Intraoperative latex anaphylaxis compounded by atracurium sensitivity: a case report.

This report describes a life-threatening anaphylactic reaction in an 8-year-old male patient that occurred shortly after intra-abdominal contact with latex surgical gloves. Follow-up skin testing as an outpatient revealed a positive reaction to both latex and atracurium while the other anesthetic agents tested proved negative. For a subsequent operation in which this patient received a kidney transplant, an uneventful anesthetic course was achieved by avoiding the use of latex products and atracurium. Careful planning for that successful procedure included use of vinyl gloves by the surgical t

[Knowledge and implementation of the S3 guideline on delirium management in Germany].

BACKGROUND: Delirium is a common complication in critical care. The syndrome is often underestimated due to its potentially no less dangerous course as a hypoactive delirium. Therefore, current guidelines ask for a structured, regular and routine screening in all intensive care units. If delirium is diagnosed, symptomatic therapy should be initiated promptly. OBJECTIVES: The aim of the current study was to evaluate recent German anesthetists' strategies regarding delirium care compared to the German guidelines for sedation and delirium in intensive care. METHODS: In an online survey German h

Who Remains Intubated After Adult Spinal Deformity Surgery? A 5-Factor Scoring System With Dual Institution External Validation.

BACKGROUND AND OBJECTIVES: Prolonged intubation is an unwanted occurrence after adult spinal deformity (ASD) surgery, yet its risk factors are poorly understood. For patients undergoing ASD surgery, we sought to: (1) report the rate of intubation until postoperative day 1 (POD 1), (2) create a scoring system to predict it, and (3) externally validate the scoring system. METHODS: A retrospective cohort study (2009-2021) was performed for patients undergoing ASD surgery. Primary outcome was prolonged intubation (POD 1 or beyond). Univariate logistic regression were performed. External validatio

Comparison of Glidescope® Go™, King Vision™, Dahlhausen VL, I‑View™ and Macintosh laryngoscope use during difficult airway management simulation by experienced and inexperienced emergency medical staff: A randomized crossover manikin study.

BACKGROUND: In pre-hospital emergency care, video laryngoscopes (VLs) with disposable blades are preferably used due to hygienic reasons. However, there is limited existing data on the use of VLs with disposable blades by emergency medical staff. Therefore, the aim of this study was to compare the efficacy of four different VLs with disposable blades and the conventional standard Macintosh laryngoscope, when used by anesthetists with extensive previous experience and paramedics with little previous experience in endotracheal intubation (ETI) in a simulated difficult airway. METHODS: Fifty-eig

A follow-up survey of total intravenous anesthesia usage in children in the U.K. and Ireland.

BACKGROUND: Total intravenous anesthesia usage in children remains relatively unpopular in the UK and Ireland. A postal survey by Hill et al in 2008 indicated that only 26% of Consultants used a propofol infusion at least once a month. AIMS: Following an increase in teaching and training opportunities in pediatric total intravenous anesthesia in the UK, we repeated the survey among Consultant members of Association of Paediatric Anaesthetists of Great Britain and Ireland and Society for Intravenous Anaesthesia, to see if this had affected total intravenous anesthesia usage in children and how

[Rapid Sequence Induction in pediatric anesthesia].

The induction of anesthesia in patients who are at risk for pulmonal aspiration is a challenge for the anesthetist. The anesthesiological management of adult patients includes strategies to avoid pulmonal aspiration in addition to a rapid, competent protection of the airway without the danger of hypoxia. The well established Rapid Sequence Induction (RSI) in adults cannot be transmitted to children for some reasons. Furthermore many anesthetists are confronted with the problem, that children are infrequent patients in their clinical everyday life, with the lack of routine bustle and nervousnes

Unexpected difficult extubation of double lumen bronchial intubation: a case report.

BACKGROUND: The anesthetist and other members of the perioperative team need to be extremely cautious for successful completion of any surgery. If the final step of any general anesthetic-extubation is not sufficiently well planned, it can lead to critical airway incidents during the extubation and hinder transportation of the patient to the post-anesthesia care unit. CASE PRESENTATION: A 48-year-old female underwent video-assisted thoracoscopic surgery (VATS) combined with left lower lobectomy. The distal end of the left branch of the tracheal tube was lodged by surgical sutures. In this cas

Effect of an aerosol box on tracheal intubation difficulty.

The aim of this study was to determine the effect of an aerosol box on tracheal intubation difficulty. Eighteen experienced anesthetists intubated the trachea of a manikin with a normal airway 6 times using a direct laryngoscope, a McGRATH™ MAC videolaryngoscope, or an airway scope AWS-S200NK videolaryngoscope with or without an aerosol box. Although the aerosol box prolonged the time to successful intubation and decreased the percentage of glottic opening (POGO) score when using a direct laryngoscope, the statistically significant differences were clinically irrelevant. When a McGRATH™ MAC an

Death during or following surgical procedure and the allegation of medical negligence: an overview.

A great variety of mishaps can occur during or following the administration of anesthesia and operative or investigational procedures that do not necessarily convey an error of judgment or negligence on the part of the surgeon or the anesthetist. However, all deaths occurring during the course of anesthesia and surgery or within a reasonable period thereafter (commonly referred to as peri-operative period) have to be reported to the police as these deaths cannot be regarded as natural. Despite the death occurring due to some preexisting disease or some co-existent condition, there may be a ten

A pharmacokinetic and pharmacodynamic study of flunitrazepam.

Flunitrazepam was given as an induction agent i.v. to 12 patients undergoing otologic operations. Pharmacokinetics were evaluated from serum samples at 5 min-72 h by 63Ni-EC-GLC. Pharmacodynamic characteristics were estimated subjectively by simple questionnaire and objectively by the anesthetist during and after anesthesia. The dose of flunitrazepam required for anesthesia varied from 14 to 33 (mean 21) micrograms/kg and was not related to age. The mean distribution volumes of flunitrazepam, calculated by the three-compartment open model, were Vd1 0.61 (SD = 0.36) l/kg, Vd2 1.4 (SD = 0.70) l/

A scavenging double mask to reduce workplace contamination during mask induction of inhalation anesthesia in dogs.

BACKGROUND: Workplace contamination by the use of volatile anesthetic agents should be kept to a minimum if a potential health hazard is to be minimised. Mask induction of animals is a common procedure. The present study investigates the efficiency of a novel scavenging double mask in reducing waste gas concentrations in the breathing zone of the anesthetist performing this procedure. METHODS: Twelve beagle dogs (ASA I) undergoing general anesthesia for a dental procedure were intravenously premedicated with medetomidine and butorphanol (10 μg/kg and 0.2 mg/kg). Anesthesia was induced via a c

A New Old Way to Look at Airway Assessment.

Imagine performing an oral endotracheal intubation with the tongue totally absent from your line of sight of the laryngeal anatomy, including the glottis. The addition of the Angle's Classification of Malocclusion, a tool some may call archaic, should be added alongside airway assessment tools that dental surgeons and nurse anesthesiologists commonly use, such as the Mallampati Classification (or Mallampati Score), Thyromental Distance (TMD), Neck Range of Motion, Size of the Tongue, and Interincisor Distance, also called Intermaxillary Distance.

Videolaryngoscopy vs. direct laryngoscopy use by experienced anaesthetists in patients with known difficult airways: a systematic review and meta-analysis.

Experienced anaesthetists can be confronted with difficult or failed tracheal intubations. We performed a systematic review and meta-analysis to ascertain if the literature indicated if videolaryngoscopy conferred an advantage when used by experienced anaesthetists managing patients with a known difficult airway. We searched PubMed, MEDLINE, Embase and the Cochrane central register of controlled trials up to 1 January 2017. Outcome parameters extracted from studies were: first-attempt success of tracheal intubation; time to successful intubation; number of intubation attempts; Cormack and Leha

The Metamorphosis of NAR to CRNA: The Caterpillar to Butterfly Analogy.

The journey from nurse anesthesiology resident (NAR) to certified registered nurse anesthetist (CRNA) is a profound transformation, one that extends beyond skill acquisition to the evolution of professional identity. Using the metaphor of a caterpillar's metamorphosis into a butterfly, this article explores how academic rigor, clinical immersion, and mentorship shape resilience, adaptability, and confidence in future CRNAs. Grounded in growth mindset theory, it highlights how embracing challenges, seeking feedback, and reframing setbacks as opportunities for learning build both competence and

Beyond the monitors: Anaesthesiologists' experiences of the process of extubation.

BACKGROUND: Although extubation is a high-risk phase associated with risk of severe complications for patients undergoing general anaesthesia, there is a lack of research about this phenomenon from the perspective of anaesthesiologists' experiences of the process of extubation in the anaesthesia setting. AIM: To describe Swedish anaesthesiologists' experiences of the extubation process in the anaesthesia setting. METHODS: A qualitative descriptive design study with individual semi-structured interviews was conducted in three hospitals in Sweden with a total of 17 anaesthesiologists. A qualit
