Undetected hepatomegaly in obese children by primary care physicians: a pitfall in the diagnosis of pediatric nonalcoholic fatty liver disease.

The diagnostic evaluation of nonalcoholic fatty liver disease (NAFLD) relies on the initial detection of hepatomegaly or elevated serum aminotransferases by the primary care provider. In this investigation, the aptitude of the primary care provider to identify children with hepatomegaly and initiate an evaluation for NAFLD is determined. The physical examination findings and requests for diagnostic testing of 18 primary care physicians (and pediatric gastroenterologists) on 11 obese school-aged children, including a subset of children with hepatomegaly and NAFLD, were analyzed. In children wit

The impact of changes in medical school admission procedures on study success: A comparative analysis at Hannover Medical School.

BACKGROUND: In the academic year 2020/21, alterations were introduced in the admission procedures for medical studies, particularly within the selection quota (AdH). These changes reduced the significance of school-leaving grades (the Abitur) in favor of the Test for Medical Studies (TMS) and considerations of professional training & voluntary service as non-cognitive criteria. The waiting time regulation (WQ) was replaced by a "Special Aptitude Quota" (ZEQ), where experienced professionals were classified based on TMS results. This article examines whether and how those changes have influence

Local Brain Activity Differences Between Herpes Zoster and Postherpetic Neuralgia Patients: A Resting-State Functional MRI Study.

BACKGROUND: Herpes zoster (HZ) can develop into postherpetic neuralgia (PHN), both of which are painful diseases. PHN patients suffer chronic pain and emotional disorders. Previous studies showed that the PHN brain displayed abnormal activity and structural change, but the difference in brain activity between HZ and PHN is still not known. OBJECTIVES: To identify regional brain activity changes in HZ and PHN brains with resting-state functional magnetic resonance imaging (rs-fMRI) technique, and to observe the differences between HZ and PHN patients. STUDY DESIGN: Observational study. SETTI

Innovative programs for improvement in reading through cognitive enhancement: a remediation study of Canadian First Nations children.

Forty-five Grade 3 students from a reservation school in Western Canada were divided into two remedial groups and a no-risk control group. One remedial group was given a classroom-administered cognitive enhancement program (COGENT) throughout the school year. The second group received COGENT for the first half of the year followed by a pull-out cognitive-based reading enhancement program (PREP). Children were assessed across phonological awareness, rapid naming, reading, and cognitive ability at the beginning of the year, mid-term, and at the end of the school year. MANOVA results showed a sig

Genome and population dynamics during chronic infection with Helicobacter pylori.

Helicobacter pylori is responsible for one of the most prevalent bacterial infections worldwide. Chronic infection typically leads to chronic active gastritis. Clinical sequelae, including peptic ulcers, mucosa-associated lymphoid tissue lymphoma or, most importantly, gastric adenocarcinoma develop in 10-15% of cases. H. pylori is characterized by extensive inter-strain diversity which is the result of a high mutation rate, recombination, and a large repertoire of restriction-modification systems. This diversity is thought to be a major contributor to H. pylori's persistence and exceptional ap

Severity of prenatal cocaine exposure and child language functioning through age seven years: a longitudinal latent growth curve analysis.

The current study estimates the longitudinal effects of severity of prenatal cocaine exposure on language functioning in an urban sample of full-term African-American children (200 cocaine-exposed, 176 noncocaine-exposed) through age 7 years. The Miami Prenatal Cocaine Study sample was enrolled prospectively at birth, with documentation of prenatal drug exposure status through maternal interview and toxicology assays of maternal and infant urine and infant meconium. Language functioning was measured at ages 3 and 5 years using the Clinical Evaluation of Language Fundamentals--Preschool (CELF-P

Phenyliodine(III) Diacetate-Mediated 1,2-ipso-Migration in Mannich Bases of Imidazo[1,2-a]pyridines: Preparation of N-Acetoxymethyl/Alkoxymethyl-N-arylimidazo[1,2-a]pyridine-3-amines.

Phenyliodine(III) diacetate -mediated 1,2-ipso-migration of an imidazo[1,2-a]pyridine ring via the formation of an aziridine intermediate in Mannich bases derived from imidazo[1,2-a]pyridines, 2-pyridylamines or arylamines, and formaldehyde is reported. The imidazo[1,2-a]pyridines bearing different substituents showed excellent migratory aptitude and resulted in corresponding N-acetoxymethyl-, N-alkoxymethyl-, and N-hydroxymethyl-N-arylimidazo[1,2-a]pyridine-3-amine derivatives in moderate to excellent (42 examples; 35-93%) yields. A possible model for the inner wall of the acetylcholine receptor channel.

A structural model of the inner wall of the acetylcholine receptor (AChR) channel is developed using assumptions derived from the results of the recent labelling experiments of the MII helices by noncompetitive blockers. The assumption of steric blocking of the channel by chlorpromazine (CPZ) in the neighbourhood of the labelled serines imposes the MII helices to be in contact at this level and allows the calculation of their minimal interaxial distance. The assumption that CPZ diffuses to this position through the upper crowded part of the channel imposes that the helices are more distant in

Identifying targets for behavior change interventions to integrate biological maturation in German football development programs: A COM-B and Behavior Change Wheel analysis.

This study examines enablers and barriers to integrating biological maturation into talent identification and development processes within German football academies. Biological maturation significantly influences physical performance, selection decisions, and injury risk in youth football. Early maturing players are often favoured during talent identification, leading to premature deselection of talented late-maturing athletes. Using the COM-B framework (Capability, Opportunity, Motivation - Behaviour), we investigated stakeholders perceived barriers and enablers to implementing maturation str

The relative age effect is widespread among European adult professional soccer players but does not affect their market value.

BACKGROUND: The relative age effect (RAE) is most prevalent in highly competitive youth soccer and persists to a lesser extent in senior soccer. However, it is known that soccer players born in the second half of the year are as successful at senior level, indicating that they are equally talented although under-represented at youth level due to bias during the selection process, in which the emphasis is on more pronounced physical qualities in a specific period of time. Examining the prevalence of the RAE among professional soccer players depending on the level of competition and playing posi

A profile of spatial abilities in people with Down syndrome.

BACKGROUND: Spatial abilities are fundamental cognitive abilities, have direct applications in daily life, serve as a cognitive foundation for many other complex skills and are used in many specialty jobs. The current study aimed to systematically and comprehensively evaluate the spatial abilities of individuals with Down syndrome (DS) relative to mental ability-matched typically developing (TD) children based on Newcombe and Shipley's double-dimension theoretical framework for classifying spatial abilities. METHODS: Forty adolescents and young adults with DS and 40 TD children completed a no

The Savant Syndrome: a Gift or a Disability? A Deeper Look into Metabolic Correlates of Hidden Cognitive Capacity.

Savant syndrome is a rare and unusual condition that occurs in the presence of severe developmental disabilities, disorders, and injuries. The syndrome can be congenital from birth to childhood or acquired as a result of a brain injury or damage to the central nervous system. There are several findings that indicate that savant syndrome usually occurs with significant brain metabolism alterations resulting in critical brain network changes. These types of changes in the brain are usually explained by the "tyranny of the left hemisphere" theory, which indicates the inhibition of the left hemisp

Mathematical Abilities in Children With Developmental Language Disorder.

Purpose This review article provides a scoping review of the literature on mathematical abilities in developmental language disorder (DLD). Children with DLD typically struggle with learning in school; however, the mechanism by which DLD impacts academic success is unclear. Mathematics involves demands in the multiple domains and therefore holds potential for examining the relationship between language and academic performance on tasks mediated by verbal and nonverbal demands. Method A scoping review was performed via computerized database searching to examine literature on mathematics and DLD

[Mathematical learning disability: A multiple origin? Examples of Turner and Fragile X syndromes].

Problems in mathematics are a frequent major complaint in neuropediatric departments, for which there are two explanatory theoretical models: the hypothesis of a genetic and modular origin (with a number sense deficit) and a multidetermined origin. The purpose of this paper is to review the mathematical difficulties described in Turner syndrome and Fragile X syndrome, because a specific mathematical disorder is usually reported in these populations, supporting the existence of a number sense. Analysis of the literature reveals highly variable cognitive phenotypes in these populations, especial

The influence of spelling ability on handwriting production: children with and without dyslexia.

Current models of writing do not sufficiently address the complex relationship between the 2 transcription skills: spelling and handwriting. For children with dyslexia and beginning writers, it is conceivable that spelling ability will influence rate of handwriting production. Our aim in this study was to examine execution speed and temporal characteristics of handwriting when completing sentence-copying tasks that are free from composing demands and to determine the predictive value of spelling, pausing, and motor skill on handwriting production. Thirty-one children with dyslexia (Mage = 9 ye

Is dyslexia necessarily associated with negative feelings of self-worth? A review and implications for future research.

This paper will provide an overview of research in the past 20 years into the relationship between dyslexia and various aspects of self-perception, including self-concept, self-esteem, self-efficacy and locus of control. Problems are identified relating to the measurement of some of the most widely used constructs, as is the need for more precise identification of key variables relating to them. Critical issues concerning the assumptions that can be drawn from largely correlational data are highlighted also. The conclusion is drawn that an alternative approach to research in this area is requi

Longitudinal Association Between Sprint and Jump Performance in Youth Male Soccer Players: Implications for Talent Development.

This study analyzed the longitudinal relationship between short sprints and counter movement jump (CMJ) in youth male soccer players. Players (n = 304, age 13.0 ± 1.9 years) from a Scottish soccer academy completed sprints (10 m, 20 m splits [s]) and a CMJ (cm), on 3 to 14 occasions across 10 seasons. Within-player (repeated measures) and between-player (mean of repeated measures) correlation coefficients were calculated, and a linear mixed model (LMM) was applied to determine whether CMJ can predict sprint changes over a meaningful threshold (0.04 s, equivalent to the suggested minimum differ

Underlying reading-related skills and abilities among adult learners.

This exploratory study identified underlying skill and ability differences among subgroups of adolescent and young adult struggling readers (N = 290) overall and in relation to a fluency-based instructional grouping method. We used principal axis factoring of participants' scores on 18 measures of reading-related skills and abilities identified in the research literature to identify a smaller set of generally uncorrelated constructs. The four underlying factors of the 18 measures explained 62.7% of the variance. We labeled these factors Encode/Decode (44.5%), Vocabulary (9.5%), Processing Spee
