For immobilization of foot-crus segment in case of damages of talocrural articulation region is proposed a short orthese, made of polyvic. Elastic properties of the material at convergence of the orthese walls allow to develop the compressive force in the region of distal tibiofibular syndesmosis up to 90 N. Immobilization reliability was judged by tensometry. An orthese allows to set the foot subluxation, make reposition of the malleolus fragments. Comparing 20 healthy subjects to 31 injured patients in this study it is demonstrated that ultrasound examination appears to be a reliable tool in the diagnosis of ligamentous inversion force injuries of the ankle. Surgical treatment was applied to 74 cases of ankle joint fractures at the author's hospital, between 1979 and 1984. Follow-up checks were made on 59 patients, within one to seven years from surgery. Luxation fractures accounted for 39 per cent. The rates of wound edge necrosis and bone infection were three per cent each. Results were evaluated by stringent and precisely defined criteria. Deformities of the leg and ankle may result from growth abnormalities of the tibia and fibula. Measurements of the distance between the proximal tibial and fibular physes, the distal tibial and fibular physes, and the angle between the long axis of the tibia and the distal tibial physis and talar dome were made to determine the changes that occur during normal skeletal development. These measurements demonstrate that early detection of growth discrepancy is possible.

We present a check-up study of 47 arthrodeses of the ankle joint, partly in combination with an arthrodesis of the subtalar joint. By resection-compression-arthrodesis following Charnley-Horwitz-Adams we reached a bony fusion of the ankle joint in all cases. 83% of our cases showed a good or at least fair result. The remaining patients complained of painful walking in spite of orthopaedic shoes. In these cases a further fusion of subtalar joints especially of the talo-navicular joint should be considered.

INTRODUCTION: Synovial chondromatosis is characterized by the presence of metaplastic cartilage nodules originating from the synovia, bursa and tendon sheaths. Chronic ankle and subtalar joint (STJ) instability is a common clinical entity and the physician must be able to determine the exact pathologic condition for proper treatment. There are many diagnostic techniques that can be used to evaluate the ankle joint and STJ. These diagnostic techniques do not take the place of a proper patient history taking. Appropriate and aggressive rehabilitation should be attempted in all cases of chronic ankle and subtalar instability before electing surgical reconstruction.

When pain persists and activity is limited as a result of lack of union in a fracture of the upper ankle joint, for us this is a clear indication for a secondary procedure. The goal, of course, is to reconstruct the normal anatomy and correct any deformities present. The time interval between the trauma and secondary reconstruction is no contraindication. The arthrosis present and the patient's age are also not drawbacks. The only contraindication is how severely is function impaired. The final result is not dependent on the degree of malposition nor the arthrosis present before the procedure.
