Approaching Foot and Ankle Injuries in the Ambulatory Setting.

Foot and ankle injuries account for a significant volume of primary care office visits each year. Given the incidence of injury and concern for long-term sequelae, it is imperative that primary care physicians familiarize themselves with commonly encountered foot and ankle injuries. Coupling a sound understanding of key anatomic structures with an appropriately gathered history can help to quickly narrow the differential diagnosis in this clinical presentation. This article focuses on key elements from the history and physical examination as well as provides a concise review of imaging modalit

[Imaging of traumatic injuries of the foot and ankle].

Traumatic injuries of the foot and ankle are frequent and constitute a true public health problem with over 6000 daily cases in France. The complex anatomy of this area and its association to other injuries in polytraumatized patients can lead to delayed diagnosis with worsened functional prognosis. Accurate diagnosis relies on good knowledge of osseous and ligamentous lesions and their associations and sometimes requires the use of additional imaging techniques including US and CT.

Ultrasound of ankle and foot: overuse and sports injuries.

Sports and overuse injuries of the ankle and foot are commonly encountered in clinical practice. Ultrasound (US) has been established as an excellent diagnostic modality for foot and ankle injuries, providing a rapid noninvasive, economical, and readily available tool that is well tolerated by the patient with acute or chronic pain. The opportunity for dynamic examination is another advantage of US in evaluating ankle and foot pathology, where maneuvers such as muscle contraction and stressing of the joint may be particularly helpful. In many cases, US can be used as a first-line and only imag

Imaging of athletic injuries to the ankle and foot.

Conventional radiographs in conjunction with clinical examination remains the primary method for evaluating the acute athletic injury. In most cases, suspected acute tendon and ligament injuries are initially treated based on physical examination. Magnetic resonance (MR) imaging, with its multiplanar capability and superb soft tissue contrast, is quickly becoming the method of choice for evaluating chronic foot and ankle pain and further defining the extent of tendon and ligament injuries. This article reviews the common acute and chronic (overuse) foot and ankle athletic injuries with an emph

Plantar flexion injuries of the ankle. An experimental study.

A biomechanical study was designed to test the hypothesis that isolated plantar flexion of the ankle can produce avulsion fractures of the medial and lateral malleoli without fracture of the posterior articular surface of the tibia. Cadaver specimens were subjected to a standard plantar flexion force in the Burstein torsion testing machine. They showed the following typical patterns of injury in 26 of 32 ankles: (1) avulsion of the anterior ankle capsule; (2) a short oblique distal fibular fracture or fibular avulsion of the anterior talofibular ligament; and (3) an oblique medial malleolar fr

Management and rehabilitation of ligamentous injuries to the ankle.

The management of ligamentous injuries to the ankle is controversial. Neither the methods for classification and diagnosis, or the procedures for treatment are clear cut. Ankle sprains are a common occurrence, with the majority involving the lateral ligament complex. Within this complex, the anterior talofibular ligament is injured most frequently, usually while the foot is in the plantar flexed position. Ankle injuries can be diagnosed through physical exam, including the anterior drawer test and/or a stress exam, or through roentgenographic evaluation. The purpose of the stress roentgenogram

Sprain or fracture? An analysis of 2000 ankle injuries.

A retrospective survey of over 2000 patients with inversion injuries of the ankle joint was undertaken to examine the validity of criteria commonly used in an accident and emergency department to assess severity. Swelling alone is an unreliable indicator of the severity of the injury. Patients with severe pain and inability to weight bear show a high incidence of fractures and must be X-rayed. Conversely, a combination of minimal pain and swelling, and ability to bear weight are indicative of a soft-tissue injury. Young people sustain most inversion injuries and have a lower incidence of signi

[Diagnosis of ligament injuries of the upper ankle joint].

Solitary rupture of ligaments of the upper ankle mostly are caused by injury of supination, more seldom by injury of pronation. The anterior and posterior syndesmosis may be damaged by forced rotation of the foot, if the foot is fixed in 90 degrees or in dorsal extension. Diagnosis of rupture of the fibular ligaments bases on physical examination and is completed by x-ray examination: pressed x-ray pictures in two levels, whereby beside the angle between tibia and talus and beside the anterior displacement of foot the increasing distance between talus and the tip of the fibula is of special im

Problems in the diagnosis and treatment of ankle injuries.

This paper discusses some of the problems encountered in the diagnosis and treatment of 371 ankle fractures treated at the University of Kansas Medical Center over a 14-year period, 1965-1979. Seven of these injuries required major reconstructive procedures after failure of initial treatment and 11 fractures required eventual ankle arthrodesis. The causes of failures of the initial treatment of these 18 cases are discussed. The extent of involvement of the articular surface of the tibia as seen on the original post-injury roentgenogram correlated closely with the extent of subsequent ankle art

Tendon injuries about the foot and ankle in children.

Sixty-four children with tendon injuries about the foot and ankle have been reviewed in order to present a plan of management when such injuries occur. One-third involved the tendo Achillis; the others were equally distributed between the dorsum and sole of the foot, and in only 12% was more than one tendon involved. Sensory nerves were frequently severed, but nearly all recovered completely after early repair. The results of immediate repair of tendons were good. When repair was delayed the results were less satisfactory, as severe deformities developed in the growing feet. Early exploration

Imaging findings of foot and ankle injuries in dancers.

Dancers are performing artists and elite athletes with unique physiological and psychological demands. Many dance styles, especially classical ballet, require extraordinary flexibility and strength throughout the foot and ankle with remarkable pain tolerance. Injuries arising from biomechanical and metabolic stresses negatively impact career progression with resultant chronic pain, permanently impaired function, and financial loss. Therefore, dancers rarely seek medical attention except for genuine problems. The unique demands of dance can result in specific injuries and distinctive radiologic

[Injuries of the foot and ankle in winter sports].

Winter sports injuries of the foot and ankle are a relevant entity due to the high enthusiasm for alpine skiing and snowboarding. The condition of the footwear and rigidity of the binding have a significant influence on the pathobiomechanics of fracture development and have been shown to influence the frequency of knee and ankle injuries. Foot injuries are rare due to the sport-specific footwear with good padding and frequent hard shell casings.

Gunshot-Related Injuries to the Foot & Ankle: Review Article.

Gunshot-related injuries remain a significant and important cause of global morbidity and cost. The foot and ankle has certain anatomic and functional considerations that make gunshot-related injuries challenging to manage. While the scientific literature regarding gunshot-related injuries is extensive, little of the existing literature focuses on the foot and ankle. While principles of management can be somewhat extrapolated from the current literature, an understanding of the body of work specific to the foot and ankle is valuable. Therefore, this review provides an overview of ballistic inj

Management of Syndesmotic Ankle Injuries in Children and Adolescents.

Pediatric ankle injuries are common, especially in athletes; however, the incidence of syndesmosis injuries in children has been scarcely reported. Injuries to the ankle syndesmosis, termed "high ankle sprains," can affect high-level and recreational athletes and have been related to delayed return to play, persistent pain, and adult injuries have been associated with long-term disability. Syndesmotic injuries do occur in children, especially those who participate in sports that involve cutting and pivoting (football, soccer) or sports with rigid immobilization of the ankle (skiing, hockey). [PARTICULAR QUALITIES OF DIAGNOSTIC ACUTE LATERAL ANKLE LIGAMENT INJURIES].

Delayed diagnosis of acute lateral ankle ligaments injury and subsequent inadequate treatment leads to the development of chronic instability and rapid progression of degenerative processes in the joint. The aim of our work was to improve treatment results by developing an diagnostic algorithm and treatment strategy of acute lateral ankle ligament injuries. The study included 48 patients with history of acute inversion ankle injury mechanism. Diagnostic protocol included clinical and radiological examination during 48 hours and after 7-10 days after injury. According to the high rate of inaccu

Analysis of PITFL injuries in rotationally unstable ankle fractures.

BACKGROUND: Reduction and stabilization of the syndesmosis in unstable ankle fractures is important for ankle mortise congruity and restoration of normal tibiotalar contact forces. Of the syndesmotic ligaments, the posterior inferior tibiofibular ligament (PITFL) provides the most strength for maintaining syndesmotic stability, and previous work has demonstrated the significance of restoring PITFL function when it remains attached to a posterior malleolus fracture fragment. However, little is known regarding the nature of a PITFL injury in the absence of a posterior malleolus fracture. The goa
