[Performance of antibiotic prophylaxis in our Urologic Department].

INTRODUCTION: The rate of surgical site infection is a good indicator of the quality of care in surgical departments. Antibiotic prophylaxis represents a measure of proven efficiency in preventing the infection in a surgical site, and its fulfilment is a main goal in management contracts of health services. The efficiency of this preventive measure requires the existence of updated protocols and performance evaluation. OBJECTIVE: To evaluate the compliance of this antibiotic protocol in our Urology Department with the global analysis of the causes of inadequate prophylaxis and by procedures.

Current practice of antibiotic prophylaxis for catheter procedures.

The practice of giving prophylactic antibiotics to patients at the time of urinary catheter insertion, change or removal is variable since guidelines for their use have yet to be established. The use of prophylactic antibiotics to prevent urinary catheter-related infections and the possibility of bacteraemia and septicaemia, despite a lack of evidence for their efficacy, is a matter of concern in light of the reported overuse of, and increased resistance to, antibiotics. This article describes an audit of, and increased resistance to, antibiotics. This article describes an audit conducted in o

Surgical antibiotic prophylaxis in a Turkish university hospital.

A prospective study was performed to assess the practice of antibiotic prophylaxis for surgical procedures in Pamukkale University Hospital, Denizli, Turkey. All surgical procedures performed between April 1 and July 31, 2001, were included. During the study period 897 operations were reviewed. 96% of all procedures were elective, 4% emergencies. Approximately 70.7% were clean surgery, 25.3% clean-contaminated, 2.8% contaminated, and 1.2% dirty. 98% of patients (879) received antibiotic prophylaxis. Although timing of prophylaxis was appropriate in all procedures, the duration of prophylaxis w

Antibiotic prophylaxis regimens and drugs for cesarean section.

BACKGROUND: Prophylactic antibiotics for cesarean section have been shown to reduce the incidence of maternal postoperative infectious morbidity. Many different antibiotic regimens have been reported to be effective. OBJECTIVES: The objective of this review was to determine which antibiotic regimen is most effective in reducing the incidence of infectious morbidity in women undergoing cesarean section. SEARCH STRATEGY: We searched the Cochrane Pregnancy and Childbirth Group trials register and the Cochrane Controlled Trials Register. The date of the most recent search was October 1998. SELE

Prospective analysis of antibiotic prophylaxis for orthognathic surgery.

A randomized prospective study of 30 patients was undertaken to evaluate antibiotic prophylaxis for orthognathic surgery. Perioperative, high-dosage antibiotic administration was compared to an existing prophylactic regimen employing longer intervals and continued postoperative antibiotics for 7 to 10 days. No significant difference was found between the two groups with respect to incidence of infection. Other variables studied included gender and age of patients, type of surgery performed, procedure length, and use of fixation.

Single-dose peroperative antibiotic prophylaxis in gastrointestinal surgery.

A single intravenous dose of tobramycin and lincomycin, given at the start of gastrointestinal operations, significantly reduced the incidence of postoperative wound infection from 34% to 5%. The occurrence of both anaerobic and aerobic bacteria was reduced. Therapeutic concentrations of the antibiotics were maintained throughout the operative period in most cases. No toxic effects of the antibiotics were detected, no anaesthetic complication occurred, and resistant strains of bacteria normally sensitive to the antibiotics were not isolated from wounds.

Indications for systemic antibiotic prophylaxis in neutropenic patients.

Four patients with malignant blood disease and neutropenia developed apparently localized infective lesions of the skin or conjunctiva from which Pseudomonas aeruginosa was cultured. In spite of appropriate local antimicrobial treatment all patients developed pseudomonas septicaemia within a short period and three died. It is argued that the isolation of pseudomonads from apparently localized lesions in patients with compromised antibacterial defences is an indication for systemic antibiotic therapy.

A Cost-effectiveness Analysis of Antibiotic Prophylaxis versus No Antibiotic Prophylaxis for Postoperative Infectious Complications for Hand-assisted Laparoscopic Donor Nephrectomy.

OBJECTIVES: Individuals who volunteer to undergo hand-assisted laparoscopic donor nephrectomy (HALDN) to help those needing renal transplants, face postoperative infection complications (POICs) risks for no corresponding clinical benefit. Prophylactic antibiotics often control POIC risk; however, there is no clear consensus on their use in HALDN. Considering the incidence of POICs, National Health Service (NHS) resource constraints, and antimicrobial stewardship priorities, a cost-effectiveness analysis (CEA) was conducted to evaluate the economic impact of prophylactic antibiotic use in HALDN

Perioperative selective decontamination of the digestive tract and standard antibiotic prophylaxis versus standard antibiotic prophylaxis alone in elective colorectal cancer patients.

INTRODUCTION: Infectious complications and especially anastomotic leakage (AL) severely impede the recuperation of patients following colorectal cancer (CRC) surgery. When the normal gut barrier fails, as in AL, pathogenic microorganisms can enter the circulation and may cause severe sepsis which is associated with substantial mortality. Moreover, AL has a negative impact on the CRC prognosis. Selective decontamination of the digestive tract (SDD) employs oral nonabsorbable antibiotics to eradicate pathogenic microorganisms before elective tumour resection. METHODS: In this multicentre random

To give or not give antibiotic prophylaxis for planned caesarean section: An assessment of risk of postpartum prescription of antibiotics depending on prophylaxis regimen.

INTRODUCTION: No national recommendation for antibiotic prophylaxis for planned caesarean section in Sweden exists. Swedish delivery units have chosen different strategies. Some units routinely administer antibiotic prophylaxis to all women for whom a caesarean section is planned, while other units give prophylactic antibiotics to risk groups only. We took advantage of this "natural experiment" to compare the risk of antibiotic prescription postpartum after planned caesarean section, by antibiotic prophylaxis strategy. MATERIALS AND METHODS: An observational study based on data from Swedish r

Multicentre randomised double-blind placebo controlled trial of combination vancomycin and cefazolin surgical antibiotic prophylaxis: the Australian surgical antibiotic prophylaxis (ASAP) trial.

INTRODUCTION: Resistant Gram-positive organisms, such as methicillin-resistant staphylococci, account for a significant proportion of infections following joint replacement surgery. Current surgical antimicrobial prophylaxis guidelines recommend the use of first-generation or second-generation cephalosporin antibiotics, such as cefazolin. Cefazolin, however, does not prevent infections due to these resistant organisms; therefore, new prevention strategies need to be examined. One proposed strategy is to combine a glycopeptide antibiotic with cefazolin for prophylaxis. The clinical benefit and

Antibiotic prophylaxis in orthopaedic surgery: difficult decisions in an era of evolving antibiotic resistance.

Prophylactic antibiotics can decrease the risk of wound infection and have been routinely employed in orthopaedic surgery for decades. Despite their widespread use, questions still surround the selection of antibiotics for prophylaxis, timing and duration of administration. The health economic costs associated with wound infections are significant, and the judicious but appropriate use of antibiotics can reduce this risk. This review examines the evidence behind commonly debated topics in antibiotic prophylaxis and highlights the uses and advantages of some commonly used antibiotics. Cite this

Antibiotic prophylaxis in surgery for closed fracture of the hand.

PURPOSE: Preoperative antibiotic prophylaxis is associated to internal fixation for closed phalangeal and metacarpal fracture, but its effectiveness is not known. METHODS: In a consecutive series of 119 adult patients undergoing Kirschner-wire fixation for phalangeal or metacarpal fracture, 56.3% (n = 67) received antibiotic prophylaxis and 43.7% (n = 52) did not. RESULTS: The rate of deep surgical site infection was 1.5% (n = 1) in the group with antibiotic prophylaxis and 1.9% (n = 1) in the group without. Minor skin irritation or infection of the pin tract occurred in 13.4% of cases (n =

Duration of Antibiotic Prophylaxis for Ballistic Fractures to the Extremities.

BACKGROUND: In the U.S. there are thousands of ballistic firearm injuries to the extremities and subsequent infections, yearly. There is a lack of consensus regarding the ideal duration of antibiotic treatment to prevent infection of these wounds. Our study investigated infection rate among ballistic extremity fracture patients based on antibiotic prophylaxis duration, operative management, wound severity, and fracture location. MATERIALS AND METHODS: Retrospective chart review of ballistic extremity fracture patients from a single trauma center from 01/01/2010 to 12/31/2020. RESULTS: Of 161

Ocular toxoplasmosis: background and evidence for an antibiotic prophylaxis.

PURPOSE OF REVIEW: The purpose of this review was to provide an overview of current data on antibiotic prophylaxis in ocular toxoplasmosis. RECENT FINDINGS: Studies showing the prophylactic effect of long-term antibiotics are discussed. Prophylaxis seems to be justified in patients with a high risk of recurrence because of antibiotic's potential side-effects. Therefore, predisposing factors leading to a higher risk of recurrence and the time period during which an antibiotic prophylaxis is most appropriate are reviewed. Finally, a patient-individualized treatment recommendation is summarized.

Antibiotic prophylaxis in obstetric and gynaecological procedures: a review.

Surgical site infections are a common complication of obstetric and gynaecological surgeries; up to 10% of gynaecological patients undergoing an operative procedure will develop a surgical site infection. In surgeries with high rates of post-operative infection, antibiotic prophylaxis (using an antibiotic with an appropriate microbiological spectrum and administered in a timely manner) can play a major role in improving outcomes. This review examines the medical literature to assess the indications and appropriate antibiotic choices for prophylaxis to prevent surgical site infection in obstetr
