New advances in amblyopia therapy II: refractive therapies.

The treatment of anisometropic or ametropic amblyopia has traditionally enjoyed a high treatment success rate. Early initiation and consistent use of spectacle correction can completely resolve amblyopia in a majority of patients. For those with anisometropic amblyopia that fail to improve with glasses wear alone, patching or atropine penalisation can lead to equalisation of visual acuity. However, successful treatment requires full-time compliance with refractive correction and this can be a challenge for a patient population that often has one eye with good acuity without correction. Other b

Prevalence of diplopia related to cataract surgery among cases of diplopia.

PURPOSE: To evaluate the prevalence of diplopia in Greece after cataract surgery among diplopia cases. METHODS: The authors retrospectively reviewed the medical records of 571 patients with diplopia over an 8-year period in order to evaluate the prevalence of diplopia related to cataract surgery. The surgical and anesthetic records were also reviewed. All patients had full orthoptic assessment including prisms with cover test in all gaze positions. The chi-square and cross-tabulation statistical tests were used for statistical analysis. RESULTS: Thirty-nine cases (6.8%) were having persisten

[Nd-YAG laser capsulotomy in intraocular lenses with reversed optics].

From a clinico-anatomical point of view the implantation of IOLs with reversed optics approximates natural conditions and has many advantages. The distance between the endothelium or the pigment epithelium of the iris and the front surface of the lens is greater. The refracting surface of the lens is nearer to the nodal point and aniseikonia is reduced. However, it is still not known whether YAG laser treatment to open the posterior capsule is possible under these conditions. The present authors used the pulsed OPL3 neodymium-YAG laser ( Meditec ) to treat 45 eyes for secondary opacification o

Dominant Eye Involvement and Thresholds for Binocular Metamorphopsia in Patients with Unilateral Epiretinal Membrane.

PURPOSE: To evaluate the impact of dominant eye involvement on binocular visual function and vision-related quality of life (VR-QoL), and to determine the monocular metamorphopsia threshold associated with binocular symptom perception in patients with unilateral epiretinal membrane (ERM). METHODS: This prospective single-center study included patients with unilateral ERM. Ocular dominance was determined using the hole-in-card test. Best-corrected visual acuity (BCVA), monocular and binocular metamorphopsia measured with M-CHARTS, aniseikonia, stereopsis, and VR-QoL using VFQ-25 were evaluated

Fall risk in patients with pseudophakic monovision.

OBJECTIVE: Vision changes can precipitate falls in the elderly resulting in significant morbidity and mortality. We hypothesized that pseudophakic monovision and ensuing anisometropia and aniseikonia impact elderly fall risk. This study assessed fall risk in patients with pseudophakic monovision, pseudophakic single vision distance (classic cataract surgery), and cataracts with no surgery. DESIGN: Retrospective single-institution cohort study PARTICIPANTS: Patients with bilateral cataracts diagnosed at 60 years of age or older who underwent bilateral cataract surgery (monovision or single vis

The Changing Natural History of Anisometropia: A Scoping Review.

PURPOSE: To review literature on anisometropia, concentrating on diagnostic criteria, contemporary prevalence and progression with regard to the changing distribution of refractive errors in many countries. Also, to consider anisometropia with respect to myopia and hyperopia control, regions/race/ethnicity, effects on visual function and associated conditions. METHODS: Scoping review based on searches of PubMed, Embase and Cochrane databases. RESULTS: Various diagnostic criteria have been used for anisometropia, most commonly a SER difference ≥1.00 D. Anisometropia is more common in people w

Neuromuscular plasticity and rehabilitation of the ocular near response.

The near response is composed of cross-coupled interactions between convergence and other distance-related oculomotor responses including accommodation, vertical vergence, and cyclovergence. The cross-coupling interactions are analogous to the body postural reflexes that maintain balance. Near-response couplings guide involuntary motor responses during voluntary shifts of distance and direction of gaze without feedback from defocus or retinal-image disparity. They optimize the disparity stimulus for stereoscopic depth perception and can be modified by optically induced sensory demands placed o

Motility and binocularity outcomes in vitrectomy versus scleral buckling in retinal detachment surgery.

BACKGROUND: Ocular motility defects and loss of binocularity are well-recognised problems following retinal detachment surgery. It is presumed that scleral buckling is primarily responsible for these effects. The increasing use of vitrectomy in the management of retinal detachment might be expected to reduce the incidence of these defects. METHOD: Two groups of patients presenting with primary uncomplicated rhegmatogenous retinal detachments were examined following a single surgical repair. The first group underwent vitrectomy (n = 17), the second group, scleral buckling/external surgical tec

The optical design of intraocular lens implants.

Intraocular lens implants have been around for more than 25 years. On the verge of a major breakthrough to ever widening popularity, there is, however, a general lack of understanding of the pertinent optical principles. A critical comparison is made between this new method and conventional methods of correcting aphakia. From this it appears that the visual needs of the patient are a decisive factor in the selection of the method of correction. Moreover, the full potential of intraocular lens implants can only be realized by appropriate selection of the power of the implant in each situation.

The effect of early astigmatism on the visual resolution of gratings.

1. Orientational differences in visual resolution were measured at a number of different luminance levels on two subjects with high astigmatism that had remained optically uncorrected until the age of 10. Because of their astigmatism both of these subjects see vertical contours more clearly than horizontal contours with the unaided eye.2. The measurements were made using sinusoidal gratings generated on the face of an oscilloscope with the refractive error carefully corrected with lenses and with the gratings viewed through 3 mm artificial pupils.3. Visual resolution was found to be much bette

Tolerance to surgically induced anisometropia-A systematic review.

Surgically induced anisometropia is well tolerated by some individuals, while others experience binocular visual complaints. Surgically induced anisometropia is a common treatment for presbyopia and may also intentionally or non-intentionally be the results after surgery for unilateral cataract with ametropia in the fellow eye. Research has shown significant variations in the tolerance of anisometropia. The aim of this systematic review was to examine the correlation between the degree of anisometropia, patient satisfaction and binocular visual complaints. We searched the databases: PubMed, EM

New Onset vs Resolution of Central-Peripheral Rivalry-Type Diplopia in Patients Undergoing Epiretinal Membrane Peeling.

IMPORTANCE: The peeling of an epiretinal membrane (ERM) is commonly performed for poor visual acuity and/or metamorphopsia, but to our knowledge, its influence on central-peripheral rivalry (CPR)-type diplopia has not been rigorously studied. OBJECTIVE: To evaluate the occurrence of either resolution or new-onset CPR-type diplopia in patients undergoing ERM peeling. DESIGN, SETTING, AND PARTICIPANTS: This prospective cohort study was conducted from July 2014 to April 2018 at a tertiary referral medical center and included 33 adults with ERM who were undergoing peeling surgery with planned pr

Binocular Interference vs Diplopia in Patients With Epiretinal Membrane.

IMPORTANCE: Patients with epiretinal membrane (ERM) sometimes close 1 eye for improved vision, but associations have not been rigorously studied. OBJECTIVE: To evaluate associations with monocular eye closure in patients with ERM, and to report binocular interference (closing 1 eye to improve visual quality). DESIGN, SETTING, AND PARTICIPANTS: Retrospective medical record review of an adult strabismus clinic at a tertiary referral center. Patients with ERM referred from retina clinicians between June 2010 and October 2019 who completed the Adult Strabismus (AS)-20 questionnaire, including th
