Accuracy of oscillometric devices in children and adults.

Clinical validation of blood pressure oscillometric devices are almost exclusively conducted in adults. Because oscillometric devices are used in children, it is critical to assess their accuracy also in this population. We compared blood pressure readings using an oscillometric automated device (Dinamap XL, model CR9340) with readings obtained with the mercury auscultatory method in children and adults. Blood pressure was measured in 30 children (15 boys and 15 girls; 9.5+/-1.0 years old) and their parents (25 mothers and 15 fathers; 41.0+/-5.0 years old). In children, mean difference (+/-SD)

[Epiglottitis: a children's disease seen in adults].

Two women, aged 58 and 62, developed epiglottitis with acute symptoms of sore throat, respiratory distress, fever, and stridor on inhalation. Both developed an upper respiratory tract obstruction requiring acute intubation to secure an open airway. The symptoms disappeared after administration of intravenous antibiotics and corticosteroids. Both women were discharged without any residual signs of infection. The incidence of epiglottitis in adults is most likely rising. The illness used to be seen almost only in children. A clear explanation for this rise in incidence cannot be given. It is imp

Growth hormone therapy in children and adults.

Growth hormone (GH) is a polypeptide hormone, secreted by somatotropic cells of the anterior part of the hypophysis. Its application in therapy, first limited to GH deficient children, has now been widened to various other clinical conditions, not necessarily related to short stature. Clinical trials conducted in recent years have proved the safety of its administration in both children and adults. The efficacy of this form of therapy varies, according to different authors, from enthusiastic data to very critical opinions. For many pediatric diseases, such as GH deficiency or Turner syndrome,

Elevated urinary dopamine in adults and children.

BACKGROUND: Information on the significance of an elevated urinary dopamine is limited and can lead to misinterpretation of the cause of such a finding. This laboratory-based study examines the associations with elevated dopamine gathered from a significant number of patients. METHODS: The urine catecholamine and metabolite results of specimens (analysed by HPLC-ECD) from 5933 adults and 467 children were examined retrospectively over a 57-month period. Those with elevated dopamine were identified and the explanation for this finding was sought. RESULTS: In adults, the conditions associated

Experience with varicoceles in children and young adults.

PURPOSE: Varicocele ligation in children and adolescents has been advocated for left testicular growth arrest. We report our experience with this population as well as the results of the microscopic inguinal technique. MATERIALS AND METHODS: Between 1994 and 1999, 184 children and young adults presented with unilateral left varicoceles. At presentation 171 patients were asymptomatic and 13 had pain. Testicular measurement was assessed with calipers in 158 cases and ultrasound in 26. Surgery was performed using a microscopic inguinal technique in 58 patients and loupes in 16 of those who prese

Purulent otitis media in children and adults.

This study was designed to compare tympanic membrane (TM) and middle ear (ME) pathologies of temporal bones from children and adults with purulent otitis media (POM). Thirty-four temporal bones were used from 22 subjects ages 2 days to 76 years with histopathologic evidence of POM. There were 55 age-matched controls. Histopathologic findings of the TM and ME in children and adults with POM were compared. Clinical histories and the presence of complications were recorded. The incidence of POM was more common in male children than in females. There was a significant increase in the thickness of

Optimizing gh therapy in adults and children.

Until the advent of modern neuroradiological imaging techniques in 1989, a diagnosis of GH deficiency in adults carried little significance other than as a marker of hypothalamo-pituitary disease. The relatively recent recognition of a characteristic clinical syndrome associated with failure of spontaneous GH secretion and the potential reversal of many of its features with recombinant human GH has prompted a closer examination of the physiological role of GH after linear growth is complete. The safe clinical practice of GH replacement demands a method of judging overall GH status, but there i

Copying strategies for patterns by children and adults.

In the present study, eve movements during the copying of a pattern were analyzed to compare visual strategies of adults and children. Subjects had to build an accurate copy of spatial block patterns. Tested variables were the incidence and the duration of ocular dwelling in the pattern area (where the pattern to be copied was located), the work area (where the copy was made), and the source area (where the blocks were that could be used for creating the copy). Furthermore, to unravel employed strategies, sequences of dwelled areas were investigated. Previous studies reported that adults emplo

Skiing injuries in children, adolescents, and adults.

We prospectively gathered data on skiing injuries that had been sustained at the Sugarbush North ski area since 1972 and at the Sugarbush South ski area since 1981. The purpose of the current study was to document the overall rates of injury in children, adolescents, and adults participating in alpine skiing. We also sought to determine the ten most common injuries in each age-group. Finally, we analyzed short-term and long-term trends to determine if changes in equipment had had an effect on the frequency or pattern of injury. From the 1981-1982 to the 1993-1994 season, there were 2.79 injuri

Binaural additivity of loudness in children and adults.

Thirty-six different binaural noises were formed by crossing six right-ear intensities of a broadband noise with the same six intensities in the left ear in a 6 X 6 factorial design. Children (6-7 years of age) and adults were presented with 2 of these 36 binaural noises on a trial and asked to indicate which noise was louder. In Experiment 1, the left- and right-ear noises were in phase and differed only in intensity. In Experiment 2, the left- and right-ear noises were in opposite phase. For both the children and adults in Experiments 1 and 2, the paired comparison judgments of binaural loud

Nails and nail disorders in children and adults.

Abnormalities of the nails can provide both subtle and obvious clues to common medical problems or severe systemic diseases. Heredity, minor injuries and a variety of infections account for many changes in the appearance of nails. Several dermatologic disorders, such as psoriasis and alopecia areata, and other multisystem disorders may present with nail changes; abnormalities of the nails may be the single most prominent feature. Common habits and the use of various cosmetics also may result in nail changes. Awareness of normal nail variants and well-defined abnormalities and their disease ass

Choledochal cysts--differences in children and adults.

Choledochal cysts in children and adults may behave differently. To identify these differences the records of 49 patients (22 children and 27 adults) who underwent surgery for choledochal cysts over a period of 7 years were analysed retrospectively. In two adult patients who had undergone a previous cholecystectomy an acquired malformation could not be excluded. Cholangitis was more common in adults. Choledochal cysts in children were predominantly Type I cystic lesions, whereas Type IV cysts were more common in adult patients. Anomalies of the pancreatic duct and associated hepatobiliary prob

Effects of hemophilia on articulations of children and adults.

The majority of bleeding episodes in hemophilic patients occur within the joints. Of these hemarthroses, the knees, elbows, and ankles account for almost 80%. Should the bleeding persist, the synovium starts to hypertrophy and a vicious cycle of chronic synovitis develops, leading to joint destruction. In immature articulation, synovitis causes hypertrophy of the epiphyseal growth plates and significant structural deficiencies may rapidly develop. This stimulus to the growth plates results in bone hypertrophy, leg length discrepancy, and angular deformities. In mature articulation, hemophilia

Bronchiolitis following infection in adults and children.

Since its recognition as a clinical entity in the 1940s, our knowledge of bronchiolitis has grown with respect to the breadth of disease, epidemiology, treatment, and long-term effects. Bronchiolitis occurs most commonly in infants and children in association with a predictably small number of respiratory pathogens. Our knowledge of the association of bronchiolitis to subsequent wheezing is in a state of evolution. Rarely, bronchiolitis may progress to a chronic obstructive process, bronchiolitis obliterans, seen most often following adenovirus infection. The diagnosis of bronchiolitis in adul

Management of urachal anomalies in children and adults.

During a period of fifteen years, 9 children and 5 adults with a variety of urachal anomalies were treated. Infected urachal cysts were more common in children whereas adults more frequently had infected urachal sinuses. After careful physical examination, a preoperative diagnosis could be made in most patients, with ultrasound examination decisive in doubtful cases. Whenever feasible, complete excision of the umbilicovesical tract is performed, but in very ill patients, a staged treatment becomes necessary. The preoperative injection of methylene blue is helpful in the identification of commu

Equestrian injuries in children and young adults.

We reviewed the English language scientific literature about equestrian injuries among children and young adults. All studies showed that more females than males were injured, with falls from horses being the most common cause of injury. Fractures were common, and head injuries were associated with the vast majority of deaths (72% to 78%) and hospitalizations (55% to 100%). Although the overall injury rate was low, equestrian athletes are at risk for serious injuries. Pediatricians should know the medical contraindications for participation in equestrian sports and encourage riders to obtain h
