BACKGROUND: ST-segment elevation on electrocardiography is classically associated with acute transmural myocardial ischemia due to coronary artery occlusion and is central to the diagnosis of ST-segment elevation myocardial infarction. However, persistent ST-segment elevation may also reflect nonischemic structural heart disease.
CASE SUMMARY: A 62-year-old woman presented with chest pain and widespread ST-segment elevation with T-wave inversion. Coronary angiography showed no obstructive disease. Echocardiography and cardiac magnetic resonance revealed midventricular hypertrophic cardiomyopa

OBJECTIVES: The aim of this study was to systematically review and quantitatively synthesize existing evidence about the prognostic value of LV apical aneurysm in patients with HCM.
BACKGROUND: Hypertrophic cardiomyopathy (HCM) represents a common inherited heart disease associated with enormous diversity in morphologic expression and clinical course. With the increasing penetration of advanced high resolution cardiovascular imaging into routine HCM practice, a subset of HCM patients with left ventricular (LV) apical aneurysm have become more widely recognized.
METHODS: Medline was searched

Mid-ventricular hypertrophic obstructive cardiomyopathy with secondary formation of apical aneurysm is a rare variant of hypertrophic cardiomyopathy. They have a unique behavior because unlike other variants it causes sustained monomorphic ventricular tachycardia, which makes it particularly severe.

BACKGROUND: In patients with hypertrophic cardiomyopathy (HCM), akinetic apical aneurysms are associated with ventricular tachycardia, heart failure, apical thrombus, and mortality. The cause of apical aneurysms remains unresolved, and there is controversy about prevalence and significance of mid-left ventricular (LV) obstruction, often present in these patients. The aim of this study was to test the hypothesis that low velocities in patients with aneurysms are due to near complete cessation of mid-LV flow, characteristically marked by a Doppler signal void.
METHODS: This was a retrospective

OBJECTIVES: Some patients with hypertrophic cardiomyopathy (HCM) develop left ventricular apical aneurysm, leading to serious cardiovascular complications. The aims of this study were to identify the incidence and clinical course of HCM patients with apical aneurysms in Japan, and to evaluate the role of electrocardiography (ECG) as a screening test to detect apical aneurysms in HCM patients.
METHODS AND RESULTS: In a retrospective, single center analysis of a population of 247 HCM patients, 21 patients (8.5%) had left ventricular apical aneurysms. Their mean age was 60 ± 14 years (range: 23-

INTRODUCTION: Hypertrophic cardiomyopathy is a genetic cardiac disease characterized by marked variability in morphological expression and natural history. The hypertrophic myocardium is often confined to the septum or lateral wall of the left ventricle, but it can also be encountered in the middle or apical segments of the myocardium. Treatment is based on medical therapy. Others therapies, such as embolization of the septal artery or ventriculomyectomy, are indicated in special situations. Surgery is the standard treatment, and it is classically done via a transaortic approach; however, in c

A 30-years-old man, subjected to an electrocardiogram for screening, showed signs of left ventricular hypertrophy and "giant" inverted T-waves in pre-cordial leads. So, a transthoracic echocardiogram and a Coronary angiography were performed that showed first, a normal ventricular function and second, normal coronary arteries. Luckily, we associated to the Coronary angiography a Ventriculography that revealed a marked hypertrophy particularly in the apex and a 90% complete tele-systolic obliteration of the left ventricle. To better address our diagnosis, we performed a cardiac magnetic resonan

We describe the case of a 21-year-old Italian male who presented with giant negative T-waves and left ventricular hypertrophy on the electrocardiogram suggestive of apical hypertrophic cardiomyopathy. Clinically, he suffered from new onset of exertional angina, dyspnea and palpitations during soccer playing or heavy exercise beginning one week before admission. Echocardiography and cardiac catheterization conformed the rare combination of a nonobstructive apical hypertrophic cardiomyopathy of the "Japanese" type coexistent with an extensive muscular bridge involving almost the entire anterior

Purpose To evaluate the predictive value of cardiac MRI-derived left atrial strain (LAS) for new-onset atrial fibrillation (AF) in apical hypertrophic cardiomyopathy (AHCM). Materials and Methods This retrospective study included patients with AHCM with no prior history of AF who underwent 3.0-T cardiac MRI between May 2016 and December 2023. LAS parameters-reservoir strain (εs), conduit strain (εe), and booster pump strain (εa)-and minimum left atrial volume index (LAVImin) were measured. The clinical end point was new-onset AF as confirmed with electrocardiography, Holter monitoring, or impl

Apical hypertrophic cardiomyopathy (ApHCM), also known as Yamaguchi syndrome, is an uncommon variant of hypertrophic cardiomyopathy that may mimic acute coronary syndromes because of its characteristic electrocardiographic abnormalities. We report the case of a 66-year-old man with hypertension, type 2 diabetes mellitus, and active tobacco use who presented with recurrent exertional chest pain and giant symmetric T-wave inversion in leads V2-V6, producing a Wellens-like electrocardiographic pattern that mimicked Wellens syndrome and initially raised suspicion for critical proximal left anterio

Yamaguchi syndrome, also known as apical hypertrophic cardiomyopathy (AHCM), is a genetic disorder predominantly affecting the apex of the left ventricle and often presenting similarly to acute coronary syndrome, making precise imaging crucial for diagnosis. This condition, first identified in Japanese populations, is more common in Asian communities but varies in frequency across different populations. We are presenting the case of a 30-year-old African-American male patient with a history of hyper-lipidemia, asthma and obesity, who reported palpitations, dizziness and chest pain radiating to

Apical hypertrophic cardiomyopathy (AHCM), also known as Yamaguchi syndrome, is a rare variant of hypertrophic cardiomyopathy (HCM) characterized by localized thickening of the left ventricular apex. Clinically, it may mimic acute coronary syndrome (ACS), myocarditis, or pericarditis. Cardiac magnetic resonance (CMR) allows the identification of myocardial involvement even in the absence of elevated serum biomarkers. A 39-year-old Hispanic man presented with a four-day history of oppressive chest pain radiating to the left arm, associated with dyspnea, fever, and dysuria. On admission, ECG dem

BACKGROUND: We investigated whether ethnicity and sex are associated with different clinical presentations and cardiovascular magnetic resonance (CMR) findings in individuals with apical hypertrophic cardiomyopathy (ApHCM).
METHODS: A retrospective observational cohort study of consecutive ApHCM patients from a large tertiary referral center in the United Kingdom (UK). Demographic, clinical, 12‑lead electrocardiogram (ECG) and CMR findings were collected. Participants presented in our clinics between 2010 and 2020. 'Pure' ApHCM was defined as isolated apical hypertrophy and 'mixed' with both

BACKGROUND: Acute left ventricular (LV) apical ballooning with normal coronary angiography occurs rarely in obstructive hypertrophic cardiomyopathy (OHCM); it may be associated with severe hemodynamic instability.
METHODS, RESULTS: We searched for acute LV ballooning with apical hypokinesia/akinesia in databases of two HCM treatment programs. Diagnosis of OHCM was made by conventional criteria of LV hypertrophy in the absence of a clinical cause for hypertrophy and mitral-septal contact. Among 1519 patients, we observed acute LV ballooning in 13 (0.9%), associated with dynamic left ventricula

Apical hypertrophic cardiomyopathy (HCM) is a phenotypic variant of nonobstructive HCM, in which hypertrophy of the myocardium predominantly involves the left ventricular apex. It is common in Japanese and other Asian populations but is rare in the United States. Apical HCM has a relatively benign prognosis in terms of cardiovascular mortality; however, morbid events such as ventricular aneurysms, apical thrombi, diastolic dysfunction, atrial fibrillation, and myocardial infarction are not uncommon. We report a case of an 18-year-old white man who presented to our hospital after an out-of-hosp

INTRODUCTION: Accurate classification of apical hypertrophic cardiomyopathy (ApHCM) subtypes is challenging due to morphological variability and overlapping phenotypes. Conventional echocardiography provides limited visualization of the apex. Artifacts induced during left ventricular opacification (LVO) complicate diagnostic interpretation. A deep learning-based framework enhances image quality and improves subtype classification.
MATERIALS AND METHODS: In this work, a deep learning-based framework is used for ApHCM subtype classification. Apical four-chamber end-diastolic frames from 3,200 i

This article reports a rare case of diffuse hypertrophic obstructive cardiomyopathy complicated with left ventricular apical aneurysm and excessive trabeculation. Genetic testing of the patient revealed a heterozygous variant of the MYBPC3 gene (c.3343G> A: p.Val1115Ile). Despite optimal medical therapy and implantable cardioverter-defibrillator (ICD) implantation, the patient developed progressive cardiac dysfunction and recurrent ICD shocks. We discuss the clinical course, genetic findings, and imaging features of this case.
