Incidence Rates of COVID-19-Associated Hospitalization and Risk Factors for Severe Disease Among American Indian and Alaska Native Persons in the Southwest USA and Alaska.

INTRODUCTION: COVID-19 causes significant morbidity in the USA, particularly among American Indian/Alaska Native (AI/AN) persons. Estimates of COVID-19 burden among AI/AN communities are needed to identify health outcome disparities and inform prevention strategies, but under-ascertainment of AI/AN status in national data may result in underestimation of COVID-19 disease burden. METHODS: Surveillance for acute respiratory illness was conducted among AI/AN persons at eight healthcare facilities in Arizona and Alaska to identify COVID-19-associated hospitalizations and outpatient visits. Weekly

Racial misidentification of American Indians/Alaska Natives in the HIV/AIDS Reporting Systems of five states and one urban health jurisdiction, U.S., 1984-2002.

OBJECTIVES: We examined racial misidentification of American Indians/Alaska Natives (AI/AN) reported to the human immunodeficiency virus (HIV)/acquired immunodeficiency syndrome (AIDS) Reporting Systems (HARS) of five U.S. states and one county. METHODS: To identify AI/AN records with misidentified race, we linked HARS data from 1984 through 2002 to the Indian Health Service National Patient Information and Reporting System (NPIRS), excluding non-AI/AN dependents, using probabilistic matching with clerical review. We used chi-square tests to examine differences in proportions and logistic reg

A Study of Multidisciplinary Perspectives on Lung Cancer Prevention Care for American Indian/Alaska Native People.

BACKGROUND: Lung cancer is the leading cause of cancer-related death among American Indian/Alaska Native (AI/AN) people. Lung cancer screening (LCS) remains underutilized among AI/AN people. Multi-disciplinary perspectives can help to inform lung cancer prevention care for this population. METHODS: We conducted a mixed-methods study with healthcare providers and researchers who intersect with AI/AN care. Participants completed an electronic questionnaire and qualitative interviews about their role(s), perceptions of commercial tobacco use and cessation services, and LCS in AI/AN communities.

What Should Physicians Consider About American Indian/Alaska Native Women's Reproductive Freedom?

Historically, American Indians and Alaska Natives (AI/AN) have been subjected to a lack of control over various aspects of their lives, including their reproductive health. In discussions of family planning with AI/AN patients, clinicians must consider past violations of reproductive rights and the need for transparent consent. This article explores the following questions: What were historical violations of AI/AN women's reproductive rights? How should physicians express respect for this history and for the autonomy of AI/AN female patients regarding surgical sterilization procedures today?

Endemic food-borne botulism: clinical experience, 1973-1986 at Alaska Native Medical Center.

Clinical features are described for 35 cases of endemic food-borne botulism managed at a single medical center over 14 years. Important diagnostic findings included dryness of mucous membranes (90%), weakness (84%), and abnormal pupillary reaction (79%). Sensory examination was normal. Dizziness, dyspnea, and weakness of the extremities were significantly more common in patients requiring assisted ventilation. Transitory neurological deterioration occurred in 38% of patients despite the early administration of trivalent (ABE) equine antitoxin. The short course of respiratory failure suggests t

Focus Groups With American Indian/Alaska Native Adults to Discuss Substance Misuse and Access to Services.

BACKGROUND: After the beginning of the COVID-19 pandemic, there was a 53% increase in overdose deaths in the U.S., with the highest rates among American Indian/Alaska Natives (AI/AN) individuals compared with all racial/ethnic groups in the U.S. Historical trauma has been linked to substance use among AI/AN individuals. First cited in the 1990s, historical trauma is an intergenerational response to the loss of AI/AN communities' land, peoples, and culture secondary to colonization in North America. Two-Eyed Seeing is the gift of multiple perspectives treasured by many Indigenous peoples, which

Trait mindfulness as a strategy to mitigate substance use among American Indian/Alaska Native adults.

American Indian/Alaska Native (AI/AN) Peoples have endured centuries of colonization, resulting in lasting mental health effects (historical trauma). Historical trauma has been linked to PTSD symptoms (e.g., grief, anger, hypervigilance) and resulting health inequities. Clinical psychology, particularly the subfield of positive psychology that centers communities, can help address maladaptive historical trauma coping (e.g., substance misuse) for AI/AN Peoples. Mindfulness is a promising intervention strategy that may mitigate the impacts of historical trauma responses among AI/AN Peoples to pr
