Diarrhea Concealing a Duodenal-Cecal Fistula Secondary to Appendiceal Mucinous Neoplasm.

Primary mucinous adenocarcinoma of the appendix is a rare gastrointestinal malignancy. Fistulous tract formation is a complication that is cited in literature. An 85-year-old man with multiple comorbidities presented with several weeks of persistent non-bloody diarrhea. Laboratory work-up was non-diagnostic. Abdominal imaging with barium contrast showed an enterocolonic fistulous tract extending from the duodenum to the cecum involving an enlarged appendiceal mass. Subsequent biopsy confirmed mucinous appendiceal neoplasm with peritoneal spread to the liver and mesentery. This is the first rep

When and When Not to Perform a Right Colon Resection with Mucinous Appendiceal Neoplasms.

BACKGROUND: Mucinous appendiceal neoplasms (MAN) with peritoneal dissemination is treated as a standard of care using cytoreductive surgery and hyperthermic perioperative chemotherapy. The extent of the resection of peritoneal surfaces and visceral structures is generally well defined. Exception to this consensus regarding structures to be removed are the right colon and adjacent ileocolic lymph nodes. METHODS: From a prospectively maintained database, all patients with a histologic diagnosis of peritoneal mucinous carcinoma (PMCA) who underwent complete cytoreductive surgery were assessed fo

Low Grade Appendiceal Mucinous Neoplasm with Pseudomyxoma Peritonei: An Enigma for Pathologist.

Malignant mucinous neoplasms of the appendix is an infrequently encountered entity. Extra-appendiceal spread of these tumor is one of the commonest etiology of pseudomyxoma peritonei, which demands a hightened vigilance in their early diagnosis. Although low-grade appendiceal mucinous neoplasms (LAMNs) largely stay confined to the appendix, but they can spread to the peritoneum as pseudomyxoma peritonei leading to an unpredictable outcome. Due to the rare occurrence of low-grade appendiceal neoplasm only tenuous and limited information is present in the medical literature. We report a case of

Next-Generation Sequencing Identifies Key Targetable Mutations in the Treatment of Appendiceal Neoplasms.

BACKGROUND: Appendiceal neoplasms are a group of rare, heterogeneous tumors that exhibit varying malignant potential. Systemic treatment options for disseminated appendiceal cancer are limited. We sought to review rates of mutations that may indicate potential roles for routine next-generation sequencing to identify targetable therapies. METHODS: An analysis of 916 appendiceal tumor samples submitted to American Association for Cancer Research Genomics Evidence Neoplasia Information Exchange (GENIE) consortium was performed to compare patient demographics and the rates of mutations that may c

Low-grade appendiceal mucinous neoplasms combined with incidental gallbladder cancer: a case report.

BACKGROUND: Mucinous tumors of the appendix are relatively rare compared to other gastrointestinal tumors, but their incidence tends to gradually increase. Due to the lack of specific clinical manifestations, mucinous tumors of the appendix are prone to misdiagnosis and missed diagnosis. According to different pathological types, mucinous tumors of the appendix can be roughly divided into low-grade mucinous tumors and high-grade mucinous tumors, with significant differences in prognosis. Gallbladder cancer is the most common type of malignant tumor in the biliary system, with a high degree of

Risk Factors for Pseudomyxoma Peritonei in Appendiceal Mucinous Neoplasms: A Single-Center Experience.

INTRODUCTION: Appendiceal mucinous neoplasms (AMNs) are rare tumors with the potential to progress to pseudomyxoma peritonei (PMP). Identification of prognostic factors may improve surgical management and follow-up strategies. OBJECTIVE: This study aimed to analyze clinicopathological factors associated with the development of PMP in patients with AMNs treated at a tertiary care hospital in Mexico. METHODS: A retrospective study was conducted in Hospital Médica Sur, Mexico City, Mexico, including 30 patients with histologically confirmed AMNs between 2018 and 2023. Clinical, surgical, histol

Laparoscopy-assisted Partial Cecum Resection Versus Ileocecal Resection for Non-malignant Appendiceal Neoplasms.

BACKGROUND/AIM: Preoperative pathological diagnosis of appendiceal tumors is difficult due to their anatomical nature. Although lymph node metastasis is rare in low-grade appendiceal mucinous neoplasms (LAMN), there is a risk of a positive surgical margin by appendectomy. Partial cecum resection is considered an option to obtain negative resection margins for LAMN; however, the benefits of this procedure via a laparoscopic approach remain unclear. PATIENTS AND METHODS: We retrospectively analyzed patients who underwent laparoscopy-assisted partial cecum resection (Lap-Ce) or ileocecum resecti

Genome-Wide Analysis of DNA Methylation in Pseudomyxoma Peritonei Originated from Appendiceal Neoplasms.

INTRODUCTION: Pseudomyxoma peritonei (PMP) is a disease characterized by progressive accumulation of intraperitoneal mucinous ascites produced by neoplasms in the abdominal cavity. Since the prognosis of patients with PMP remains unsatisfactory, the development of effective therapeutic drug(s) is a matter of pressing concern. Genetic analyses of PMP have clarified the frequent activation of GNAS and/or KRAS. However, the involvement of global epigenetic alterations in PMPs has not been reported. METHODS: To clarify the genetic background of the 15 PMP tumors, we performed genetic analysis usi

Predicting Underlying Neoplasms in Appendiceal Mucoceles at CT: Focal Versus Diffuse Luminal Dilatation.

OBJECTIVE. The purpose of this study was to determine whether a novel morphologic characteristic of appendiceal mucoceles at CT-focal distal appendiceal dilatation with a segment of morphologically normal appendix proximally-could predict an underlying neoplastic pathologic abnormality before surgery and histopathologic assessment. MATERIALS AND METHODS. A retrospective study was performed that assessed CT cases from 2012 through 2014. Cases showing morphologic features of a mucocele were identified and categorized into two subgroups: focal distal dilatation with a segment of normal appendix p

Incidental Finding of a Low-Grade Appendiceal Mucinous Neoplasm: A Case Study.

Low-grade appendiceal mucinous neoplasms (LAMNs) are rare benign tumors of the appendix with possible life-threatening complications. A 25-year-old female patient presented to the emergency room with abdominal pain with symptoms concerning for acute appendicitis. After imaging, it was decided to proceed with an appendectomy. Final pathology showed low-grade appendiceal mucinous neoplasm. Due to their ability to mimic appendicitis, it is important to include LAMNs in the differential diagnosis for patients of all ages, as they can cause life-threatening pseudomyxoma peritonei (PMP). Although th

Confined Low Grade Appendiceal Mucinous Neoplasm With Coexisting Distant Metastasis.

BACKGROUND/AIM: Appendiceal mucinous neoplasms (AMNs) are tumors with dysplastic mucinous epithelium, a pushing growth pattern but no infiltrative invasion to the appendiceal wall. Some AMNs are associated with pseudomyxoma peritonei, characterized by intraperitoneal mucinous involvement. Recent studies have demonstrated that LAMNs confined to the appendix have low or no risk for disease recurrence, progression, and peritoneal involvement during follow up. CASE REPORT: Here, we present two extremely rare cases with confined low grade appendiceal mucinous neoplasm (pTis and pT3) and simultaneo

Cellularity of mucus is a prognostic variable in low-grade appendiceal mucinous neoplasms.

For peritoneal metastases from a primary appendiceal mucinous neoplasm to exist, the thin wall of the appendix must perforate to allow mucus or mucus plus tumor cells to gain access to the peritoneal spaces. The proportion of specimens containing tumor cells within mucus as compared to mucus only outside the appendix may have prognostic significance. The histopathology of tumor masses was determined from the specimens resected at the cytoreductive surgery (CRS). The presence versus absence of tumor cells in mucus and the proportion of specimens with tumor cells was determined and correlated wi

[A Case of Low-Grade Appendiceal Mucinous Neoplasm-A Complex Preoperative Diagnosis].

A 63-year-old woman was admitted to our hospital with suspected appendicitis. Abdominal CT revealed an enlarged appendix, and findings consistent with some small nodules. She was diagnosed with appendicitis and successfully treated with antibiotics. Contrast-enhanced CT performed 2 months after discharge revealed a reduction in small nodules, and tumor markers were not elevated. Therefore, we concluded that this case was unlikely to be an appendiceal tumor. Interval appendectomy was performed laparoscopically, and histopathological analysis revealed a low-grade appendiceal mucinous neoplasm(LA

Surveillance Colonoscopy Revealing Asymptomatic Low-Grade Appendiceal Mucinous Neoplasm.

Appendicular mucinous neoplasms are a collection of rare tumors with diverse clinical presentations and pathologic potential, which can cause diagnostic and therapeutic challenges. Traditionally, they are diagnosed by radiologic imaging or identified intraoperatively; however, rarely, they may be diagnosed during an endoscopic procedure. In this unusual case, we present the case of a 62-year-old Caucasian male undergoing routine surveillance colonoscopy due to a history of colonic neoplasia. During the colonoscopy, a submucosal, non-bleeding 1cm mass of benign appearance was found in the appen

Treatment of a giant low-grade appendiceal mucinous neoplasm: A case report.

BACKGROUND: Low-grade appendiceal mucinous neoplasm (LAMN) is extremely rare and easily misdiagnosed before surgery. CASE SUMMARY: We report the treatment of an asymptomatic case of LAMN diagnosed by magnetic resonance imaging (MRI) and surgical findings. A 70-year-old woman presented with an adnexal mass found by physical examination in July 2020. Gynecologic ultrasonography revealed a cystic mass in the right adnexa, and computed tomography showed a cystic mass in the pelvic cavity. All tumor markers were normal. A further MRI examination suggested mucinous neoplasm in the right pelvic cavi

Appendiceal Mucinous Neoplasm: Nurse Education About a Rare and Complex Disease.

BACKGROUND: Appendiceal mucinous neoplasms (AMNs) are a rare and complex spectrum of disease involving a mucinous tumor within the appendix, which can range from benign to malignant. If not limited to the appendix, the mucinous tumor can spread diffusely throughout the peritoneum. OBJECTIVES: Because of the low incidence of AMN, this study describes the diagnosis and treatment process for this disease, which is not well known to most care providers. METHODS: An extensive analysis of the current literature, including incidence, diagnosis, and surgical treatment, was performed. A review of the

Diverticular disease of the vermiform appendix: a diagnostic clue to underlying appendiceal neoplasm.

Acquired diverticula of the vermiform appendix are rare and arise as a result of different pathogenetic mechanisms. One of the etiologies includes proximally located, often unsuspected small neoplasms. Although the association of appendiceal diverticulosis and neoplasia is known, it remains underemphasized in the teaching and practice of surgical pathology. To investigate the frequency of appendiceal neoplasms with acquired diverticulosis, we conducted a retrospective analysis of all appendectomy specimens received in our institution for a 55-month period (January 2002-July 2006).
