AIM: Traumas are one of the most relevant pathological events in health care, because of clinical and prognostic relevance. Morbidity and mortality rates are strongly affected by the timely and correct approach to the patient (golden hour). The objective of this study was to investigate the possible influence of the diagnostic time and of the Injury Severity Score (ISS) on outcomes in trauma patients.
MATERIAL OF STUDY: Out of a total of 240,833 emergency patients, we observed, 447 polytrauma. All patients were assessed according to the Advanced Trauma Life Support (ATLS) guidelines, diagnose

OBJECTIVES: The trauma tertiary survey (TTS) is an essential part of the continued care for major trauma patients which is performed to ensure that all injuries have been identified and none have been overlooked during the patient's stay. Although the Advanced Trauma Life Support Course states a need for a tertiary survey, there is currently no standard for what this survey comprises.
METHODS: Using local consultant expert opinion and a literature search we identified a set of 32 TTS potential features that may be included within a TTS pro forma. Major trauma center (MTC) documents were reque

Unstable pelvic fractures very frequently occur with haemorrhage, not only from the broken pelvis but from the presacral venous plexus and/or iliac arterial or venous branches which may cause hypotension and increases the mortality rate. Very often this type of injury is concurrent with injuries in other organ systems. The compounded nature of these injuries makes it necessary for doctors who may encounter this type of patient to be educated in the techniques to effectively stabilise and treat the patient's complex injuries. After completing the international standard ATLS (Advanced Trauma Lif

To compare emergency department (ED) use by elderly patients (age 65 yrs or older) with that by younger adult patients (age 15-64 yrs), a prospective consecutive chart review study was conducted from August 1, 1995 through May 31, 1996 in eight designated hospitals in Taipei city (which has 2.6 million residents, 8.4% of whom are 65 yrs or older). Patient age, sex, mode of arrival, living status, triage category, advanced life support (ALS) eligibility, referral, confinement to bed, intensive care unit (ICU) admission, impression, trauma mechanism, and disposition were recorded and compared be

BACKGROUND: Loss of facial structures due to trauma, congenital anomalies, or ablative surgery induces profound psychosocial consequences that impact beyond functional impairment. While rehabilitation with advanced prosthetic materials have improved esthetics and comfort, standardized measurement of psychosocial outcomes remains limited.
AIM: To assess psychosocial reintegration following maxillofacial prosthetic rehabilitation using a modified Psychosocial Impact of Assistive Devices Scale (PIADS-10) and to support the findings through a focused review of the literature.

INTRODUCTION: Emergency medicine is a relatively new field in sub-Saharan Africa and dedicated training in pediatric emergency care is limited. While guidelines from the African Federation of Emergency Medicine (AFEM) regarding emergency training exist, a core curriculum in pediatric emergency care has not yet been established for providers at the district hospital level.
METHODS: The objective of the project was to develop a curriculum for providers with limited training in pediatric emergencies, and contain didactic and simulation components with emphasis on treatment and resuscitation usin

OBJECTIVES: We aimed to provide the first national study of out-of-hospital cardiac arrests (OHCA) witnessed by emergency medical services (EMS) during interfacility transport (IFT).
METHODS: We used a nationwide prehospital patient care report dataset from 2023 to complete a cross-sectional observational study of EMS encounters for IFT during which OHCA occurred. Descriptive statistics were computed, stratified by level of EMS care provided. Multivariable logistic regression analysis was completed to evaluate factors associated with return of spontaneous circulation (ROSC) at the end of the

Extracorporeal membrane oxygenation (ECMO), an advanced life support method, was developed to treat severe cardiac and pulmonary failure in critically ill patients. ECMO was previously used to treat ARDS, cardiogenic shock, and after heart or lung transplant. It has since become a versatile therapeutic and surgical tool. When conventional methods fail, this technique works well for high-risk procedures such as tracheal resections, ventricular tachycardia ablations, and complicated percutaneous coronary interventions. These uses demonstrate ECMO's ability to oxygenate and stabilize the hemodyna

INTRODUCTION: The advancement of interventional neuroradiology has drastically altered the treatment of stroke and trauma patients. These advancements in first-world hospitals, however, have rarely reached far forward military hospitals due to limitations in expertise and equipment. In an established role III military hospital though, these life-saving procedures can become an important tool in trauma care.
MATERIALS AND METHODS: We report a retrospective series of far-forward endovascular cases performed by 2 deployed dual-trained neurosurgeons at the role III hospital in Kandahar, Afghanist

Background: Necrotizing fasciitis (NF) is a rapidly progressing and life-threatening soft tissue infection. It commonly affects the extremities and is triggered by trauma, surgical wounds or compromised skin integrity. Although there have been advancements in diagnosis and treatment, NF continues to be a complex condition due to comorbidities and the wide range of involved pathogens. The aim of this study was to assess the epidemiology, risk factors, and outcomes of NF in Switzerland, focusing on the distinction between polymicrobial and monomicrobial infections and their associated management

BACKGROUND: People experiencing homelessness have high rates of multi-morbidity and age-related conditions at a young age. Despite having high support needs, they have disparately low access to palliative care services and often die at a young age. To facilitate access to support for this group towards the end of life, a multi-professional approach should be taken. Over recent years, clinical and research activities have begun to address this issue. However, until now, there has been no centralised United Kingdom-based group to facilitate collaboration and shared learning.
AIM: To build multi

Objective Pediatric traumas are high-stakes yet relatively low-frequency events, limiting resident exposure and learning. We developed a low-cost, accessible virtual reality (VR) experience using 360-degree video to supplement pediatric trauma education and explored resident perceptions of its usability, relevance, and educational value. Methods We performed a mixed-methods descriptive study at an academic pediatric emergency department. Residents used VR headsets to experience a simulated pediatric blunt abdominal trauma resuscitation which was created with 360-degree video equipment. Interac

This article proposes a counter-argument to standard Advanced Trauma Life Support (ATLS) training--which advocates bladder catheterisation to be performed as an adjunct to the primary survey and resuscitation for early decompression of the bladder and urine output monitoring. We argue the case for delaying bladder catheterisation until after definitive truncal Computed Tomography (CT) imaging. To reduce pelvic volume and associated bleeding, our trauma team delay catheter insertion until after the initial CT scan. The benefits of a full bladder also include improved views on initial Focussed A

This paper introduces the Trauma THOMPSON dataset, designed to advance AI-driven decision support for life-saving interventions (LSIs) in emergency care, particularly in resource constrained humanitarian settings. The dataset comprises 3,717 high resolution and egocentric video clips of both regular and just-in-time (JIT) procedures. The JIT procedures consists of videos of the same LSI procedures, but with makeshift tools, and is useful for studying human medical commonsense. Each clip is annotated by medical professionals with verb-noun format, such as "take scalpel" and "make incision". In

Mild traumatic brain injury (mTBI) is one of the most common neurological injuries, yet its diagnosis remains challenging because of heterogeneous case definitions, the limited sensitivity of conventional imaging, and the nonspecific nature of post-traumatic symptoms. Computed tomography is widely used in acute trauma care to exclude life-threatening intracranial lesions, but it is often normal in patients with mTBI, including those with persistent symptoms. Conventional magnetic resonance imaging (MRI) may improve lesion detection, although many patients still show no clear abnormalities desp

INTRODUCTION: Helicopter Emergency Medical Services (HEMS) exists to supplement the operations of ground-based emergency care providers, mainly in high acuity cases. One of the important procedures frequently carried out by HEMS personnel is endotracheal intubation. Several HEMS providers exist in South Africa, with a mix of advanced life support personnel, however intubation success rates and adverse events have not been described in any local HEMS operation.
METHODS: This was a retrospective chart review of intubation-related data collected by a HEMS operation based in Johannesburg over a 1

INTRODUCTION: Pelvic fractures (PF) occur in up to 9 % of trauma cases, primarily from high-impact events, and are associated with increased morbidity and mortality due to frequent concomitant injuries. Thoracolumbar (TL) spinal fractures, particularly at the T10-L2 junction, are also common in high-energy trauma but are less frequently examined in association with PF. Missed TL fractures can lead to serious neurological deficits. Although Advanced Trauma Life Support (ATLS) recommends screening for TL fractures in trauma patients, no standardized guidelines exist. This study aims to explore t

Sexual and gender minority (SGM) individuals continue to experience disparities in cancer risk, screening, treatment, and outcomes. Despite advances following the 2017 ASCO recommendations, inequities persist, driven by systemic barriers, stigma, and discrimination in health care. State-level and federal-level actions threaten health care access to for sexual minority women (SMW) and transgender and gender-diverse (TGD) people creating additional barriers to safe and evidence-based health care. This review outlines the literature and gives practical recommendations for breast and gynecologic c

INTRODUCTION: Impalement injuries are rare but often fatal injuries due to their complex nature involving multiple organ systems and the risk of massive hemorrhage. Patient outcome depends upon early resuscitation, the location, extent of organ involvement and post-operative care.
PRESENTATION OF THE CASE: We report a case of a 55-year-old man who fell onto an exposed rod from approximately 10 ft in height. On presentation to the Emergency Department in an ambulance, he was hemodynamically stable. He had absent sensory and motor functions below the T7-T8 levels of the trunk. Multiplanar CT sh
