Primary Progressive Aphasias: Diagnosis and Treatment.

Background and Objective: Primary Progressive Aphasias (PPAs) are rare neurodegenerative disorders classified within frontotemporal lobar degeneration (FTLD) and typically manifest between 45 and 70 years of age. In Mexico-and many other countries-reliable epidemiological data are lacking; however, estimates suggest that PPA accounts for 0.5-2.5% of neurodegenerative disease cases in Memory Clinics, with an incidence of approximately 1 per 100,000 and an average survival of 8 years. This review aims to provide clinicians with an overview of PPA's epidemiology, clinical features, and classifica

Pantomime, praxis, and aphasia.

The production and comprehension of pantomimed movements by asphasic subjects were studied with respect to their relationship to the aphasic deficit on one hand and apraxia on the other. At issue was whether impaired use of pantomime is a manifestation of reduced symbolic capacity or purely a manifestation of an apraxic impairment of purposeful movement or both. Tests of pantomime included a Pantomime Recognition Test, a nonverbal Transitive Pantomime Production Test, and an Intransitive Pantomime Production Test (to oral command). Imitation of Nonmeaningful Movements served as a measure of ap

Application of neuroscience to technology in stroke rehabilitation.

The past decade has seen remarkable advances in our understanding of mechanisms that drive functional neuroplastic change after brain injury and the mirror neuron system that appears essential for language learning and communicative interaction. This article describes five neuroscience-based interventions available for clinical practice, with a discussion of the potential value of mirror neurons in stroke rehabilitation. Case-study data on three adults with aphasia who received various combinations of neuroscience-derived technological interventions are provided to inform the clinician of the

Speech production: Wernicke, Broca and beyond.

We investigated the brain systems engaged during propositional speech (PrSp) and two forms of non- propositional speech (NPrSp): counting and reciting overlearned nursery rhymes. Bilateral cerebral and cerebellar regions were involved in the motor act of articulation, irrespective of the type of speech. Three additional, left-lateralized regions, adjacent to the Sylvian sulcus, were activated in common: the most posterior part of the supratemporal plane, the lateral part of the pars opercularis in the posterior inferior frontal gyrus and the anterior insula. Therefore, both NPrSp and PrSp were

Two forms of ideomotor apraxia.

Destruction of parietal areas containing visuokinesthetic motor engrams, where motor acts may be programmed, should be distinguishable from apraxia induced by disconnection of these parietal areas from frontal motor areas. Destruction should result in inability to distinguish well-performed from poorly performed movements, whereas disconnection should not. We gave movement and act-discrimination tasks to apraxic and nonapraxic patients with anterior lesions or nonfluent aphasia, and to patients with posterior lesions or fluent aphasia. On both tasks, the performance of posterior/fluent patient

[Beriberi after esophagectomy].

A 55-years-old man with a history of alcoholism, hypertension and obesity was diagnosed of epidermoid carcinoma of the middle third portion of the esophagus. He was treated with two cycles of cytostatics with cisplatin and 5-fluorouracil. Due to his poor general health an inability to swallow solids and liquids, he received parenteral nutrition for 20 days using a commercial formula lacking in vitamins and minerals. During distal esophagectomy we observed a tendency to hypotension and severe metabolic acidosis that was unexplained by the hemodynamic profile and that persisted throughout the fi

Incomplete Kluver-Bucy syndrome and fluent aphasia.

A case of probable herpetic viral encephalitis is presented. The clinical picture following the acute phase of disease was dominated by severe fluent aphasia, prominent oral tendencies, visual agnosia, and a decrease in fear reactions. Most of these symptoms, with the obvious exception of aphasia, are similar to those observed by Kluver and Bucy in rhesus monkeys that underwent bitemporal lobectomy. These manifestations may be explained by the tendency of the herpes simplex virus to invade the temporal lobes. "Agnosia" in the auditory field and changes in vocal behavior were observed by H. Klu

Spontaneous recovery of language in patients with aphasia between 4 and 34 weeks after stroke.

The paper describes the spontaneous recovery of language abilities of 52 stroke patients who were aphasic for more than 4 weeks. These patients had been randomly allocated to receive no speech therapy and had been assessed at 6-weekly intervals after a stroke. There was improvement in language abilities over time. Age, sex and aphasia type were not related to the amount of improvement. An aphasic patient's level of language ability at 6 months could be predicted on the basis of the test score on the Porch Index of Communicative Ability at 4 weeks.

[Henry Head (1861-1940) and his importance for neurology].

Nowadays, Henry Head is best known for his Head zones. The concept was understood very differently by Head in comparison to what current medical books falsely describe them to be. In reality, there is no direct relationship between one particular skin zone and one single organ. It is certain that the drawings considered depictions of the Head zones in today's medical textbooks were actually not created by Head. From a neurological point of view, Head is important for two reasons: his self-experiment in 1909 to damage one of his own peripheral nerves followed by regeneration was heroic. It has

[The symptomatology of frontal and temporal lobe damages].

In this paper, we review the symptoms associated with damages to the frontal and/or temporal lobes. Similarities and differences between the symptoms observed after a stroke and in frontotemporal lobar degeneration are also discussed. Frontal lobe damages may lead to various apraxic disorders, including limb-kinetic, ideomotor, gait, buccofacial, and ocular motor apraxia. Language dysfunction can arise from perisylvian lesions as well as from extra-perisylvian regions. Broca's aphasia, Wernicke's aphasia, pure word deafness, and aphemia are typical examples of disorders caused by damages to th

Communicative disorders in Wernicke's aphasics.

The purpose of this study was to analyze some aspects of communicative capacities in 25 predominantly Wernicke's aphasics. The patients with moderate and mild deficits were compared with severely affected patients. Answers in a standard interview were studied for two aspects: answer adequacy to question and conceptual thinking fluency. An initial examination was carried out in the first stage of aphasia and the second examination took place after 3 or 4 months since onset. Both parameters showed more important deficits in the group with severe disorders at onset. An improvement of communicativ

Altered effective connectivity within the language network in primary progressive aphasia.

Primary progressive aphasia (PPA) is a neurodegenerative dementia syndrome principally characterized by the gradual dissolution of language functions, especially in the early stages of disorder. In a previous functional neuroimaging study, PPA patients were found to activate core language areas similarly to control subjects when performing semantic and phonological processing tasks (Sonty et al., 2003). In the present study, functional magnetic resonance imaging (fMRI) and dynamic causal modeling (DCM) were used to study multiregional effective connectivity in early-stage PPA (n = 8) and contr

Impairment in processing meaningless verbal material in several modalities: the relationship between short-term memory and phonological skills.

Phonological processing abilities were studied in a patient who, following focal brain damage, showed selective impairment in non-word reading, writing, and repetition and also a severe short-term memory (STM) deficit specific for auditorily presented verbal material. The patient could execute tasks involving phonemic manipulation and awareness perfectly. Our data, in contrast with earlier observations in a case of developmental phonological dyslexia, show that acquired impairment in non-word reading, writing, repetition, and immediate memory may occur despite good phonological processing abil

[The seizure syndrome combined with sensorimotor aphasia and alalia (the Landau-Kleffner syndrome)].

Electric brain activity and clinical status were studied in children with Landau-Kleffner syndrome. The paper covers 21 cases of the disease in patients aged from 2 years 11 months to 16 years in whom speech disorders and convulsive episodes varied in onset. A characteristic EEG pattern was recorded in the majority of the examinees. It comprised slow high-amplitude spike-and-wave complexes localized primarily in the temporal regions, more frequently on the left side. Clinically, the children belonged to 2 groups: epileptic aphasia and epileptic alalia patients. The latter had more significant

Recurrent Acute Ischemic Stroke Associated with Dolichoectatic Aneurysm of the Internal Carotid Artery.

BACKGROUND: Cranial artery dolichoectasia is characterized by dilated, elongated, and tortuous cranial arteries. Dolichoectasia and dolichoectatic aneurysm-particularly of the internal carotid arteries (ICAs)-are rare but can lead to stroke. We report a case of suspected recurrent acute cerebral infarction and dolichoectatic aneurysm of the left ICA combined with right dolichoectatic aneurysm, vertebrobasilar dolichoectasia, and atrial fibrillation. CASE DESCRIPTION: An 80-year-old man experienced 3 days of recurrent aphasia episodes. He had a history of atrial fibrillation and hypertension.

Phonological transformations in conduction aphasia.

Different explanations and subtypes of conduction aphasia are analyzed. Characteristics of literal paraphasias in parietal-insular conduction aphasia are discussed, emphasizing that paraphasias in conduction aphasia are articulatory-based (articulatory literal paraphasias) and due mainly to phoneme substitutions and phoneme deletions; they result basically in switches in phoneme manner and place of articulation. Similarities between errors in ideomotor apraxia and conduction aphasia language deficits are presented. It is proposed that language deviations (in oral as in written language) in con

Interhemispheric transfer of language in patients with left frontal cerebral arteriovenous malformation.

Cerebral arteriovenous malformations (AVMs) are frequently evaluated before therapeutic embolization by superselective injection of anesthetics into individual arterial branches so as to determine whether permanent occlusion would affect eloquent function. In Experiment 1, we used this adaptation of the Wada procedure to study three right-handed adult patients with left frontal cerebral AVMs by injecting vessels in Wernicke's and Broca's areas, respectively, and assessing language functions. The results showed that superselective testing in the inferior division of the left MCA in all three pa

Grammatical comprehension deficits in non-fluent/agrammatic primary progressive aphasia.

IMPORTANCE: Grammatical comprehension difficulty is an essential supporting feature of the non-fluent/agrammatic variant of primary progressive aphasia (naPPA), but well-controlled clinical measures of grammatical comprehension are unavailable. OBJECTIVE: To develop a measure of grammatical comprehension and examine this comparatively in PPA variants and behavioural-variant frontotemporal degeneration (bvFTD) and to assess the neuroanatomic basis for these deficits with volumetric grey matter atrophy and whole-brain fractional anisotropy (FA) in white matter tracts. DESIGN: Case-control stud
