A meta-analysis of weekend admission and surgery for aortic rupture and dissection.

We performed a meta-analysis to determine whether weekend admission and surgery for ruptured abdominal/thoracic aortic aneurysm (RAAA/RTAA) and acute aortic dissection (AAD) is associated with increased mortality. MEDLINE and EMBASE were searched from January 1946 to December 2016 using PubMed and OVID. Eligible studies were prospective or retrospective, comparative or cohort studies enrolling patients admitting or undergoing surgery for RAAA/RTAA/AAD and reporting mortality after weekend (including holiday) versus weekday admission/surgery. Our search identified 11 studies including a total o

Influence of time of delivery on risk of adverse neonatal outcome in different size of delivery units: a retrospective cohort study in Finland.

PURPOSE: To evaluate the impact of time of birth on adverse neonatal outcome in singleton term hospital births. MATERIALS AND METHODS: Medical Birth Register Data in Finland from 2005 to 2009. Study population was all hospital births (n = 263,901), excluding multiple pregnancies, preterm births <37 weeks, major congenital anomalies or birth defects, and antepartum stillbirths. Main outcome measures were either 1-minute Apgar score 0-3, 5-minute Apgar score 0-6, or umbilical artery pH <7.00, and intrapartum and early neonatal mortality. We calculated risk ratios (ARRs) adjusted for maternal ag

Effects of time of hospital admission on outcomes after severe traumatic brain injury in Austria.

BACKGROUND: The goal of this study was to compare outcomes of patients with severe traumatic brain injury (TBI) who had been admitted either during workdays from 7 a.m. to 7 p.m. ("regular service") or during any other time ("on-call service"). MATERIAL AND METHODS: Between March 2002 and April 2012, 17 Austrian centers enrolled TBI patients into two observational studies that focused on effects of guideline compliance (n = 400) and on prehospital and early hospital management (n = 777), respectively. Data on trauma severity, clinical status, treatment, and outcomes were collected prospective

Outcome of patients with ST-segment elevation myocardial infarction undergoing primary percutaneous coronary intervention during on- versus off-hours (a Harmonizing Outcomes with Revascularization and Stents in Acute Myocardial Infarction [HORIZONS-AMI] trial substudy).

Patients with ST-segment elevation myocardial infarction (STEMI) admitted during nonregular working hours (off-hours) have been reported to have greater mortality than those admitted during regular working hours (on-hours), perhaps because of the lower availability of catheterization laboratory services and longer door-to-balloon times. This might not be the case, however, for hospital centers in which primary percutaneous coronary intervention (PCI) is invariably performed. We conducted a substudy using the Harmonizing Outcomes with Revascularization and Stents in Acute Myocardial Infarction

Causes of gastrointestinal colic at an equine referral hospital in South Africa (1998-2007).

The most common causes of gastrointestinal colic at an equine referral hospital in South Africa were determined following retrieval of the medical records of horses admitted during a 10-year study period. The study included 935 horses of which 28% were admitted after hours. Most horses were Thoroughbreds (54%), male (57%), with a mean age of 8.2 years and originated from the Gauteng Province (81%). Heart rate (98%), mucous membrane colour (95%) and auscultation of the abdomen (91%) were the clinical data commonly obtained at admission. Packed cell volume, total serum protein and white cell cou

Absence of a Weekend Effect on In-Hospital Mortality in Upper Gastrointestinal Bleeding: A Multisite Study from an Integrated Health System.

OBJECTIVES: The "weekend effect"-worse outcomes among patients admitted on Saturdays or Sundays-has been reported in upper gastrointestinal bleeding (UGIB) and attributed to delayed endoscopy and reduced after-hours resources. We examined whether weekend admission is associated with worse outcomes within a contemporary integrated health system with continuous endoscopy availability. METHODS: We performed a retrospective cohort study of adults (18-89 years) admitted to Northwell Health hospitals with a primary or secondary diagnosis of UGIB between January 2019 and January 2024. Weekend admiss

Antibiotic prescribing during office hours and out-of-hours: a comparison of quality and quantity in primary care in the Netherlands.

BACKGROUND: Unnecessary and non-first-choice antibiotic prescribing is a significant problem in primary care. It is often argued that irrational prescribing is higher during out-of-hours (OOH) consultations. AIM: To obtain insight into the quantity and quality of OOH antibiotic prescribing for commonly presented infectious diseases. DESIGN AND SETTING: Two two-way comparisons of 1) nationally dispensed antibiotics during office hours and OOH care, using data from the Dutch Foundation of Pharmaceutical Statistics, and 2) regional prescribing quality data from 45 primary care practices from Ut

Off-Hours Endoscopic Management and Clinical Outcomes in Acute Esophageal Variceal Bleeding: A Real-World Cohort Study.

Background and Objectives: Acute esophageal variceal bleeding is a life-threatening gastrointestinal emergency associated with high morbidity and mortality. Endoscopy plays a central role in its management; however, the impact of off-hours endoscopic care on clinical outcomes remains controversial. This study aimed to compare regular-hours and off-hours endoscopic management in patients with acute esophageal variceal bleeding. Materials and Methods: This retrospective single-center cohort study included adult patients who underwent endoscopy for acute esophageal variceal bleeding between Janua

No Weekend or After-Hours Effect in Acute Ischemic Stroke Patients Treated by Telemedicine.

BACKGROUND: Stroke outcomes have been shown to be worse for patients presenting overnight and on weekends (after-hours) to stroke centers compared with those presenting during business hours (on-hours). Telemedicine (TM) helps provide evaluation and safe management of stroke patients. We compared time metrics and outcomes of stroke patients who were assessed and received intravenous recombinant tissue plasminogen activator (IV-tPA) via TM during after-hours with those during on-hours. METHODS: Analysis of our TM registry from September 2015 to December 2016, identified 424 stroke patients who

Impact of disinvestment from weekend allied health services across acute medical and surgical wards: 2 stepped-wedge cluster randomised controlled trials.

BACKGROUND: Disinvestment (removal, reduction, or reallocation) of routinely provided health services can be difficult when there is little published evidence examining whether the services are effective or not. Evidence is required to understand if removing these services produces outcomes that are inferior to keeping such services in place. However, organisational imperatives, such as budget cuts, may force healthcare providers to disinvest from these services before the required evidence becomes available. There are presently no experimental studies examining the effectiveness of allied hea

[Diagnostic accuracy for heart failure – data from the Akershus Cardiac Examination 2 Study].

BACKGROUND: Diagnosing heart failure in an on-call setting can be difficult, and international studies report diagnostic accuracy among duty doctors, as measured using area under the ROC curve (AUC), to be 0.76-0.90. This study has examined the accuracy with which doctors in the internal medicine out-of-hours service in a Norwegian university hospital distinguish heart failure from no heart failure in patients with dyspnoea. MATERIAL AND METHOD: Information was gathered on 468 patients admitted to Akershus University Hospital with dyspnoea between June 2009 and November 2010, and 314 patients

Stroke recognition and early care pathways following telephone triage in out-of-hours primary care: a register-based study from the Central Denmark Region.

BACKGROUND: Out-of-hours primary care (OOH-PC) plays a key role in initial stroke triage, yet the accuracy of telephone assessments by general practitioners (GPs) and their impact on prehospital stroke care remain insufficiently studied. AIMS: To evaluate the sensitivity of stroke and transient ischemic attack (TIA) identification by GPs during OOH-PC telephone triage and examine associations with time from first call to admission, treatment rates, and admission pathways. PATIENTS AND METHODS: We conducted a retrospective cohort study including patients admitted to stroke centers in the Cent

Implementation of an acute care vascular surgery "Surgeon of the Week" model improves efficiency of inpatient care delivery.

OBJECTIVE: Acute care vascular surgery (ACVS) comprises a significant proportion of modern vascular surgery practice. Given the burden of ACVS at our institution, we have transitioned from a traditional "on call" model to a "Surgeon of the Week" (SOW) model, in which a single surgeon covers all daytime inpatient consults and resultant operations over the course of a week. The SOW surgeon has no overnight call, outpatient clinic, and minimizes elective operations during this week, enabling dedicated care to inpatient responsibilities. In this study, we report our 2-year experience with the SOW

The Profile of Women Attending the National Maternity Hospital Emergency Out Of Hours Service – Two Decades On. A Retrospective Review.

Introduction This study aimed to review the profile of women attending an out of hours emergency service (OHS) in a maternity hospital and compare it to a previous study. Methods Retrospective study. Demographic information was recorded including timing of presentation, reason and outcome. Results In 2017, 9,020 women attended the OHS, a 334% increase since 1993. On detailed review of 2,143 charts, most (84.7%, n=1,815) were antenatal. The most common reason for presenting to the OHS then was first trimester bleeding. In 2017, over half of antenatal women (51%, n=1,092) presented with “other”

The impact of weather factors, moon phases, and seasons on abdominal aortic aneurysm rupture.

BACKGROUND: Several studies have documented that weather factors, seasons of the year, time of the day, and even changes in moon phases have an impact on the occurrence of rupture of an abdominal aortic aneurysm (RAAA); however, the available data are confounding. The objective of this study was to determine the impact of these factors on the prevalence and mortality rate of RAAA. METHODS: This is a retrospective analysis of medical records of patients treated for RAAA over a 10-year period. Is pelvic ultrasound associated with an increased time to appendectomy in pediatric appendicitis?

BACKGROUND: Appendicitis is a common pediatric condition requiring urgent surgical intervention to prevent complications. Pelvic ultrasound (US) as a diagnostic aid has become increasingly common. Despite its advantages, evidence suggests US can lead to delayed definitive management. OBJECTIVE: The objective was to test the hypothesis that US is associated with an increased time to appendectomy in children with acute appendicitis. METHODS: A chart review was conducted of all children aged 0-17 years who presented to the pediatric emergency department (ED) with a discharge diagnosis of append
