[Serious postoperative metabolic alkalosis during hospitalization].

Metabolic alkalosis is a frequent and serious abnormal acid-base disturbance with a high-pH-dependent morbidity and mortality rate. This situation is easy to recognize, and the treatment is relatively simple. We present a case history dealing with metabolic alkalosis as a consequence of severe acid-base disturbance with electrolyte and fluid imbalance caused by continuous postoperative vomiting and nasogastric suction. Our aim is to attract greater attention to this acid-base disturbance and a better understanding of the pathogenesis, which in turn should lead to earlier intervention and more

The kidney and the balance of sulfur and nitrogen as fundamental components of pH homeostasis.

The homeostasis of blood and tissue pH is fundamental for life, and pH imbalances may lead to coma and death. The regulation of the acid-base balance involves primarily the lungs and kidneys, and is strictly integrated via the endocrine and nervous systems. Crucial theoretical concepts have been developed to describe these functions, for example the Base Excess (BE) for the interpretation of the blood gas analysis or the Net Acid Excretion (NAE) for the interpretation of the kidney contribution to acid-base homeostasis. However, these concepts, coming from different research fields, have been

A Quick Reference on Anion Gap and Strong Ion Gap.

Metabolic acid-base disorders frequently occur in emergency and critically ill patients. Recognizing these disturbances can be challenging, particularly when multiple acid-base imbalances coexist within a single patient. The anion gap (AG) and strong ion gap (SIG) are valuable calculations that help identify complex metabolic acid-base disturbances associated with unmeasured anions. This review provides definitions, reference values, indications, limitations, and guidelines for interpreting AG and SIG changes in clinical settings.

[Comparison of different methods for monitoring of acid base status in dairy cow herds].

OBJECT AND PURPOSE: Acid-base disorders in dairy herds can be diagnosed by determining urinary net base excretion (NBE). Modifications of this method are the differential NBE (dNBE) with determination of the urinary concentration-independent base-acid ratio (BAR) and the simplified NBE test with reduced urine volume (sNBE). The aim of this study was to compare these methods among themselves and as a pooled test, in their assessment of cow group acid base status as well as to derive recommendations for practical use. ANIMALS, MATERIAL, AND METHODS: The concentrations of NBE, dNBE, and sNBE wer

Studies on acid-base status (Stewart's model) of young camels (Camelus dromedarius) after acid-load with NH4Cl.

UNLABELLED: The identification of the reference values of Stewart variables and the changes in the acid-base status of the blood and urine in relation to the age after acid-load were studied in desert species of camels. 14 healthy young camels (age: 3-5 months) and 22 adult camels (age: 5-8 years) were used to provide the reference and percentile ranges of the Stewart variables. In the experiments, 24 healthy young camels (age: < 3-5 months) were infused with 5M NH4Cl solution (dose: 1.0 ml/kg) through a permanent intravenous catheter. Venous blood and urine samples were collected before infus

Primer on clinical acid-base problem solving.

Acid-base problem solving has been an integral part of medical practice in recent generations. Diseases discovered in the last 30-plus years, for example, Bartter syndrome and Gitelman syndrome, D-lactic acidosis, and bulimia nervosa, can be diagnosed according to characteristic acid-base findings. Accuracy in acid-base problem solving is a direct result of a reproducible, systematic approach to arterial pH, partial pressure of carbon dioxide, bicarbonate concentration, and electrolytes. The 'Rules of Five' is one tool that enables clinicians to determine the cause of simple and complex disord

Use of the anion gap in clinical medicine.

The anion gap (AG) in the serum equals the concentrations of Na-(Cl + HCO3). It is becoming increasingly useful in the interpretation of acid-base disorders and in the diagnosis of other conditions. In an acidemic patient, an elevated AG usually indicates the presence of an organic acidosis, rhabdomyolysis, nonketotic hyperglycemic coma, uremia, or certain intoxications. An increased AG with alkalemia suggests severe alkalosis with hemoconcentration or use of anionic antibiotics (eg, carbenicillin) or salts of organic acids (eg, citrate). An elevated AG with a normal serum pH could be an artif

Overhydration: A cause or an effect of kidney damage and how to treat it.

Volume overload can be both the cause and effect of chronic kidney disease (CKD). Overhydration often accompanies renal insufficiency. In cardiovascular disease (CVD), fluid overload can also be the cause of renal function impairment. Beside salt restriction, loop diuretics are the first-line therapy. Frequently developed resistance can be overcome by switching to intravenous administration, adding albumin alone or in combination with other diuretics. Transient factors like infection or contrast media can impair diuretic response and contribute to congestion. Apart from conservative management

Dietary Chloride Deficiency Syndrome: Pathophysiology, History, and Systematic Literature Review.

Metabolic alkalosis may develop as a consequence of urinary chloride (and sodium) wasting, excessive loss of salt in the sweat, or intestinal chloride wasting, among other causes. There is also a likely underrecognized association between poor salt intake and the mentioned electrolyte and acid-base abnormality. In patients with excessive loss of salt in the sweat or poor salt intake, the maintenance of metabolic alkalosis is crucially modulated by the chloride-bicarbonate exchanger pendrin located on the renal tubular membrane of type B intercalated cells. In the late 1970s, recommendations we

Clinical utility of anion gap in deciphering acid-base disorders.

The anion gap (AG) measurement is a very useful tool in the evaluation of patients with acid-base disorders. Once metabolic acidosis is identified, AG will provide the important first step in the differential diagnosis of disorders that either increase the AG and those that leave the AG unchanged. Delta gap is the comparison between change (delta) in the AG and the change (delta) in bicarbonate (HCO(3)(-)). Delta ratio, defined as delta AG:delta HCO(3)(-) is usually 1:1 in patients with an uncomplicated high AG acidosis. A value below 1:1 suggests a combined high and normal AG acidosis. A valu

Sex differences in renal acid-base regulation.

PURPOSE OF REVIEW: Sex-dependent differences are evident in most organs and contribute to variability in physiological function and disease susceptibility, which can consequently affect therapeutic responses. The kidney is no exception to this finding and recent data suggest that sexual dimorphism exists in the regulation of acid-base homeostasis. RECENT FINDINGS: While proteins involved in the renal regulation of acid-base homeostasis have been characterized over several decades, researchers have only recently begun investigating how biological sex affects these processes. Although males and

Quantitative Acid-Base: A Simplified Approach for the Emergency Physician.

BACKGROUND: A deep understanding regarding acid-base physiology has been generally absent from what is considered the skill set of Emergency clinicians. The calculations needed to obtain a solid comprehension of acid-base are thought to be too complex and time consuming, making them impractical for the chaotic environment of the Emergency Department. OBJECTIVES: Make acid-base more approachable and accessible for the Emergency Physician. DISCUSSION: We present a simplified approach to quantitative acid-base, allowing clinicians to gain a general understanding of even the most complex acid-ba

Physicochemical properties of abnormal blood acid-base buffering.

This paper describes two new features 1) development of physicochemically based, two-compartment models describing acid-base-state changes in normal and abnormal blood and 2) use of model results to view and describe physicochemical properties of blood, in terms of Pco2 as the causative independent variable and effected [H+] changes as the dependent variable. Models were derived from an in vitro experimental study, where normal blood was made both hypoproteinemic and hyperalbuminemic and then equilibrated with CO2. Strong-ion gap (SIG) values were selected to match model and experimental pH. T

The therapeutic importance of acid-base balance.

Baking soda and vinegar have been used as home remedies for generations and today we are only a mouse-click away from claims that baking soda, lemon juice, and apple cider vinegar are miracles cures for everything from cancer to COVID-19. Despite these specious claims, the therapeutic value of controlling acid-base balance is indisputable and is the basis of Food and Drug Administration-approved treatments for constipation, epilepsy, metabolic acidosis, and peptic ulcers. In this narrative review, we present evidence in support of the current and potential therapeutic value of countering local

A mathematical model of the four cardinal acid-base disorders.

Precise maintenance of acid-base homeostasis is fundamental for optimal functioning of physiological and cellular processes. The presence of an acid-base disturbance can affect clinical outcomes and is usually caused by an underlying disease. It is, therefore, important to assess the acid-base status of patients, and the extent to which various therapeutic treatments are effective in controlling these acid-base alterations. In this paper, we develop a dynamic model of the physiological regulation of an HCO3-/CO2 buffering system, an abundant and powerful buffering system, using Henderson-Hasse
