 142 PD with probable RBD, 87 DLB, and 596 controls) underwent T1-weighted MRI and longitudinal clinical assessments. Brain atrophy was quantified using verte

Primary wake disorders encompass various conditions of excessive daytime sleepiness and/or increased nighttime sleep, of unknown origin beginning most often in adolescence and of chronic or recurrent natural history. The best known of these conditions is narcolepsy associating two major clinical features, irresistible episodes of sleep, sleep onset REM periods and an almost constant association with HLA DR2-DQ1. The prevalence of the condition is close to the one of multiple sclerosis but positive diagnosis requires most often over 10 years to be made. The treatment of excessive daytime sleepi

Increasing evidence indicates that bioactive lipids participate in the regulation of synaptic function and dysfunction. We have demonstrated that signaling mediated by platelet-activating factor (PAF) and cyclooxygenase (COX)-2-synthesized PGE2 is involved in synaptic plasticity, memory, and neuronal protection [Clark GD, Happel LT, Zorumski CF, Bazan NG. INTRODUCTION: Considering that psychosis in Parkinson disease (PD) is associated with worse outcomes, including dementia, we aimed to study the characteristics, correlates, and assessment of PD psychosis in those without dementia.
METHODS: 101 PD subjects without dementia (Montreal Cognitive Assessment ≥21/30) were recruited to participate in a study of neuropsychiatric symptoms in PD. This study included a baseline standard neurological exam and common PD symptom assessments. Using the Scale for the Assessment of Positive Symptoms (SAPS) and separate assessment of visual illusions and sense

Pathogenic variants in ATP1A3 encoding the neuronal Na/K-ATPase cause a spectrum of neurodevelopmental disorders including alternating hemiplegia of childhood (AHC). Three recurrent ATP1A3 variants are associated with approximately half of known AHC cases and mouse models of two of these variants (p.D801N, p.E815K) replicated key features of the human disorder, which include paroxysmal hemiplegia, dystonia and seizures. Epilepsy occurs in 40-50 % of individuals affected with AHC, but detailed investigations of seizure phenotypes were limited in the previously reported mouse models. Using gene

There is a body of evidence suggesting that mitochondrial dysfunction is involved in bipolar disorder (BD) pathogenesis. Studies suggest that abnormalities in circadian cycles are involved in the pathophysiology of affective disorders; paradoxical sleep deprivation (PSD) induces hyperlocomotion in mice. Thus, the present study aims to investigate the effects of lithium (Li) and valproate (VPA) in an animal model of mania induced by PSD for 96 h. PSD increased exploratory activity, and mood stabilizers prevented PSD-induced behavioral effects. PSD also induced a significant decrease in the acti

BACKGROUND: Cognitive impairment (CI) is a prevalent non-motor complication of Parkinson's disease (PD), yet accessible and reliable screening aids remain limited. This cross-sectional study aimed to quantify the strength of association and the cross-sectional classification value of routine blood indicators combined with standardized sleep questionnaires for prevalent CI in PD, using a strictly leakage-free model-development pipeline.
METHODS: A total of 347 PD patients were enrolled and classified into PD-CI (n = 143) and PD-NCI (n = 204) groups based on the Montreal Cognitive Assessment (M

BACKGROUND: Isolated rapid eye movement sleep behavior disorder (iRBD) serves as a prodromal phase of Parkinson's disease (PD) and dementia with Lewy bodies (DLB). Blunted tachycardia (BT) during postural changes indicates neurogenic orthostatic hypotension, a marker of autonomic dysfunction. We aimed to investigate whether BT is associated with cardiac sympathetic neurogenic denervation. Additionally, we conducted a preliminary short-term follow-up to examine the potential prognostic significance of BT regarding phenoconversion and mortality.
METHODS: Forty-three patients with iRBD at Shiga

BACKGROUND: The histological feature of Parkinson's disease is the presence of intraneuronal aggregates of phosphorylated α-synuclein (αSyn). In patients with Parkinson's disease, deposits of αSyn are found in the autonomic nerve fibres of the submandibular gland. Since patients with idiopathic rapid-eye-movement sleep behaviour disorder (IRBD) can develop Parkinson's disease and other synucleinopathies, we investigated whether αSyn deposits could also be detected in their submandibular gland nerve fibres.
METHODS: We did a case-control study at the Hospital Clinic de Barcelona (Barcelona, Sp

BACKGROUND: In Parkinson's disease (PD), two pathogenetic subtypes have been proposed: a 'brain-first', with α-synuclein pathology arising in one hemisphere and spreading secondarily to the peripheral autonomic nervous system, and a 'body-first' subtype, with the pathology originating in the enteric or peripheral autonomic nervous system and subsequently spreading symmetrically to the brain. To dissect these subtypes, we assessed the association between pupillary dysfunction, mesencephalic raphe and substantia nigra changes, vagus nerve atrophy and vagal electrocardiographic parameters in PD p

BACKGROUND: Isolated REM sleep behavior disorder (iRBD) is a prodromal stage of alpha-synucleinopathies. Biomarkers are crucial for predicting and monitoring its progression, warranting long-term neuroimaging studies. While the Parkinson's Disease Related Pattern (PDRP) from 18F-FDG PET is a recognized Parkinson's Disease (PD) biomarker, its role in tracking progression in prodromal PD remains unclear.
OBJECTIVE: To explore PDRP expression across three time points using 18F-FDG PET over an 8-year follow-up in iRBD.
METHODS: Thirteen iRBD subjects underwent 18F-FDG PET brain scans at baseline
