Patients with Parkinson's disease (PD) may develop progressive dementia late in their clinical course. Dementia in PD is mostly related to neuropathological findings of extensive Lewy bodies (LBs), with or without the coexistence of Alzheimer's disease (AD) pathology. Aphasia has been reported in patients with LB diseases with AD pathology; however, there have been no reports of typical PD patients developing progressive aphasia during their clinical course. We describe a female PD patient who later developed progressive conduction aphasia characterized by phonemic paraphasia and disturbance i

This article explores how consideration of acquired speech and language disorders from the perspective of neuroscience permits new insights into the content and design of therapy for people with aphasia. Key proposals are that aspects of current therapies often neglect the sensory-motor components of speech and language processing, and the interconnectivity of sensory-perceptual and motor systems. Furthermore, current therapy regimes are often administered at too low an intensity to stimulate neural reorganization. Neuroscientific perspectives on learning are explored and in particular the iss

We describe a brief pilot study undertaken to investigate the potential benefit(s) of using a SenseCam in aphasia therapy. Five post-stroke persons with aphasia and their caregivers agreed to participate. Each person with aphasia wore the SenseCam for 1 day during the daytime. Slide shows and printed images were created from the images obtained and presented at a (videotaped) weekly group conversation session. Therapists' observations, reflections, and opinions were subsequently elicited in a group interview and online survey. Wearable, sensor-triggered automatic imaging devices offer potentia

At 45 years of age, Charles Baudelaire suffered a left hemispheric stroke that left him with a right hemiplegia and severe aphasia. In this chapter, we investigate the nature of his symptoms, drawing mostly on his own and his contemporaries' correspondence. Before specifically examining his aphasia, we put the poet's life, work, and health in context, notably his tormented mind, his probable syphilitic infection and the intellectual milieu of 19th century France. The time when Baudelaire was struck with aphasia coincides with early discoveries and debates that centered on the nature and implic

Ten autopsied patients from a prospectively followed, clinically defined, neuropsychologically and radiologically documented cohort with primary progressive aphasia were histologically characterized. All were variants of frontotemporal degeneration (Pick complex): Pick body dementia, n=3, corticobasals degeneration (CBD), n=4, and tau and synuclein negative ubiquitinated inclusions of the motor neuron disease type, n=3. All shared superficial cortical spongiosis, neuronal loss, and gliosis. Although most patients had fluent anomic aphasia at onset, all progressed to a nonfluent or mute state.

OBJECTIVE: To review the change in number of referrals to speech and language therapy for dysphasia and dysphagia over one decade.
DESIGN: This retrospective study analyses data collected on an information system developed specifically for the use of speech and language therapists in the management of their services.
SETTING: Eleven health care providers used the system between 1985 and 1995 and recorded details of 73758 patients referred to speech and language therapy during that period.
RESULTS: Whilst there were 12 times as many dysphasic patients as dysphagic patients referred to speech

Transcortical sensory aphasia (TSA) is characterized by impaired auditory comprehension with intact repetition and fluent speech. We induced TSA transiently by electrical interference during routine cortical function mapping in six adult seizure patients. For each patient, TSA was associated with multiple posterior cortical sites, including the posterior superior and middle temporal gyri, in classical Wernicke's area. A number of TSA sites were immediately adjacent to sites where Wernicke's aphasia was elicited in the same patients. Phonological decoding of speech sounds was assessed by audito

It has long been a matter of debate whether recovery from aphasia after left perisylvian lesions is mediated by the preserved left hemispheric language zones or by the homologous right hemisphere regions. Using PET, we investigated the short-term changes in the cortical network involved in language comprehension during recovery from aphasia. In 12 consecutive measurements of regional cerebral blood flow (rCBF), four patients with Wernicke's aphasia, caused by a posterior left middle cerebral artery infarction, were tested with a language comprehension task. Comprehension was estimated directly

A patient, aged 58, with sudden loss of speech and right-sided haemiparesis is described. CT brain showed intracerebral haemorrhage in the left thalamus. The examination of speech functions established aphasia characterized by anomia, comprehension disorders, paraphasia, perseveration, reduction of spontaneous speech, dyslexia and agraphia. Relation between thalamic aphasia and cortical aphasic syndromes is discussed. The obtained results are in agreement with the theory that emphasizes a specific role of thalamus in verbal communication.

The response of five adults with chronic aphasia to a 4-week intensive treatment course is presented. Using four language tests, pre-intervention stability was demonstrated over the month preceding the treatment programme. Following the intervention period, two subjects showed improvement in one test, and more widespread changes occurred in three subjects. On further assessment 1 month after treatment had finished, one of the two subjects who had made least progress performed better than at the end of the course, and one had returned to pre-treatment level. Although there were indications of r

The very rare aphasia following damage to the right cerebral hemisphere in right-handed subjects is described, with a survey of the views on this problem and presentation of two cases of this so-called crossed aphasia in right-handed subjects. On one of these cases the lesion was cortico-subcortical in a part of the frontal lobe, temporal lobe and a part of the occipital lobe, with sparing of the deep structures of the right hemisphere and with clinical pattern resembling that of Broca aphasia. In the second case the lesion involved the subcortical and deep structures with destruction of the l

A 74-year-old, right-handed woman suffered acute aphasia and left hemiplegia secondary to a cerebral infarction in the right cerebral hemisphere. The lesion was located deep in the parietal lobe and extended to the posterior limb of the internal capsule and the head of the caudate nucleus. The patient's aphasia was characterized by severe impairment in auditory and visual comprehension and auditory retention span, as well as by anomia, agraphia, and dyscalculia. She showed rapid recovery from her aphasia, with residual deficits in writing, naming, calculation, and memory.

The function of subcortical nuclear structures in language is uncertain, and language disorders after injury to these structures are described incompletely. We report 15 patients with left putaminal or thalamic hemorrhage, describe the range of language and behavioral disorders produced, and review the potential mechanisms of these disorders. Clinicoanatomic correlations revealed no definite differences between aphasia after hemorrhage in putamen or in thalamus.

It has been proposed that the comprehension of written language requires transcoding from the visual (grapheme) to the auditory (phoneme). It has also been proposed that visual word images can be comprehended without grapheme-phoneme transcoding. We describe three aphasic patients with left hemisphere impairment who had poor speech comprehension but could comprehend written language. One of these patients had a subsequent right hemisphere lesion and lost his ability to read. We propose that the right hemisphere in some individuals may be capable of extracting semantic information from iconic i

Introduction: Depressive symptoms are a major drawback of aphasia, negatively impacting on functional outcomes. In a previous study, Intensive Language-Action Therapy (ILAT) was effective in improving depression and low mood in persons with chronic non-fluent aphasia. We present a proof-of-concept case-control study that evaluates language and mood outcomes amongst persons with fluent post-stroke aphasia.Participants: Thirteen Spanish speaking persons with fluent aphasia due to chronic stroke lesions in the left hemisphere participated in the study.Intervention: Five participants (intervention

BACKGROUND Recovery from post-stroke aphasia is a long and complex process with an uncertain outcome. Various interventions have been proposed to augment the recovery, including constraint-induced aphasia therapy (CIAT). CIAT has been applied to patients suffering from post-stroke aphasia in several unblinded studies to show mild-to-moderate linguistic gains. The aim of the present study was to evaluate the neuroimaging correlates of CIAT in patients with chronic aphasia related to left middle cerebral artery stroke. MATERIAL AND METHODS Out of 24 patients recruited in a pilot randomized blind

INTRODUCTION: Life with aphasia affects the whole family with shorter, less frequent conversations, frustration, reduced social networks, isolation and tension in relationships. Evidence suggests communication partner training (CPT) benefits families. However, expected improvements are poorly articulated. The Aphasia Partnership Training (APT) project aimed to identify target outcomes of a new family dyad CPT programme through persons with aphasia (PWA), family member and speech and language therapist (SLT) consensus.
METHOD: Consensus on desired outcomes was achieved through nominal groups w
