BACKGROUND: Controversies about laparoscopic appendectomy (LA) focus mainly on the high intraabdominal infection rate. In 2005, Serour et al described a distinct complication specific to LA, termed "postlaparoscopic appendectomy complication" (PLAC). There are many well known causes to acute inflammation of the appendix. During the past 50 years it has been discussed if blunt abdominal trauma can be related to acute appendicitis. There are several anatomical and pathophysiological speculations of why the appendix is vulnerable to inflammation following injury. In this case a patient developed appendicitis few hours after a car accident and was diagnosed by a trauma computed tomography of the abdomen. The anatomical and pathophysiological causes have been described.

For the successful management of an emergency appendectomy and abscess drainage in an Eisenmenger's patient with diffuse peritonitis, it may be preferable to use general anesthesia with awake intubation. Epidural anesthesia is commonly combined with this anesthetic management for emergency abdominal surgery. However, the combination of topical anesthesia with epidural anesthesia should be avoided for fear of toxicity of local anesthetics in a patient with complications due to Eisenmenger's syndrome.

We report a case of a 13-year-old boy who developed an acute left hemiscrotum following a laparoscopic appendectomy for a perforated appendicitis.
