[Anatomy and immunology of the eye].

The eye has all the mechanisms necessary for detection and processing (afferent immune reaction) as well as adequate initiation of an (efferent) immune response. Apart from the typical antigen-processing cells, locally present elements (e.g. glial cells and retinal pigment epithelium) can also be involved in the afferent reaction. For the efferent mechanisms a complex regulative system exists, which includes cellular and humoral responses and is essentially determined by surface molecules. In addition, the ocular environment is rich in immunosuppressive molecules that contribute to the regulat

Anatomy of trisomy 18.

Trisomy 18 is the second most common aneuploidy after trisomy 21. Due to its multi-systemic defects, it has a poor prognosis with a 50% chance of survival beyond one week and a <10% chance of survival beyond one year of life. However, this prognosis has been challenged by the introduction of aggressive interventional therapies for patients born with trisomy 18. As a result, a review of the anatomy associated with this defect is imperative. While any of the systems can be affected by trisomy 18, the following areas are the most likely to be affected: craniofacial, musculoskeletal system, cardia

Patellofemoral anatomy and biomechanics.

Patellofemoral disorders are common. There is a broad spectrum of disease, ranging from patellofemoral pain and instability to focal cartilage disease and arthritis. Regardless of the specific condition, abnormal anatomy and biomechanics are often the root cause of patellofemoral dysfunction. A thorough understanding of normal patellofemoral anatomy and biomechanics is critical for the treating physician. Recognizing and addressing abnormal anatomy will optimize patellofemoral biomechanics and may ultimately translate into clinical success.

[Anatomy of the oesophagogastric junction].

The oesophagogastric junction is increasingly gaining importance due to the difficulty of allocating adenocarcinomas that develop in this region to the stomach or the oesophagus. The present article gives an overview of important anatomic structures in this area, which are responsible for the closure of the distal oesophagus and the entry of the stomach. Moreover, the structures of the mucosa in healthy and under pathological conditions (e.g., oesophagogastric reflux disease and infection with Helicobacter pylori) are discussed in this article.

Anatomy of the visual pathways.

The axons of the retinal ganglion cells form the optic nerve. The two optic nerves meet at the optic chiasm, where the nerve fibers originating in the nasal retina of each eye decussate to join the temporal fibers of the fellow eye. From the chiasm, the same axons continue on as the optic tract. These axons travel to and synapse in the lateral geniculate nucleus, the cells of which send their axons through the optic radiations to the visual cortex. The main blood supply to visual cortex is provided by the posterior cerebral arteries and their branches (the calcarine, posterior temporal, and pa

[Anatomy and physiology of sexuality].

Knowledge of the physiology of male and female sexuality has advanced considerably. Initially there is always desire with its biological neuroendocrine components and its emotional field which is particularly marked in women. There is a distinction between "spontaneous" sexual desire related to intrinsic affective, cognitive stimuli, and fantasies, and "reactive" sexual desire in response to physical arousal. There are similarities between men and women concerning the activation of cerebral zones in sexual arousal contexts in laboratory conditions. The neural pathways for sexual arousal are si

Anatomy of the thymus gland.

In the case of the thymus gland, the most common indications for resection are myasthenia gravis or thymoma. The consistency and appearance of the thymus gland make it difficult at times to discern from mediastinal fatty tissues. Having a clear understanding of the anatomy and the relationship of the gland to adjacent structures is important.

Correlative anatomy for the mediastinum.

Diseases of the mediastinum comprise a wide spectrum of benign and malignant entities that share the same anatomic site within the chest. Correct management often requires a multidisciplinary approach. Diagnostic imaging modalities such as computed tomography (CT), magnetic resonance imaging (MRI), ultrasonography, and positron emission tomography play a major role in the diagnosis of mediastinal diseases and in guiding minimally invasive diagnostic procedures, minimizing the risk of imaging-guided biopsies. This article describes the mediastinal anatomy, correlating the findings of plain radi

Correlative anatomy of the diaphragm.

The diaphragm acts as a partition between the thoracic and abdominal cavities. On computed tomography, it is seen as a curved soft-tissue density with fat below and aerated lung above. The direct multiplanar capability of magnetic resonance technology can improve depiction of normal or abnormal diaphragmatic anatomy. Despite the usefulness of these imaging modalities, adequate visualization of the diaphragm can be difficult. Thoracic surgeons must be familiar with the correlative anatomy of the diaphragm because this knowledge is a prerequisite to making an accurate diagnosis of diaphragmatic

Anatomy of the thoracic duct.

The thoracic duct is a major anatomic structure of the upper part of the abdomen, chest, and the lower part of the neck. This article reviews the embryology, anatomy, and multiple variations of the thoracic duct. Proper knowledge of this anatomy should ease understanding the pathophysiology of diseases involving the lymph channels and also prevent injury to the duct during major procedures in which the duct or its tributaries can be involved.

[Cardiac anatomy and the radiologist].

We report a case of myocardial infarction due to occlusion of the left anterior descending artery seen on a routine chest CT performed in order to exclude aortic dissection. This underscores the need for the radiologist to look at the heart and coronary arteries on each thoracic CT-scan, even if acquired without EKG-gating.

Update on orbital anatomy.

OBJECTIVES: Portal hypertensive gastropathy (PHG) is a diagnosis made based on endoscopic findings in the appropriate clinical setting. Biopsy may be taken during endoscopy for correlation, but the pathologist may encounter a myriad of nonspecific histologic findings. We undertook this study to evaluate contexts where a histologic diagnosis of PHG might be rendered on biopsy. METHODS: Two cohorts were established: stomach biopsy specimens from patients with cirrhosis or undergoing varices screening (n = 188) and stomach biopsy specimens with findings interpreted as PHG in the pathology report

Pelvic anatomy and MRI.

An in-depth knowledge of the anatomy of the pelvis and pelvic sidewall is necessary before a gynaecologist can even contemplate making an initial examination and start management in cases of pelvic pathology or malignancy. This chapter provides basic information on gross pelvic anatomy structures that are of clinical relevance and discusses their correlation with medical imaging, especially magnetic resonance imaging (MRI). MRI is an ideal non-invasive technique in the assessment of normal anatomy and tissue characterization of pelvic pathology. The excellent soft-tissue contrast and the abili
