Effects of antihistamines and local anesthetics on excess tachycardia in conscious dogs.

Atropine produces cardioacceleration in conscious dogs partly by reversing the pre-existing parasympathetic cardioinhibition and partly by inducing "excess tachycardia" (ET). Because ET occurs independently of adrenergic mechanisms, the present study sought to determine whether ET is generated by a positive chronotropic action of endogenous histamine on pacemaker cell H1 receptors. ET was antagonized in conscious dogs by the H1-receptor antagonists pyrilamine, diphenhydramine, chlorpheniramine and promethazine (1-5 mg/kg i.v.), but not the H2-receptor antagonist cimetidine (2-10 mg/kg i.v.). Effect of local anesthetic concentration on the anesthetic level in digital transthecal block.

Transthecal digital block is an effective anesthesia. The purpose of this study was to analyze the effect of different anesthetic concentrations on the anesthetic level of transthecal block. From July 2003 to August 2003, 17 patients with trigger finger received local anesthesia by transthecal digital block. The patients were randomized to receive either 1 mL 1% lidocaine and 0.5 mL betamethasone (7 mg/mL), or 1 mL 2% lidocaine and 0.5 mL betamethasone. Anesthetic intensity was checked over different aspects of digits. The 2% lidocaine group showed significantly better pain scores over the dor

Safety of Additional Local Anesthetics Soon After Liposomal Bupivacaine: A Retrospective Cohort Study.

Introduction Liposomal bupivacaine (LB) produces a prolonged serum bupivacaine level. It carries a warning to avoid additional local anesthetics for 96 hours due to a theoretical additive risk of local anesthetic systemic toxicity (LAST). Whether patients exposed to additional local anesthetics soon after LB have a clinically detectable increased risk of LAST is unknown. Materials and methods Data from a single academic center were used in a cohort study of adults who received LB intraoperatively. The exposure was to additional local anesthetics within 96 hours of LB administration, and the ou

Sustained-release local anesthetics: advances in drug delivery systems for prolonged postoperative analgesia.

Effective perioperative pain control is essential for patient comfort and for optimizing surgical outcomes and the quality of recovery. Multimodal analgesic strategies have become a cornerstone of modern perioperative pain management. Although regional analgesia is widely used within this framework, the clinical utility of conventional local anesthetics remains limited by their relatively short duration of action. Continuous infusion techniques can sustain local anesthetic delivery; however, catheter-based systems have several device- and technique-related limitations. In response, sustained-r

Role of Pharmacology in Dentistry: A Review of Analgesics, Antibiotics, and Local Anesthetics.

This review explores the essential role of pharmacology in dentistry, emphasizing the use of analgesics, antibiotics, and local anesthetics in managing pain, controlling infections, and ensuring procedural comfort. The integration of these pharmacological agents has significantly advanced dental care, allowing for more effective and patient-friendly treatments. Nonsteroidal anti-inflammatory drugs (NSAIDs) and acetaminophen serve as the primary analgesics, while opioids, despite their efficacy in severe cases, require careful prescribing due to their high risk of dependency and adverse effects

Mydriasis mediated by local anesthetics: an unexpected adverse event or new therapeutic indication?

The increasing off-label use of medications needs a robust pharmacovigilance system. This is particularly crucial given the abundance of scientific data that can be harnessed to ensure a product's safety. Our review focuses on the off-label use of local anesthetics, a common practice in topical and intracameral applications. However, the occurrence of mydriasis, as indicated in the monographs/summaries of product characteristics, is an unexpected adverse event. Our aim is to provide a comprehensive understanding of mydriasis caused by local anesthetics, both as an unexpected adverse event and

Local anesthetics allergy in children: Evaluation of diagnostic tests with Real-Life data.

BACKGROUND: Local anesthetic (LA) drugs are commonly used in clinical practice to provide effective analgesia, including in dentistry and minor surgical procedures. The perception of a high risk of allergy in daily applications leads to the referral of atopic patients and those with other drug allergies to allergy clinics for the evaluation of allergic reactions to LA. The aim of this study was to determine who should be referred to the allergy clinic for LA allergy testing, assess the frequency of LA allergy in pediatric patients, and identify the negative predictive value of skin tests in di

The Effect of Local Anesthetics and Contrast Agents on Radiofrequency Ablation Lesion Size.

BACKGROUND: Radiofrequency ablation (RFA) is a validated treatment option for the treatment of chronic pain in patients with lumbar spondylosis. Lesion size has been suggested to correlate with good clinical outcomes. This has created an abundance of scientific interest in the development of products with larger lesion characteristics. Needle characteristics, energy transfer, and heat rate are known to modify lesion size. Here, we demonstrate that common intraoperative solutions, such as lidocaine, iodine, and gadolinium-based products, can also affect lesion shape. OBJECTIVES: To determine w

Outcomes of External Dacryocystorhinostomy under General and Local Anesthetics in a Tertiary Clinic.

OBJECTIVES: In our clinic, although we prefer to perform dacryocystorhinostomy (DCR) under general anesthetics, we also perform it under local anesthetics for patients with a high risk of general anesthesia. Herein this study, we aimed to present our experiences in external DCR under both general and local anesthetics in a tertiary clinic. METHODS: Medical records of the patients who had epiphora, were followed up in the oculoplastic section between January 2014 and December 2020 were collected. Patients who underwent external DCR were included in the study. Patients were divided into two gro

Distinct neurotoxic effects of select local anesthetics on facial nerve injury and recovery.

BACKGROUND: Local anesthetic toxicity has been well-documented to cause neuronal injury, death, and dysfunction, particularly in a susceptible nerve. OBJECTIVE: To determine whether select local anesthetics affect neuron survival and/or functional recovery of an injured nerve. METHODS: This report describes 6 separate experiments that test immediate or delayed application of local anesthetics in 3 nerve injury models. Adult C57/black6 male mice underwent a facial nerve sham, transection, or crush injury. Local anesthetic or saline was applied to the facial nerve at the time of injury (immedi

Reversing Effect of Insulin on Local Anesthetics-Induced Sciatic Nerve Block in Rats.

BACKGROUND: Local anesthetics are used in various purposes from topical and infiltration anesthesia to peripheral nerve or central neural blockade. Even though local anesthetics are relatively safe, they can have some toxic and adverse effects. Prolonged sensory and motor block is another example of an unwanted complication. The primary objective of this study was to determine whether insulin has a reversal effect on the peripheral (sciatic) nerve block with lidocaine or bupivacaine. METHODS: The surgically exposed sciatic nerves in rats were blocked with lidocaine or bupivacaine, and then 0.

MRI verification of ultrasound-guided infiltrations of local anesthetics into the piriformis muscle.

OBJECTIVE: Patients presenting with buttock pain syndromes are common. Up to 8% of these conditions may be attributed to piriformis syndrome. Included in several therapeutic and diagnostic approaches, injections directly into the piriformis muscle may be performed. Because the muscle lies very close to neurovascular structures, electromyographic, fluoroscopic, computed tomographic, and magnetic resonance imaging (MRI) guidance have been employed. In few studies, an ultrasound-guided technique was used to inject a local anesthetic into the piriformis muscle without impairing adjacent neuronal s

A pregnant woman with previous anaphylactic reaction to local anesthetics: a case report.

A 43-year-old woman with a history of lidocaine allergy required dermatologic surgery during pregnancy. To accomplish this without the use of general anesthesia, intradermal allergy testing was performed. This information enabled appropriate anesthetic care for a tissue biopsy at 29 weeks' gestation and subsequent labor analgesia and obstetric management at term.
