Transdermal oxybutynin in the treatment of overactive bladder.

Overactive bladder is a dreadful syndrome that affects a considerable number of patients. Antimuscarinics are the mainstay of pharmacotherapy for this condition. Transdermal (TD) oxybutynin (OXY) bypasses the first-pass metabolism and reduces the formation of N-desethyloxybutynin, a compound believed to be associated with anticholinergic side effects. The 3.9 mg matrix TD system is applied twice weekly and transports OXY directly into the systemic circulation. The patch can be applied to abdomen, buttock, and hip, and provides continuous OXY delivery that minimizes peak and trough fluctuations

"Apraxia of lid opening," a focal eyelid dystonia: clinical study of 32 patients.

We have seen 32 patients with "apraxia of lid opening" (ALO) in the following clinical settings: as an isolated condition (3 patients), idiopathic blepharospasm (BSP, 20 patients, including 4 familial cases), progressive supranuclear palsy (PSP, 7 patients), and dystonic parkinsonian syndrome (2 patients). Twenty-nine patients treated with botulinum toxin into the orbicularis oculi muscle were rated before and after treatment and 83% of the patients improved on a clinical scale. Best results were obtained with injections directed toward the junction of the preseptal and pretarsal parts of the

Paroxysmal oculogyric dystonia associated with a de novo 3q29 microdeletion.

3q29 deletion syndrome is caused by a heterozygous 1.6 Mb deletion on chromosome 3, which occurs in about 1 in 30 000 births. Phenotypic features of this syndrome include mild-to-moderate intellectual disability, autism spectrum disorder, slightly dysmorphic facial features, ataxic gait, and chest-wall deformity. Gastrointestinal disorders, dental abnormalities, feeding problems during infancy, recurrent ear infections, and heart defects have also been observed. Since the incidence of the deletion is rare, the phenotype has not been fully described, particularly in adults. This report describe

New approaches in the treatment of the dystonias.

At this point the treatment of dystonias remains highly empirical. Secondary dystonias, especially those related to specific drug treatment, have to be ruled out carefully. A few dystonic subgroups respond well to levodopa medication. In the other syndromes, anticholinergics are the usual first choice. In focal conditions botulinum toxin injections seem to be the most effective regimen, although there are only a few long-term studies. Surgical procedures are an ultimate option.

Behavioural problems associated with dementia: the role of newer antipsychotics.

Behavioural disorders are a common feature in dementia, especially in the later stages of the disease. The most frequent disorders are agitation, aggression, paranoid delusions, hallucinations, sleep disorders, including nocturnal wandering, incontinence and (stereotyped) vocalisations or screaming. Behavioural disorders, rather than cognitive disorders, are the main reason why caregivers place patients with dementia in a nursing home. However, although behavioural disorders are important, there is still no international agreement with respect to the description and definition of symptoms and

A practical approach to managing agitated toxidromes.

Agitated patients may present with life-threatening emergency. Differential diagnoses span a wide range, conveniently remembered by the acronym VITAMINS (Vascular, Infections, Toxins/Trauma, Auto-immune/-inflammatory, Metabolic, Iatrogenic, Neoplastic, and pSychiatric aetiology). Common toxidromes include anticholinergic syndrome, serotonin toxicity, sympathomimetic toxicity, neuroleptic malignant syndrome, and withdrawal from alcohol or sedative-hypnotics. Diagnosis is primarily clinical, based on exposure history and characteristic signs. A focused physical examination is often sufficient wh

[Maintenance of the efficacy of the oxybutynin patch for the treatment of overactive bladder syndrome despite the change in the area of application: study of four cases.].

The transdermal route for administration of anticholinergic drugs can provide efficacy with less systemic adverse effects. The transdermal oxybutynin patch (OXYTDS) offers advantages over oral administration for patients treated for Overactive bladder (OAB) syndrome. The limited evidence on the OXY-TDS patch application areas of the skin, makes difficult counseling patients who require a change to skin zones other than those recommended by the manufacturer. The preliminary experience of four patients included in this case report suggests that changing the application area for the OXY-TDS patch

Scopolamine fails to diminish chronic haloperidol-induced purposeless chewing in rats.

Chronic haloperidol treatment for 4-12 months gradually induces spontaneous, irregular, purposeless oral chewing movements (CMs), apparently involuntary, in some but not all treated rats. Based on phenomenologic and pharmacologic similarities, this laboratory preparation has been used as an animal model of tardive dyskinesia (TD), which is the human hyperkinetic motor syndrome associated with chronic antipsychotic administration. This putative animal model has received the most severe challenge to its validity by claims that its oral movements can be suppressed by anticholinergic treatments, s

Wells syndrome (eosinophilic cellulitis): Proposed diagnostic criteria and a literature review of the drug-induced variant.

BACKGROUND: Wells syndrome is an uncommon inflammatory dermatosis first described in 1971 by Wells. The clinical eruption is characterized by varying morphology and severity and usually follows a relapsing remitting course. The majority of the reported cases are of unknown etiology, drug induced Wells syndrome has rarely been reported. A literature search using MEDLINE was performed. We recorded the features of our case and of the additional cases of drug induced Wells syndrome in the literature. MAIN OBSERVATIONS: Including our case there are 25 cases of drug-induced Wells syndrome reported.

Association of Drug Burden Index with grip strength, timed up and go and Barthel index activities of daily living in older adults with intellectual disabilities: an observational cross-sectional study.

BACKGROUND: Drug Burden Index (DBI), a measure of exposure to medications with anticholinergic and sedative activity, has been associated with poorer physical function in older adults in the general population. While extensive study has been conducted on associations between DBI and physical function in older adults in the general population, little is known about associations in older adults with intellectual disabilities (ID). This is the first study which aims to examine the association between DBI score and its two sub-scores, anticholinergic and sedative burden, with two objective measure

Treatment of Primary Biliary Cholangitis including Transplantation.

Ursodeoxycholic acid (UDCA) is the first-line treatment of primary biliary cholangitis (PBC). Long-term UDCA use significantly reduces progression to cirrhosis. UDCA improves liver enzymes and transplant-free survival rates. Despite the association between PBC and hyperlipidemia, treatment is indicated under specific circumstances with statins and fibrates being safe options. Osteoporosis, which is frequently seen, is usually managed based on data from postmenopausal women. Sicca syndrome is treated similarly to its standalone condition with the use of hydroxypropyl methylcellulose eye drops a

Peppermint oil in irritable bowel syndrome.

In a literature search 16 clinical trials investigating 180-200 mg enteric-coated peppermint oil (PO) in irritable bowel syndrome (IBS) or recurrent abdominal pain in children (1 study) with 651 patients enrolled were identified. Nine out of 16 studies were randomized double blind cross over trials with (n = 5) or without (n = 4) run in and/or wash out periods, five had a randomized double blind parallel group design and two were open labeled studies. Placebo served in 12 and anticholinergics in three studies as comparator. Eight out of 12 placebo controlled studies show statistically signific

The psychopharmacology of primary and metastatic brain tumors and paraneoplastic syndromes.

Brain tumors and paraneoplastic syndromes can cause various neuropsychiatric symptoms. Rarely, psychiatric symptoms may be the initial presentation of the underlying neurologic lesion. Brain imaging studies are crucial in the diagnosis of brain tumors. Paraneoplastic syndromes are mostly immune-mediated, and antineuronal antibodies may be detected in the blood or cerebrospinal fluid. Clinical suspicion is very important in assisting the diagnostic workup. Treatment of the psychiatric symptoms depends on the nature of the symptoms. Selection of the psychotropic agent has to be done carefully to

Acute psychosis and serotonin syndrome in the setting of "Triple-C" overdose: a case report.

BACKGROUND: Over-the-counter medication overdose is a difficult diagnostic challenge for many physicians as common drug screening assays cannot detect these substances. We present a case of acute psychosis, serotonin syndrome, and anticholinergic overdose-like properties in the setting of Coricidin HBP Cough & Cold tablets, known by their street name Triple-C. This is the first case report we are aware of involving a patient presenting with these symptoms and requiring critical-care-level support. CASE PRESENTATION: A 31-year-old African American female with a past medical history of anxiety,

[Possibilities of treatment of dystonic syndromes with akineton].

Treatment of dystonia is a complex problem of current neurology due to the etiological and neurochemical heterogeneity of this clinical syndrome. Central cholinolytics is a most effective group of drugs for patients with dystonia and dystonic tremor. The authors present the results of the successful treatment with biperiden (akineton), a centrally active anticholinergic drug with additional peripheral choline- and ganglion-blocking effect in cervical dystonia. The time of response to treatment and duration of clinical effect, its possible predictors are analyzed. Perspectives of using cholinol

Dystonia update.

Dystonia is a syndrome of sustained muscle spasms of presumed central nervous system origin. Recent advances in molecular biology have permitted clearer understanding of the genetics of various forms of dystonia and suggest pathophysiological deficits at the origin of the clinical signs. Treatment has involved centrally-acting drugs, specifically the anticholinergic medications, as well as peripherally acting agents that block neuromuscular transmission (botulinum toxin). Some forms of dystonia are particularly responsive to levodopa. A systematic approach to the diagnostic and treatment evalu

Risk for definite neuroleptic malignant syndrome. A prospective study in 223 consecutive in-patients.

The occurrence of neuroleptic malignant syndrome (NMS) was studied prospectively in two series of consecutive psychiatric in-patients (n = 223). The first group (n = 120) suffered from schizophrenia and was treated only with haloperidol. The second group (n = 103) was treated with diverse neuroleptics. All patients were on a single antipsychotic agent with no anticholinergic drug as prophylaxis. The incidence of full NMS per admission and first neuroleptic exposure was 5/223 (2.2%). Patients with bipolar affective disorder and those treated with injections were significantly over-represented i

Bromocriptine, physostigmine, and neurotransmitter mechanisms in the dystonias.

We studied eight patients with different types of dystonia by administering agonists and antagonists of acetylcholine (ACh) and dopamine (DA). In all seven patients who received physostigmine, there was an exacerbation of the dystonia. All six patients who received anticholinergic agents improved, as did five of eight who received bromocriptine. Neuroleptics worsened symptoms in four patients with generalized dystonia and improved symptoms in one patient with Meige syndrome. Our results suggest that DA-ACh imbalance may underlie the pathophysiology of dystonia in some patients and that bromocr
