Spinal cord stimulation as a novel neuromodulation strategy for enhancing language recovery in post-stroke aphasia: a hypothesis.

Post-stroke aphasia is a debilitating language disorder that significantly impairs communication and quality of life. Although speech therapy and non-invasive brain stimulation techniques such as repetitive transcranial magnetic stimulation (rTMS) and transcranial direct current stimulation (tDCS) have shown clinical benefit, treatment responses remain variable and incomplete. We hypothesize that spinal cord stimulation (SCS), encompassing invasive epidural SCS (eSCS) and two mechanistically distinct non-invasive transcutaneous modalities, transcutaneous spinal direct current stimulation (tsDC

Brain structural and functional correlates of the heterogenous progression of mixed transcortical aphasia.

Mixed transcortical aphasia (MTCA) is characterized by non-fluent speech and comprehension deficits coexisting with preserved repetition. MTCA may evolve to less severe variants of aphasias or even to full language recovery. Mechanistically, MCTA has traditionally been attributed to a disconnection between the spared left perisylvian language network (PSLN) responsible for preserved verbal repetition, and damaged left extrasylvian networks, which are responsible for language production and comprehension impairments. However, despite significant advances in in vivo neuroimaging, the structural

Telehealth practice in aphasia: A survey of UK speech and language therapists, with a focus on assessment.

BACKGROUND AND OBJECTIVES: Evidence suggests telehealth in speech and language therapy can enhance access to care, cost-effectiveness and satisfaction. However, little is known about use of telehealth in the United Kingdom. Moreover, many assessments/outcome measures for aphasia have been tested for face-to-face administration only, posing challenges to reliable use within the telehealth context. We explored the experiences and views of speech and language therapists (SLTs) working with people with aphasia on using telehealth to conduct assessments/outcome measures, perceived barriers and faci

Task-State Functional MRI of Brain Regions with Reduced Activation in Aphasia Patients: A Meta-analysis.

OBJECTIVE: Language is one of the most celebrated hallmarks of human cognition. With the continuous improvement of medical technology, functional MRI (fMRI) has been used in aphasia. Although many related studies have been carried out, most studies have not extensively focused on brain regions with reduced activation in aphasic patients. The aim of this study was to identify brain regions normally activated in healthy controls but with reduced activation in aphasic patients during fMRI language tasks. METHODS: We collected all previous task-state fMRI studies of secondary aphasia. The brain r

Effects of Transcranial Direct Current Stimulation in writing: a case report of deep agraphia.

We present the case report of a 61-year-old male participant with chronic conduction aphasia and deep agraphia after ischemic stroke who received training on writing under dictation associated with transcranial direct current stimulation. The treatment consisted of five 50-minute dictation sessions with the application of 2 mA of anodal transcranial direct current stimulation for 20 minutes over the left occipitotemporal cortex. The participant improved his written production of pseudowords and regular low-frequency words, via the phonological route, in addition to a small improvement in the p

Effects of transcranial direct current stimulation on language recovery in chronic post-stroke aphasia.

Background: Aphasia is a major cause of long-term disability in post-stroke patients. Non-invasive brain stimulation, particularly transcranial direct current stimulation (tDCS), has shown promise in enhancing language recovery. However, evidence from Indonesia remains scarce. This study aimed to evaluate the effects of tDCS on language recovery in chronic post-stroke aphasia (PSA). Methods: This quasi-experimental study included 30 patients with chronic PSA, divided into 2 groups: 15 received 5 sessions of tDCS combined with language training, while 15 underwent language training alone. Music-based interventions for nonfluent aphasia: A systematic review of randomized control trials.

Music-based interventions (MBIs) offer promising strategies for addressing speech-language impairments in individuals with nonfluent aphasia. This systematic review summarizes the current literature of MBIs for nonfluent aphasia recovery by types of MBIs to determine the efficacy of MBIs and assesses the risk of bias to identify common methodological limitations. A systematic search was conducted of MEDLINE, PubMed, and APA PsycInfo for the 20 years preceding July 2024. Risk of bias assessment was performed using the revised Joanna Briggs Institute critical appraisal tool for randomized contro

Aphasia and acquired reading impairments: What are the high-tech alternatives to compensate for reading deficits?

BACKGROUND: People with aphasia (PWA) frequently present impairments in reading comprehension. Such impairments can be particularly debilitating due to the limitations and constraints they impose on everyday life. Recent technological advancements in the field of information and communication technologies offer many compensative tools for PWA. However, most technological tools are designed for patients with speech production impairments. Instruments addressing reading impairments associated with aphasia remain scarce and underrepresented in the scientific literature. AIMS: To conduct a state-

Network-specific enhancement of global blood-oxygen-level-dependent signals and CSF coupling after language therapy in post-stroke aphasia.

Emerging evidence indicates that stroke may impair glymphatic function by disrupting cerebrospinal fluid clearance. The coupling between global blood-oxygen-level-dependent signals and cerebrospinal fluid flow has been proposed as a non-invasive biomarker of glymphatic activity. However, it remains unclear whether this coupling can be modulated through rehabilitation. This study investigated whether language therapy in patients with post-stroke aphasia enhances global blood-oxygen-level-dependent- cerebrospinal fluid coupling, thereby reflecting potential restoration of fluid-brain interaction
