Ideomotor apraxia: a call to action.

Although ideomotor apraxia (IM) has been a topic of investigation since the early 20th century, progress in studying the disorder has been hindered by important (and sometimes unarticulated) differences in the underlying theoretical models of various investigative groups. As a result, it is difficult to draw conclusions about the expected performance of IM patients on various tests of praxis (e.g. pantomime, single object use, gesture imitation, and naturalistic action); the relationship of IM to ideational apraxia (IA); the degree to which specific error types are diagnostic of one or the oth

Crossed apraxia: implications for handedness.

Liepmann posited that right hand preference relates to left hemisphere dominance for learned skilled movements. Limb apraxia, impairment of skilled movement, typically occurs in individuals with left hemisphere (LH) lesions. The occurrence of apraxia in right-handed individuals following right-hemisphere lesions appears to refute Liepmann's hypothesis. We studied the apraxia of a right-handed man, RF, following a right frontal lesion to determine whether his apraxia paralleled the apraxia seen following LH lesions. Results of behavioral testing indicated that, like individuals with apraxia fol

Functional brain imaging in apraxia.

BACKGROUND: An extensive literature describes structural lesions in apraxia, but few studies have used functional neuroimaging. We used positron emission tomography (PET) to characterize relative cerebral glucose metabolism in a 65-year-old, right-handed woman with progressive decline in ability to manipulate objects, write, and articulate speech. OBJECTIVE: To characterize functional brain organization in apraxia. DESIGN AND METHODS: The patient underwent a neurological examination, neuropsychological testing, magnetic resonance imaging, and fludeoxyglucose F 18 PET. The patient's magnetic

Ideational apraxia and naturalistic action.

It is frequently claimed that ideational apraxia, an impairm ent of the performance of complex actions with objects, is a left-hemisphere syndrome. We assessed the consequences of lefthemisphere damage for naturalistic action performance in two studies. In Study 1, we compared the action errors of left-hemisphere stroke patients (LCVA) to previously reported patients with right-hemisphere stroke (RCVA) and closed head injury (CHI), and found that LCVA were no more vulnerable to errors of action than the other patient groups once differences in severity were controlled. In Study 2, we compared

Visual-imitative dissociation apraxia.

Liepmann posited that, in right handers, the left parietal lobe contains movement formulas or representations. Therefore, performance failures may be induced by degraded representations, a failure of these representations to influence motor systems or a failure of stimuli to fully access these representations. Imitation may help the performance of subjects with degraded representations. However, patients who have impaired visual access to movement representations may perform more poorly with imitation than to verbal command. Trajectories of repetitive 'slicing' gestures made by a previously re

Speed of motor execution and apraxia.

We used a reaction time paradigm to explore the relationship between motor execution and apraxia. The task required reaching for one to three keys. The instruction was varied by introducing a model of a hand indicating which fingers to use. Whereas patients with right-brain damage were slower than controls regardless of condition, the performance of patients with left-brain damage was only impaired when movements had to be carried out according to the model. Although this indicates a deficit in movement planning, there was no correlation between the impairment of patients with left-brain damag

Ataxia-oculomotor apraxia syndrome.

Ataxia-oculomotor apraxia is a distinct entity first comprehensively described in 1988. The features include early childhood onset of ataxia and oculomotor apraxia, mimicking ataxia telangiectasia but without the extraneurologic findings of ataxia telangiectasia. We add to the clinical description of the ataxia-oculomotor apraxia syndrome by reporting eight patients, ages 2 to 15 years, from four families, suggesting autosomal recessive inheritance, with the longest follow-up over 6 years. After initial gait deterioration, all had a nonprogressive course. We have postulated that ataxia-oculomo

Subcortical origin of visuomotor apraxia.

Visuomotor apraxia (VMA) is a clinical syndrome characterized by a failure to make use of visual information when performing a target-directed movement. Visuomotor apraxia has traditionally been assumed to result from a disconnection of cortico-cortical fibres between visual and motor areas following occipito-parietal lesions. We describe a patient who developed a permanent contralesional and a temporary ipsilesional visuomotor apraxia as part of a complex neurological syndrome after a right [corrected] thalamic haemorrhage. MRI showed that the suprathalamic white matter was not involved but t

The nature of apraxia in corticobasal degeneration.

Although apraxia is one of the most frequent signs in corticobasal degeneration, the phenomenology of this disorder has not been formally examined. Hence 10 patients with corticobasal degeneration were studied with a standardised evaluation for different types of apraxia. To minimise the confounding effects of the primary motor disorder, apraxia was assessed in the least affected limb. Whereas none of the patients showed buccofacial apraxia, seven showed deficits on tests of ideomotor apraxia and movement imitation, four on tests of sequential arm movements (all of whom had ideomotor apraxia),

Callosal apraxia without agraphia.

We describe a patient with left unilateral ideomotor apraxia without left-sided agraphia caused by a callosal lesion that was demonstrated by magnetic resonance imaging. The clinical features, together with data in the literature, suggest that the callosal fibers for writing are concentrated in the posterior corpus callosum, while those for praxis cross in the more rostral part of the posterior half of the callosum.

Conceptual apraxia in Alzheimer's disease.

Theoretical models of praxis have two major components, a praxis conceptual system that includes knowledge of tool use and mechanical knowledge and a praxis production system that includes the information needed to program skilled motor acts. Because patients with Alzheimer's disease may have an impairment of the central conceptual system, we wanted to learn if they had a conceptual apraxia by testing their knowledge of the type of actions associated with tool use, their ability to associate tools with objects that receive their action, their ability to understand the mechanical nature of prob

Ideomotor apraxia in Huntington's disease.

The pattern of movement errors in ideomotor apraxia suggests an abnormality in selection and sequencing of component movements. Individuals with Huntington's disease were evaluated prospectively for the presence of apraxia, and aspects of motor and cognitive function were correlated with apraxic errors. Based on a conservative apraxia rating, ideomotor apraxia occurred in three (33%) of nine patients with a mean duration of disease of 10.4 years. Only two (22%) individuals made no apraxic errors, however, and the group as a whole made apraxic errors in 26% of movements. Apraxia was associated

[Speech apraxia--an apraxic disorder?].

This article presents arguments supporting the view that apraxia of speech is an apraxic disorder. In a first part the most salient clinical signs of the syndrome are reviewed. Among these, the symptom of inconsistent distortions and categorical aberrations of speech sounds is emphasized for its relevance in differentiating apraxia of speech from the dysarththrias on the one hand and from aphasic phonological disturbances on the other. In a second part, the diagnostic criteria of apraxia of speech are compared to those of oral (nonverbal) apraxia. In this comparison, a full correspondence betw

Motor learning in ideomotor apraxia.

Using Luria's motor sequence test, we studied the learning ability of 34 right-handed patients with unilateral hemisphere lesions. Patients with ideomotor apraxia needed 4 to 6 trials in the test, while the remainder needed only 1 or 2. Ideomotor apraxics also required more time to complete the test. The lesions of patients who failed to master this test were not always found in the frontal lobes. These results suggest that the ability to learn motor sequences is impaired in apraxic patients and that the left hemisphere of the brain plays the major role in learning a new motor sequence. Lip movement in apraxia of speech.

Peak articulatory velocity of the lower lip and temporal coordination between the upper and lower lips were studied in 5 neurologically impaired subjects with speech behaviors consistent with a diagnosis of apraxia of speech. Differences in velocity and the timing between the movement onset of the two lips were compared for accurate and inaccurate productions of words. Peak articulatory velocity also was measured during the productions of the syllable [pae] and during a nonverbal movement. There were no systematic differences across accurate and inaccurate productions of words in peak articula

Timing deficits in apraxia of speech.

This paper deals with a particular aspect of speech motor control in patients suffering from apraxia of speech. Three experiments are reported concerning the phase relations between individual speech gestures. These include the timing of laryngeal, velar and labial movements relative to lingual gestures. A total of 8 patients and 12 normal controls were examined using speech material which was designed according to appropriate phonetic paradigms. Evaluation was performed on the basis of speech signal parameters referring to the kinematics of inter-articulatory phasing. Deviations of the patien

Pantomime comprehension and ideomotor apraxia.

In a prior study it was shown that apraxic patients with posterior infarctions that included the parietal lobe could not discriminate between gestures. In this study these observations were replicated using a nonverbal paradigm in which the subjects did not have to discriminate between gestures, but instead had to comprehend their meaning. Pantomimed acts on videotape were shown to six apraxic-aphasic patients, seven nonapraxic-aphasic patients, and six normal subjects. Four drawings were also shown, one of which matched the pantomime (for example, if the pantomime was of hammering, one drawin
