Cost-effectiveness analysis comparing liposomal anthracyclines in the treatment of AIDS-related Kaposi's sarcoma.

Liposomal formulations have been shown to alter the efficacy and toxicity profiles of anthracylines for patients with HIV-related advanced Kaposi's sarcoma (KS). Using decision-analysis models, the costs and cost-effectiveness of the two U.S. Food and Drug Administration (FDA)-approved liposomal formulations of these agents were estimated. Estimates of costs, effectiveness, and cost-effectiveness were derived from clinical trial data of separate, randomized phase III trials of pegylated liposomal doxorubicin (20 mg/m2 every 3 weeks) and liposomal daunorubicin (40 mg/m2 every 2 weeks). Neoadjuvant anthracycline-containing vs. anthracycline-free chemotherapy with dual HER2 blockade in HER2-Positive breast cancer: a Systematic Review and meta-analysis.

OBJECTIVES: Neoadjuvant chemotherapy with dual HER2 blockade is the standard of care for early-stage HER2-positive breast cancer. However, the role of anthracyclines remains controversial. This meta-analysis aimed to evaluate the efficacy and safety of neoadjuvant dual HER2 blockade combined with anthracycline-containing vs. anthracycline-free chemotherapy. METHODS: A literature search was conducted across PubMed, EMBASE, and the Cochrane Library. Eligible studies included patients with resectable stage I-III HER2-positive breast cancer receiving neoadjuvant dual HER2 blockade with either an

Effects of anthracyclines on arteriosclerosis and the protective effect of dexrazoxane in breast cancer patients.

Anthracyclines are essential agents in the treatment of breast cancer. This study investigated the effect of anthracyclines on arteriosclerosis. The study group consisted of 60 patients not receiving anticoagulants and was divided into two groups: a treatment group (n = 30) receiving dexrazoxane and a control group (n = 30) not receiving dexrazoxane. Basal oscillometric arterial and echocardiographic cardiac parameters were recorded prior to and one month following the completion of chemotherapy protocols. In the dexrazoxane group, only left ventricular end-diastolic pressure increased in pre-

Prediabetes is associated with increased cardiac events in patients with cancer who are prescribed anthracyclines.

BACKGROUND: Prediabetes, which is a precedent of overt diabetes, is a known risk factor for adverse cardiovascular outcomes. Its impact on adverse cardiovascular outcomes in patients with cancer who are prescribed anthracycline-containing chemotherapy (ACT) is uncertain. The objective of this study was to evaluate the association of prediabetes with cardiovascular events in patients with cancer who are prescribed ACT. METHODS: The authors identified patients with cancer who received ACT from 2000 to 2019 from Clinical Data Analysis Reporting System of Hong Kong. Patients were divided into dia

Anthracyclines, Diastolic Dysfunction and the road to Heart Failure in Cancer survivors: An untold story.

Many cardiovascular diseases are characterized by diastolic dysfunction, which associates with worse clinical outcomes like overall mortality and hospitalization for heart failure (HF). Diastolic dysfunction has also been suspected to represent an early manifestation of cardiotoxicity induced by cancer drugs, with most of the information deriving from patients treated with anthracyclines; however, the prognostic implications of diastolic dysfunction in the anthracycline-treated patient have remained poorly explored or neglected. Here the molecular, pathophysiologic and diagnostic aspects of an

Comparison of P-glycoprotein expression and function with in vitro sensitivity to anthracyclines in AML.

The importance of P-glycoprotein (P-gp) in AML has been well documented. Resistance to the anthracyclines can be overcome by several agents including Cyclosporin A (CsA), PSC833 and GF120918. We describe an investigation into the expression, using MRK16 and UIC2, and function of P-gp using daunorubicin with and without modulators by flow cytometric analysis on previously frozen blast cells from 27 patients with primary or secondary AML. We compared this with the in vitro chemosensitivity, using the MTT assay, of fresh blast cells from the same patients. Whilst we found a correlation between P-

Rapid up-regulation of mdr1 expression by anthracyclines in a classical multidrug-resistant cell line.

Studies were carried out in a variant human multidrug-resistant (MDR) cell line CEM/A7R, which expresses very low levels of mdr1 mRNA and P-glycoprotein (P-gp). The induction of mdr1 RNA expression by three anthracyclines, (doxorubicin, daunorubicin, epirubicin), VP-16 and two vinca alkaloids (vincristine, vinblastine) was semiquantitatively assessed by scanning Northern blots on a phosphorimager. The relative level of mdr1 expression was expressed as ratio of mdr1 to the internal RNA (actin). A significant increase (P < 0.02) in expression of mdr1 was noted within 4 hrs of exposure to 1.5 mic

Neoadjuvant anthracycline-free versus anthracycline-containing chemotherapy combined with dual HER2 blockade in HER2-positive breast cancer: a real-world comparative study.

BACKGROUND: Anthracycline-free neoadjuvant chemotherapy combined with dual HER2 blockade represents an alternative to anthracycline-containing regimens in early-stage HER2-positive breast cancer; however, real-world evidence remains limited. METHODS: We retrospectively analyzed 162 patients with stage I-III HER2-positive breast cancer treated with neoadjuvant trastuzumab and pertuzumab between 2022 and 2025. Patients received anthracycline-free (TCHP/TCH; n = 51) or anthracycline-containing (AC-THP/AC-TH; n = 111) chemotherapy. The primary endpoint was pathological complete response (pCR; ypT

Various anthracyclines exhibit differential cytotoxic effects related to CBR1-induced resistance in lung cancer cells.

Anthracyclines are widely used anticancer agents with a complex mechanism of action involving topoisomerase II inhibition and DNA intercalation. Despite their clinical efficacy, their use is limited by cancer cell resistance, linked to the formation of secondary alcohol metabolites via carbonyl reductase 1 (CBR1)-mediated reduction. These metabolites exhibit reduced anticancer activity, positioning CBR1 as significant factor in determining therapy outcomes. The study aimed to elucidate the role of CBR1 in mediating the differential responses of various anthracyclines in cancer cells. The role

Global longitudinal strain decline after anthracyclines in a relatively low-risk Japanese breast cancer cohort.

BACKGROUND: Anthracyclines are known to exhibit dose-dependent cardiotoxicity, leading to cancer therapy-related cardiac dysfunction (CTRCD). The significance of routine global longitudinal strain (GLS) measurements remains uncertain in patients at low or moderate risk for cardiotoxicity. This study aimed to investigate the appropriateness of the contemporary CTRCD surveillance strategy in this population. METHODS: We prospectively enrolled women with breast cancer undergoing anthracycline-based chemotherapy at Keio University Hospital from April 2018 to November 2023. Cardiotoxicity risk was

Late Cardiac Toxicity After Anthracyclines and Radiotherapy for Lymphoma-A Regression Analysis of Dose-Response.

Late cardiac toxicity after radiation therapy and/or systemic therapy with anthracyclines for lymphomas is a serious concern. To estimate the risks with different treatment combinations, reliable estimates of the dose-response relationships are needed. We performed a literature-based regression analysis of long-term cardiac events after lymphoma treatment. The objective was to identify dose-response relationships for anthracycline and radiotherapy regarding congestive heart failure (CHF), ischemic heart disease (IHD), and valvular heart disease (VHD). We included papers published January 2000-

Anthracyclines effect on mitochondrial function and biogenesis in normal blood cells of hodgkin lymphoma patients.

The preferential accumulation of anthracyclines in the mitochondrial matrix has been proposed to trigger a self-perpetuating vicious cycle with mitochondrial DNA (mtDNA) alteration and redox stress enhancing one each other to lead to a progressive mitochondrial impairment. To test this hypothesis, we monitored oxygen consumption rate (OCR) and mt-DNA copy number (mtDNA-CN) of peripheral blood mononuclear cells (PBMCs) harvested from 23 patients with Hodgkin lymphoma (HL) submitted to adriamycin-bleomycin-vinblastine-dacarbazine (ABVD) for the whole treatment, according to a Deauville score ≤ 3

Cardiac function in BRCA1/2 mutation carriers with history of breast cancer treated with anthracyclines.

Animal data suggest that defects in BRCA1/2 genes significantly increase the risk of heart failure and mortality in mice exposed to doxorubicine. Women with BRCA1/2 mutations who develop breast cancer (BC) may receive anthracyclines but their risk of cardiac dysfunction has not been investigated. Our study tested the hypothesis that women with history of BRCA1/2 mutation-associated BC treated with anthracyclines have impaired parameters of cardiac function compared to similarly treated women with history of sporadic BC. Women with history of BC and anthracycline treatment underwent an echocard

Docetaxel-ifosfamide combination in patients with HER2-non-overexpressing advanced breast cancer failing prior anthracyclines.

The feasibility of the docetaxel-ifosfamide combination, as well as the definition of maximum tolerated doses (MTD) in a previous phase I study, led us to continue evaluating the regimen in an extended phase II study in patients with HER2-non-overexpressing, anthracycline pre-treated advanced breast cancer. Patients with histologically confirmed metastatic breast cancer failing prior anthracycline-based chemotherapy were treated with docetaxel 100 mg/m2 over 1 h on day 1 followed by ifosfamide 5 g/m2 divided over days 1 and 2 (2.5 g/m2/day over 1 h), and recycled every 21 days with prophylacti

Comparison of acute effects of anthracyclines on cardiac electrophysiological parameters of isolated guinea-pig hearts.

This study was performed to evaluate the acute effects of two anthracycline derivatives, doxorubicin and 4'O-tetrahydropyranyl-doxorubicin [(THP)-doxorubicin], on the conduction intervals, heart rate and refractoriness of isolated spontaneously beating guinea-pig hearts using a high-resolution ECG recording technique (SST-ECG). Doxorubicin as well as (THP)-doxorubicin were added to the perfusate in increasing concentrations of 0.1, 1 and 10 microM. Doxorubicin did not significantly alter the heart rate or conduction intervals. Only the rate-dependent QT interval was significantly shortened und

Empagliflozin and intermittent fasting as a strategy to mitigate anthracycline-induced cardiotoxicity.

Empagliflozin (EMPA) has shown cardioprotective potential by enhancing myocardial energy availability, a mechanism also observed with intermittent fasting strategies, such as time-restricted feeding (TRF). Given the cardiotoxicity of anthracyclines as Doxorubicin (Dox), integrating pharmacological and nonpharmacological interventions is of growing interest in cardio-oncology. This study assessed EMPA, alone or with TRF, in mitigating Dox-induced cardiovascular damage using both an experimental model and a clinical case. Rats were assigned to control, Dox, Dox + EMPA, Dox + TRF, or Dox + EMPA +
