[Effects of lumbar epidural anesthesia on brainstem auditory response].

Auditory brainstem responses (ABR) are used increasingly to monitor the integrity of neural pathways in anesthetized patients. To evaluate the effect of epidural anesthesia on the central nervous system, we studied the effects of lumbar epidural anesthesia with lidocaine on the ABR in 10 patients. The seven peak latencies (waves I-VII) and amplitudes (waves I-VII) of the ABR were recorded before and 15 min after epidural anesthesia. Peak latencies of waves III, IV, V, VI, VII increased after epidural anesthesia compared with control values. Amplitude of waves II only decreased significantly fo

Caudal epidural anesthesia reduces blood loss during hypospadias repair.

We studied 24 boys who were randomized to receive caudal epidural anesthesia with 0.33 ml.kg.-1 0.25% bupivacaine either before (group A) or after (group B) Mathieu repair of distal hypospadias. The 2 groups did not differ in regard to age or weight, and all boys received a standardized anesthetic with halothane and nitrous oxide in oxygen. Intraoperative blood loss was measured with sponge weights and microcalibrated suction canisters. Halothane requirements were reduced in group A (0.5 versus 1.1 plus or minus 0.1%, p less than 0.001). Measured blood loss was reduced in group A (16 plus or m

Retroperitoneal air, extracorporeal shock wave lithotripsy and epidural anesthesia.

During a 13-month period, 1,344 patients underwent extracorporeal shock wave lithotripsy. Plain x-rays done routinely 24 hours after lithotripsy to assess stone fragmentation revealed evidence of retroperitoneal air in 6 patients. All 6 patients had epidural anesthesia induced by loss of resistance to air in a syringe to identify the epidural space. It was believed likely that this was the cause of the air and 2 types of distribution were identified: 1 showed tracking of air along spinal nerves and 1 along tissue planes. This hypothesis eventually was confirmed by 2 patients in whom the air wa

[Lumbar epidural hematoma and spinal abscess following peridural anesthesia].

Epidural application of local anesthetics or opiates is commonly used for treatment of severe pain or arterial obstruction. We discuss two cases of complications following peridural anesthesia. As an acute complication, a lumbar epidural hematoma developed hours after placement of the catheter and caused sciatic pain and nearly complete paraplegia. The hemorrhage might have been promoted by heparinization. After decompressive hemilaminectomy the patient recovered but was disabled by a persistent paresis. The second patient suffered from renal failure, arterial obstruction, and phantom limb pai

Neurotoxic Myelitis Following Accidental Epidural Injection of Chlorhexidine During Obstetric Epidural Anesthesia: A Case Report.

We report a case of neurotoxic myelitis following the accidental epidural injection of chlorhexidine during obstetric anesthesia at a peripheral hospital. A 32-year-old parturient in labor received an epidural catheter for vaginal delivery, during which 3 mL of chlorhexidine was mistakenly injected into the epidural space. This led to progressive paraplegia, severe headaches, and respiratory distress requiring intubation. The patient was subsequently transferred to our anesthesia and critical care department for further management. Radiological findings revealed centromedullary edema and myeli

[A case of general anesthesia combined with epidural anesthesia in a pregnant woman undergoing laparoscopic cholecystectomy].

We report a case of general anesthesia for laparoscopic cholecystectomy at 12 weeks of gestation. A 20-year-old woman weighing 123 kg was admitted with epigastralgia. She was diagnosed as pregnancy of 6 weeks of gestation and acute cholecystitis. Percutaneous trans-gallbladder drainage was performed to delay operation until 12 weeks of gestation. Laparoscopic cholecystectomy was performed uneventfully under general anesthesia combined with epidural anesthesia. There were no clinical signs of fetal distress during the perioperative period.

[Advantage of combined spinal, epidural and general anesthesia in comparison to general anesthesia in abdominal surgery].

Type and technique of anesthesia have an important effect on per operative surgical course. The aim of the study was prospective analyses of advantages of combined spinal, epidural and general anesthesia (CSEGA) versus general anesthesia (GA) in abdominal surgery according to: 1. operative course (haemodynamic stability of patients, quality of analgesia, undesirables effects), 2. postoperative course (quality of analgesia, unfavourable effects, temporary abode of patients in intensive care). Using prospective randomized double blind controlled study, we evaluated two groups of patients whom th

Epidural Anesthesia in a Patient With Turner Syndrome: A Case Report.

Turner syndrome (TS) is associated with short stature, cardiac abnormalities, and musculoskeletal deformity, which pose unique obstetric anesthesia challenges in the small proportion of TS patients who go through spontaneous or assisted pregnancy. There is very limited data available on the anesthetic management of these patients. This case report presents the management of a 32-year-old gravida 2 para 0 (G2P0010) patient with mosaic TS undergoing labor induction at 37 weeks of gestation. An indwelling epidural catheter placed under ultrasound guidance provided adequate labor analgesia with ab

Effect of epidural anesthesia in labor; pregnancy with cardiovascular disease.

OBJECTIVE: We studied the effect and safety of epidural anesthesia during labor in pregnant women with cardiovascular disease. MATERIALS AND METHODS: In the pregnant women with cardiovascular disease, we compared maternal outcome and suppressive effect on blood pressure retrospectively between the epidural group (anesthetized epidurally during labor) and the no-epidural group (not anesthetized epidurally during labor). RESULTS: A total of 277 patients were included in the analyses. Cardiovascular events decreased significantly in epidural group (P < 0.05). Cardiovascular events occurred in 1

Epidural anesthesia and analgesia in the neonate: a review of current evidences.

The role of single shot spinal anesthesia has been established in ex-premature infants at risk of apnea. However, use of epidural anesthesia in neonates is on the rise. In this systematic analysis, we have reviewed the current evidence on the safety and efficacy of the use of single shot and continuous epidural anesthesia/analgesia in neonates. Current clinical practice is guided by evidence based mostly on non-randomized studies, prospective/retrospective case series and surveys. Single shot caudal blockade as a sole technique has been used in neonates mainly for inguinal hernia repair and ci

Impact of morbid obesity on epidural anesthesia complications in labor.

OBJECTIVE: We sought to determine whether morbid obesity is associated with increased maternal hypotension or fetal heart rate (FHR) abnormalities after epidural anesthesia placement during labor. STUDY DESIGN: This was a retrospective cohort study of women undergoing epidural anesthesia during labor at term from April 2008 through July 2010. RESULTS: A total of 125 morbidly obese patients were matched for age and race with 125 normal-weight patients. Morbidly obese patients had more frequent persistent systolic (16% vs 4%, P = .003) and diastolic (49% vs 29%, P = .002) hypotension and more

[Lumbar and thoracic epidural anesthesia in children's hospitals in Japan].

A retrospective study of the lumbar and thoracic epidural anesthesia (catheterization) in infants and children younger than 12 years was undertaken in children's hospitals in Japan. Seventeen institutions replied to our questionnaire and 10 institutions experienced these procedures in the year preceding September 1994. The total number of the lumbar and thoracic epidural anesthesia performed was 324, with average of 32.4 +/- 31.1, and maximum of 113 and the minimum of 2. The number of thoracic epidural anesthesia performed was 56 a much smaller figure than that of lumbar anesthesia. Infants yo
