BACKGROUND: Circumcision is a commonly performed surgical procedure in boys and caudal analgesia is frequently used to minimize discomfort of penile surgery postoperatively. For humanitarian and physiological reasons pain should be anticipated and effectively controlled while ensuring patient safety. Several non-caudal analgesic techniques can be used including: penile block, systemic opioids and topical local anaesthetic cream,emulsion or gel.

BACKGROUND: Clonidine produces analgesia by actions on alpha 2-adrenoceptors and enhances both sensory and motor blockade from epidural injection of local anaesthetics. Low-dose clonidine has been used so far for caudal injection in children. Our aim was to study the perioperative effects of high-dose caudal clonidine when added to low concentration of bupivacaine for combined epidural and general anaesthesia in children.

The authors describe the different forms of postoperative pain therapy which can be applied after a surgical repair of hypospadias. Hereby we chose for the caudal analgesia. It is effective, easy to perform and is seldom associated with complications. An overview is given of the technique, the points for attention and the different products which can be injected. Systemic analgesia and penile nerve blockade are often less effective and should only be applied in case caudal analgesia can not be administered.

In this report we describe a complication of a caudal block in a 4-year-old child with trisomy 13. The patient's history was remarkable for microcephaly, developmental delay, seizures, apnea, and prolonged emergence times. Induction of anesthesia and tracheal intubation were uneventful. We evaluated the effectiveness and safety of ureteroscopic pneumatic lithotripsy (URS-PL) for the treatment of urological calculi under sacral block anesthesia. URS-PL was performed with a semirigid ureteroscope under sacral block anesthesia in 90 patients between January 2006 and April 2010.
