[Effect of anesthesia on tumor recurrence.].

Cancers cause nearly a quarter of all deaths in Hungary. The long-term success of tumor resection operations, i.e., the absence of recurrence and metastasis as well as survival, are also influenced by anesthetic methods. This was confirmed by experiments on cell cultures and animal models. Propofol and local anesthetics have been shown to reduce tumor cell viability and metastatic potential compared to inhalation anesthetics and opioids. However, studies conducted on patient groups only confirmed the advantage of propofol compared to inhalation anesthetics. Unfortunately, the epidural, additio

Incidence of malignant hyperthermia in Denmark.

Questionnaires were sent to all anesthesia departments in Denmark to determine the total number of anesthetics given per year, and the distribution of different types of anesthesia. All cases of suspected malignant hyperthermia forwarded to the Danish Malignant Hyperthermia Register during a 6.5 yr period were reviewed and divided into subgroups according to clinical criteria. The incidence of suspected malignant hyperthermia in these subgroups was calculated in relation to type of anesthesia. The results are based on information about 386,250 anesthetics and 154 cases of suspected malignant h

Interaction of trichloroethylene with phosphatidylcholine and its localization in the phosphatidylcholine vesicles: a 1H-NMR study.

The interaction of trichloroethylene, an inhalation general anesthetic, with egg yolk phosphatidylcholine (EYPC) and its localization in small unilamellar vesicles (SUV) has been studied by Nuclear Magnetic Resonance. These studies demonstrate that inhalation anesthetic can interact nonspecifically with lipid molecules. Furthermore, it suggests that the action of inhalation anesthetics is directed primarily to the interfacial region of the lipid membrane.

Effects of noxious stimuli and anesthetic agents on substance P content in rat central nervous system.

Effects of noxious electrical tooth stimulations and intraarterial administration of bradykinin or inhalation of volatile anesthetics on substance P content in the diencephalon-mesencephalon, pons-medulla and the spinal cord were examined in the rat. Noxious stimulation by electrical long duration stimulation (type 2) of tooth pulp caused an increase of substance P content in the ponsmedulla. Inhalation of volatile anesthetics such as methoxyflurane and diethylether produced an increase of substance P content in the spinal cord; and in addition, methoxyflurane produced a decrease of substance

The effect of racemic ketamine on the large conductance Ca(+2)-activated potassium (BK) channels in GH3 cells.

Recently, inhalation anesthetics have been reported to block BK channels in adrenal chromaffin cells. To determine if BK block was characteristic only of inhalation anesthetics or was also a property of other general anesthetics we examined the effects of ketamine, an intravenous general anesthetic which is structurally different than inhalation anesthetics. Cell-attached and excised patch single channel and standard whole cell recording techniques were used to examine the effect of racemic ketamine on the BK channel activity in GH3 cells. When solutions containing 150 mM KCl are used in both

Laryngoscopic endotracheal intubation of rats for inhalation anesthesia.

Endotracheal intubation of the rat under direct vision is described together with the details of procedures and apparatus for conducting inhalation anesthesia in this species. Our intubation method requires no special manufacture of equipment, because it employs the human laryngoscope equipped with an infant blade (size 0). Using inhalation anesthetics such as enflurane or halothane for induction, clear laryngoscopic visualization of the glottis is reliably obtained, allowing rapid and routine intubation of the rat in a highly predictable amount of time. In contrast, the injected anesthetics s

Pharmacogenomics in Anesthesia.

A significant number of commonly administered medications in anesthesia show wide clinical interpatient variability. Some of these include neuromuscular blockers, opioids, local anesthetics, and inhalation anesthetics. Individual genetic makeup may account for and predict cardiovascular outcomes after cardiac surgery. These interactions can manifest at any point in the perioperative period and may also only affect a specific system. A better understanding of pharmacogenomics will allow for more individually tailored anesthetics and may ultimately lead to better outcomes, decreased hospital sta

Efficacy of remimazolam in preventing postoperative nausea and vomiting: a systematic review and meta-analysis.

This meta-analysis aimed to evaluate the efficacy of remimazolam, a newly introduced anesthetic, in preventing postoperative nausea and vomiting (PONV) compared to other anesthetics. A systematic search of studies published up to January 2024 in PubMed, Cochrane Library, and Embase was conducted to identify randomized controlled trials (RCTs) comparing remimazolam with other anesthetics in terms of PONV. Following data synthesis, outcomes were reported as the relative risk (RR) with 95% confidence interval (95% CI). Subgroup analyses were performed according to the type of comparator anestheti

Malignant hyperthermia associated with enflurane anesthesia.

Malignant hyperthermia is induced by potent inhalation anesthetics. Enflurane must be added to the list of those anesthetic agents (such as halothane and succinylcholine) that are associated with this condition. The patient in our study was a young woman with no history of prior exposure to general anesthetics, and no family history of complications following administration of anesthetics. The other possible causes of hyperthermia in the patient were investigated and eliminated, and the condition was finally associated with enflurane. Enflurane should not be used in patients with a family hist

[Applied pharmacology of anesthetics].

Inhalation anesthetics, intravenously applied anesthetics, opiates, muscle relaxants and local anesthetics are the most commonly used anesthetic drugs. From the direct anesthetic effects interactions with other organ systems and functions, especially those involved in vital functions such as the pulmonary and cardio-vasculary systems, must be distinguished. Although the safety-margins for individual drugs vary, side effects are minimal on appropriate dosage and use. Substance-related complications, such as severe hepatic damage or malignant hyperthermia, are rare. Procedure-related differences

Anesthetics and excitatory/inhibitory responses of midbrain reticular neurons.

The effects of nitrous oxide, halothane, ether, isoflurane, thiopental, and thiamylal on the excitatory as well as inhibitory responses of single neurons in the midbrain reticular formation (MRF), believed to be one of the most important sites for the regulation of wakefulness, were studied by long-term, extracellular microelectrode recording in cats and rats. All anesthetics except nitrous oxide suppressed the excitatory responses of MRF neurons evoked by somatosensory stimulation. The inhibitory responses markedly were potentiated by both barbiturates but variously affected by other inhalati

The imidazobenzodiazepine Ro 15-4513 antagonizes methoxyflurane anesthesia.

Parenteral administration of the imidazobenzodiazepine Ro 15-4513 (a high affinity ligand of the benzodiazepine receptor with partial inverse agonist qualities) produced a dose dependent reduction in sleep time of mice exposed to the inhalation anesthetic, methoxyflurane. The reductions in methoxyflurane sleep time ranged from approximately 20% at 4 mg/kg to approximately 38% at 32 mg/kg of Ro 15-4513. Co-administration of the benzodiazepine receptor antagonist Ro 15-1788 (16 mg/kg) or the inverse agonists DMCM (5-20 mg/kg) and FG 7142 (22.5 mg/kg) blocks this effect which suggests that the re

Multi-Unit and Multi-Path system of the neural network can explain the steep dose-response of MAC.

PURPOSE: The slope of the dose-response curves of inhalation anesthetics is steep around the minimum alveolar concentration of inhalation anesthetics (MAC) value. Contrastingly, the anesthetic dose-response curves of ion channels and enzymes are gradual. This discrepancy in the steepness may be a key to solve the mechanisms of anesthesia. To explain the steepness we propose a mathematical model of the neural network related to MAC. METHODS: We assumed that, in order to show movement in response to a noxious stimulus, a signal needed to be transmitted from A to B. There are m conduction pathwa

[Anesthetic management of a child with Schwartz-Jampel syndrome].

A 6-year-old child with Schwartz-Jampel syndrome (SJS) underwent tenotomy of bilateral lower limbs under general anesthesia. Patient with SJS has problems such as difficulty of intubation owing to microstomia and jaw muscle rigidity, and is susceptible to malignant hyperthermia by using volatile inhalation anesthetics. In this case, we used a laryngeal mask for airway management and anesthesia was maintained with inhalation of nitrous oxide and continuous i.v. infusion of propofol with caudal block, and his clinical course was uneventful.

Isoflurane activates the type 1 ryanodine receptor to induce anesthesia in mice.

Inhaled anesthetics were first introduced into clinical use in the 1840s. Molecular and transgenic animal studies indicate that inhaled anesthetics act through several ion channels, including γ-aminobutyric acid type A receptors (GABAARs) and two-pore domain K+ (K2P) channels, but other targets may mediate anesthetic effects. Mutations in the type 1 ryanodine receptor (RyR1), which is a calcium release channel on the endoplasmic reticulum membrane, are relevant to malignant hyperthermia, a condition that can be induced by inhaled anesthetics. However, it was previously uncertain whether inhale

[Halothane or enflurane treatment in life-threatening asthma].

We performed inhalation anesthetic therapy in an attempt to produce improvement in cause of life-threatening asthma, which were standard pharmacological therapy. We analysed the results obtained in 6 cases given inhalation anesthetic therapy (4 cases were treated with halothane and 2 cases with enflurane). The following observations were made: 1) The criteria for starting inhalation anesthetic therapy were persistent hypoxycemia or hypercapnia, persistently high inspiratory intra-airway pressure, clinical exhaustion and bronchial toilet with bronchofiberscope. 2) We treated the patients with h

Cardiovascular safety and actions of high concentrations of I-653 and isoflurane in swine.

The ratio of lethal-to-anesthetic concentration can be used to define the margin of safety of an inhaled anesthetic. In mechanically ventilated swine the fatal concentration of I-653, a new inhaled anesthetic, was 23.9 +/- 0.06% (mean +/- SE), and of isoflurane, 6.22 +/- 0.23%. The ratio of fatal anesthetic concentration-to-MAC for I-653 (2.45 +/- 0.11) was less than that determined for isoflurane (3.02 +/- 0.13; P less than 0.01) but relatively greater than that reported previously for other inhaled anesthetics. As with other inhaled anesthetics, the concentration of I-653 causing cardiovascu

[Anesthetic management for bronchoscopy in patients with congenital tracheomalacia].

We experienced the anesthetic management for two patients with congenital tracheomalacia. Inhaled anesthetics are considered to worsen the respiratory condition of tracheomalacia because of its bronchodilating effect. Thus we tried awake intubation in one case, but it was difficult. In another case, we used slow induction with sevoflurane and the trachea was intubated smoothly. Inhaled anesthetics have possibility of worsening the degree of tracheomalacia and have been used very carefully. However, we considered that slow induction with inhaled anesthetics in children with congenital tracheoma
