Characteristics of rural Appalachian women who enroll in a tobacco dependence treatment clinical trial.

INTRODUCTION: Clinical trials are needed to inform evidence-based smoking cessation approaches in rural and Appalachian women, but trial enrollment in these groups is sparse. Little is known about factors associated with enrollment of Appalachian women in tobacco dependence treatment randomized clinical trials (RCT). We report a comparison of Appalachian women enrolling in a tobacco dependence treatment RCT to those declining and identify correlates to enrollment. METHODS: Smokers identified during a cervical health-related survey among Ohio Appalachian women were invited to enroll in a tobac

Cemeteries are effective sites for monitoring la crosse virus (LACv) and these environments may play a role in LACv infection.

La Crosse encephalitis (LAC) is the leading arboviral disease among children, and was previously limited to the upper Midwest. In 2012 nine pediatric cases of LAC occurred in eastern Tennessee, including one fatal case. In an attempt to identify sites near an active LACv infection and describe the abundance and distribution of potential LACv vectors near a fatal LAC case in the Appalachian region, we initiated an end of season study using a combination of questing and oviposition mosquito traps placed at forty-nine sites consisting of cemeteries and houses within 16 radial kilometers of two pe

A Multi-level Model to Understand Cervical Cancer Disparities in Appalachia.

The Appalachian region experiences higher incidence and mortality due to cervical cancer compared with other regions of the United States. The goal of the Ohio State University Center for Population Health and Health Disparities (CPHHD), called the Community Awareness Resources and Education (CARE) project, was to understand reasons for this disparity. The first wave (2003-2008) of funding included three projects focusing on the known risk factors for cervical cancer, lack of screening, smoking, and infection with human papillomavirus (HPV). On the basis of the results of these projects, the s

Emergence of wasp dope in rural Appalachian Kentucky.

BACKGROUND AND AIMS: Recent reports have highlighted the emergence of 'wasp dope' as an issue of concern, but epidemiological evidence is lacking. Wasp dope is a crystalline substance created by electrifying pyrethroid-containing insecticides (e.g. wasp sprays) that may give users a methamphetamine-like 'rush'. This paper describes wasp dope use and correlates of use in a sample of people who use drugs (PWUD) in Appalachian Kentucky, a region that has been an epicenter of opioid use and related harms in the United States. METHODS: Respondent-driven sampling and targeted street outreach were u

Rural risk environments for hepatitis c among young adults in appalachian kentucky.

BACKGROUND: Rural Kentucky is an epicenter of hepatitis C(HCV), especially among young adults who inject drugs. While the Risk Environment Framework (REF) has been used widely to study and address socio-ecological determinants of infectious disease among people who inject drugs (PWID), it has been almost exclusively applied to urban environments. Applying REF to rural environments can enhance our understanding of the drivers of HCV epidemics in these hard-hit areas, and inform the creation and implementation of harm reduction interventions in this local context. METHODS: Participants were rec

Effectiveness of a self-care intervention addressing social determinants of health in reducing cardiovascular disease risk in rural populations.

PURPOSE: HeartHealth is a multi-component cardiovascular disease (CVD) risk reduction intervention developed for rural Appalachia participants. Its effectiveness in reducing CVD risk factors has been demonstrated, and although HeartHealth was developed to address the negative impact of social determinants of health (SDOH), it remains unclear whether its impact is modified by key SDOH. The aims of the study were to evaluate whether the intervention effect on the Framingham CVD Risk Score (FRS) differs by financial status, education level, sex, depressive symptoms, and health literacy. METHODS:

Exploring cannabis as a treatment for chronic pain in a rural Appalachian population.

BACKGROUND AND OBJECTIVES: Many cannabis laws include medical use with pain as an indication. However, it is not known the extent of cannabis use for pain, the types of pain treated, and the perceived effectiveness of cannabis for chronic pain in this region. The purpose of this study was to understand cannabis use for pain in West Virginia patients. METHODS: Ten thousand chronic pain patients were surveyed, and users and non-users of cannabis were compared. RESULTS: One thousand three hundred fifty-one people responded, with 24% using cannabis. They were in pain for longer, most >10 years.

Cultural motivations, instrumental support, and depressive mood among dementia family caregivers in rural Appalachia.

OBJECTIVES: Although many dementia family caregivers receive instrumental help from relatives and friends for caregiving tasks, prior research on its impact on caregivers' mood has been inconsistent. Guided by the sociocultural stress and coping model, we used daily diary data to examine whether receiving instrumental support is associated with lower depressive mood among dementia family caregivers, and whether this association varies by caregivers' cultural motivations for caregiving (e.g. sense of duty). METHOD: We analyzed data from 157 dementia family caregivers living in rural Appalachia

Changing Trends in Nonmedical Gabapentin Use Among a Cohort of Rural Appalachian People Who Use Drugs: A Longitudinal Study Over the Time Period of Scheduling Changes in Kentucky.

BACKGROUND: Recently, nonmedical gabapentin (Neurontin) use has increased in the United States, leading to its classification as schedule V substance in Kentucky in 2017. This paper examines patterns in nonmedical gabapentin (Neurontin) use in people who use drugs (PWUD) over the time of scheduling changes. METHODS: Longitudinal data (2008-2020) from the Social Networks among Appalachian People (SNAP) study were analyzed. SNAP participants (n = 503) met criteria of being ≥ 18 years old, residing in Appalachian Kentucky, and reporting recent nonmedical prescription opioid, methamphetamine, coc

Transforming Access to Hearing Care: Community Perspectives on School-Based Telehealth in Rural Appalachia.

OBJECTIVES: School-based hearing screening serves as a critical resource for children in rural areas to be screened and connected to hearing healthcare. Telemedicine interventions in schools have shown promise in connecting children to providers; however, there is limited research on systematic adaptation and deployment of telemedicine in rural schools. Obtaining community perspectives and preferences on school-based telemedicine hearing evaluation is essential to ensure such interventions are deployable in a rural context. Grounded in the ADAPT-ITT framework, this research focused on assessin

Prevalence and correlates of receptive syringe-sharing among people who inject drugs in rural Appalachia.

BACKGROUND AND AIMS: Syringe-sharing significantly increases the risk of HIV and viral hepatitis acquisition among people who inject drugs (PWID). Clearer understanding of the correlates of receptive syringe-sharing (RSS) is a critical step in preventing bloodborne infectious disease transmission among PWID in rural communities throughout the United States. This study aimed to measure the prevalence and correlates of RSS among PWID in a rural county in Appalachia. DESIGN: Observational, cross-sectional sample from a capture-recapture parent study. SETTING: Cabell County, West Virginia (WV),

Predictors of high chlamydia and gonorrhea positivity rates among men in the southern United States.

BACKGROUND: Routine screening for Chlamydia trachomatis and Neisseria gonorrhoeae in men in the United States is not recommended. However, untreated men remain a potential reservoir for chlamydial and gonococcal infections and reinfection among women. Chlamydia and gonorrhea positivities and associated epidemiology were evaluated among males in the southern United States. METHODS: Data were analyzed from 603320 males, aged 15 to 60 years, who were undergoing chlamydia and gonorrhea testing in sexually transmitted disease, family planning, correctional, college, and other facilities between 20

The relationship of personal financial status to the utilization of community mental health centers in rural Appalachia.

As a follow-up to an aggregate study of the relationship of economic factors to the utilization of mental health facilities, this study, using individual data, attempted to determine the associative strengths of income factors--welfare and unemployment--for utilization of community mental health centers (CMHCs) in rural Appalachia. On the basis of the earlier study and of previous studies of social class and utilization of CMHCs it was hypothesized that for this sample of 151 clients of eight CMHCs in Ohio and West Virginia, unemployment and welfare factors would be most strongly associated wi

Germline EGFR T790M mutation and lung cancer risk.

Most lung cancers are tobacco related, with genetic factors influencing smoking behavior identified through genome-wide association studies. However, inherited risk in familial and non-smoking-related lung cancers, including risk in carriers of EGFR T790M, remains poorly understood. Here, in more than 3.3 million individuals, the EGFR T790M germline variant is significantly associated with lung cancer risk, with no increased risk for 17 other cancers and no interaction with polygenic risk. This risk exceeds that conferred by smoking and is several-fold higher in never-smokers. Global geographi

Factors Associated With Mortality in Appalachian Nursing Homes During the COVID-19 Pandemic.

INTRODUCTION: Disasters, including the recent COVID-19 pandemic, have disproportionately impacted nursing homes (NHs). NH residents experienced higher mortality during the pandemic, but not all NH were affected equally. Appalachia has a history of reduced health compared to the general United States. Therefore, this study is focused on NHs in Appalachia during the COVID-19 pandemic. PURPOSE: This study aimed to investigate how the neighborhood and NH characteristics are associated with mortality in Appalachian NHs during the COVID-19 pandemic. METHODS: Using publicly available datasets, incl
