Neonatal automated oxygen titration algorithms: response characteristics and performance.

OBJECTIVE: Neonatal respiratory support devices now have algorithms for automated adjustment of oxygen concentration (fraction of inspired oxygen (FiO2)) in response to oxygen saturation (SpO2) disturbances, but their performance has not been systematically examined. We investigated the response of six available devices and algorithms to standardised and simulated SpO2 disturbances. DESIGN: Bench-top study in neonatal centres. METHODS: Devices under study received SpO2 input from a vital signs simulator at 1 Hz; device response was recorded as measured FiO2 in the respiratory circuit. Respon

HFpEF Diagnosis: A Challenge in CKD with Current Algorithms.

BACKGROUND: Chronic kidney disease (CKD) is associated with a high burden of cardiovascular remodeling and increased risk of heart failure with preserved ejection fraction (HFpEF). However, the interpretation of natriuretic peptide-based HFpEF diagnostic remains challenging in CKD populations, where structural cardiac abnormalities and elevated NT-proBNP levels frequently coexist. METHODS: We conducted a cross-sectional study including ambulatory patients with CKD stages G3-G4 and NYHA II dyspnea. Clinical, metabolic, vascular, and echocardiographic assessments were performed. HFpEF was asses

Determining the value of hand surgery through mapping algorithms.

INTRODUCTION: Health state utility values describe the desirability of health states and are needed to calculate quality-adjusted life years. Traditionally, they are derived using generic measures such as the EuroQol 5-Dimension scale. No hand-specific measures can be used to derive them. This study aimed to develop and evaluate mapping algorithms to estimate the EuroQol 5-Dimension utility index from the Patient Evaluation Measure. METHODS: Matched Patient Evaluation Measures and EuroQol 5-Dimension 5-Level data from 2043 patients were used to develop mapping algorithms. Transfer-to-utility

Comprehensive evaluation of optimization algorithms for medical image segmentation.

Medical image segmentation algorithms play a crucial role in assisting healthcare professionals with disease identification, research, and diagnosis. Numerous digital image segmentation methods have been developed, with multilevel thresholding techniques consistently outperforming others in terms of evaluation metrics. The standard algorithms include classical statistical methods, such as the Otsu and Kapur methods, which yield highly accurate results. However, when applied to multilevel thresholding, these methods incur significant computational costs, presenting an optimization challenge. In

Methylation and algorithms in biological aging: a scoping review.

The role of DNA methylation in the process of biological aging is a particularly active area of research, where methylation changes may be a consequence or a driver in the deviation between biological and chronological age. We employ a scoping review strategy to analyze the results of 435 relevant research papers, 167 of which employed methylation-based strategies to interrogate biological age. Our work details the progression and refinement of these strategies over time, as well as the development of novel methylation-based clocks and algorithmic methods. Our chosen review strategy allows for

Reducing Readmission for Sepsis by Improving Risk Prediction Algorithms.

Unplanned readmissions after sepsis, rates of which range from 17.5% to 32%, pose substantial challenges for health care systems. Associated costs for sepsis surpass those for other critical conditions. Existing readmission risk models rely primarily on clinical indicators, which limits their predictive accuracy for patients with sepsis. This review explores how integrating social determinants of health into readmission models can enhance model precision and applicability for predicting 30-day readmission among sepsis survivors. Although socioeconomic status, neighborhood deprivation, and acce

Assessing Deceased-Donor Kidneys Through Posttransplant Survival Prediction Algorithms.

RATIONALE & OBJECTIVE: The Kidney Donor Risk Index (KDRI) is widely used to rank the quality of deceased-donor kidneys and is integrated into the U.S. kidney allograft allocation system. However, the KDRI has modest predictive accuracy for allograft survival, and recent revisions to the KDRI, which removed donor race and hepatitis C virus status, also revealed model calibration problems. This study aimed to evaluate novel approaches for predicting posttransplant allograft survival. STUDY DESIGN: Retrospective cohort study using Organ Procurement and Transplantation Network data from May 1, 20

Temporal robustness of biomarker-based classification algorithms for sepsis.

PURPOSE: Heterogeneity of the host response in sepsis hampers development of effective treatments. Several immunobiologically distinct subphenotypes (or endotypes) have been identified using data-driven analyses of single-timepoint biomarker data, but their temporal stability remains uncertain due to dynamic biology and statistical limitations. METHODS: We analyzed data from 345 sepsis patients across two ICU cohorts. 30 immune biomarkers were measured every 8 h for up to 7 days. Latent profile analysis was used to identify classes upon admission and re-classify patients at later timepoints.

Machine learning algorithms reveal the secrets of mitochondrial dynamics.

Mitochondria exist as dynamic networks whose morphology is driven by the complex interplay between fission and fusion events. Failure to modulate these processes can be detrimental to human health as evidenced by dominantly inherited, pathogenic variants in OPA1, an effector enzyme of mitochondrial fusion, that lead to network fragmentation, cristae dysmorphology and impaired oxidative respiration, manifesting typically as isolated optic atrophy. However, a significant number of patients develop more severe, systemic phenotypes, although no genetic modifiers of OPA1-related disease have been i

Electrocardiographic interpretation of pacemaker algorithms enabling minimal ventricular pacing.

Cardiac pacing from the apex of the right ventricle has been shown to result in left ventricular dysfunction, atrial fibrillation, and increased mortality. To counter these effects, one of the strategies developed is avoidance of ventricular pacing when not necessary, using programmable algorithms to minimize ventricular pacing. Seven algorithms are available from 5 manufacturers. Four of the manufacturers have mode conversion algorithms that pace AAI(R) but, in the presence of failed atrioventricular (AV) conduction, demonstrate algorithm-offset and convert to DDD(R) with ventricular pacing.
