Changes in Ascitic Fluid Polymorphonuclear Cell Count After Antibiotics Are Associated With Mortality in Spontaneous Bacterial Peritonitis.

Spontaneous bacterial peritonitis (SBP) is a feared complication of ascites that affects 10%-30% of hospitalized patients with cirrhosis with an associated mortality rate of approximately 20%.1-3 Although efforts have been undertaken to encourage prompt evaluation and treatment of SBP, outcomes have generally remained dismal.3 There is significant interest in identifying factors that can reliably predict mortality among individuals with SBP.

Reappraisal of the Role of Ascitic Fluid Adenosine Deaminase for the Diagnosis of Peritoneal Tuberculosis in Cirrhosis.

BACKGROUNDS/AIMS: Although peritoneal tuberculosis (TB) is one of the important differential diagnoses among cirrhotic patients with ascites, a peritoneal biopsy is not always available. High ascitic fluid adenosine deaminase (ADA) has been indicative of peritoneal TB. On the other hand, studies to assess its diagnostic utility based on the confirmation of peritoneal biopsy in cirrhotic patients are scarce. METHODS: Patients with new-onset ascites were enrolled prospectively from a tertiary hospital. Peritoneal biopsy was applied according to clinical judgment when required. Based on patholog

Utility of Ascitic Fluid Adenosine Deaminase Levels in the Diagnosis of Tuberculous Peritonitis in General Medical Practice.

BACKGROUND: Tuberculous peritonitis is difficult to diagnose due to its varying clinical features, in addition to the low yield on bacterial culture or polymerase chain reaction using ascitic fluid samples. This study aimed to investigate the sensitivity and specificity of elevated adenosine deaminase (ADA) levels as a diagnostic marker for tuberculous peritonitis. METHODS: A retrospective cohort of 181 adult patients who underwent ascitic fluid ADA level examination at Jichi Medical University Hospital between January 2006 and December 2015 were included. We collected data regarding ascitic

Measurement of Adenosine Deaminase in Ascitic Fluid Contributed to the Diagnosis in a Case of Tuberculous Peritonitis.

A diagnosis of tuberculous peritonitis (TBP) is difficult because of nonspecific manifestation and limited effectiveness of conventional diagnostic tools. Recently, the usability of measurement of ascitic adenosine deaminase (ADA) was shown. We report here a case of TBP in which measurement of ascitic ADA contributed to the diagnosis. A 93-year-old male developed a large amount of ascites. Analyses of the ascitic fluid revealed exudation, though antibiotics treatment was ineffective. Using paracentesis, the ADA level in the ascites was measured and shown to be high. Under suspicion of TBP, an

A case of perforated sigmoid diverticulitis in which gram staining of ascitic fluid was useful for diagnosis.

An 85-year-old woman was admitted to our hospital for steroid therapy for relapsing nephrotic syndrome. During hospitalization, she complained of sudden epigastric pain at night. Although there were signs of peritoneal irritation, CT showed a large amount of ascitic fluid, but no free intraperitoneal gas. Gram staining of ascitic fluid obtained by abdominal paracentesis showed Gram-negative rods, which raised a strong suspicion of gastrointestinal perforation and peritonitis. Therefore, emergency surgery was performed. Exploration of the colon showed multiple sigmoid diverticula, one of which

Ascitic fluid due to type II herpes simplex virus infection: report of a case with immunocytochemical confirmation.

Herpes simplex virus (HSV) infection is usually observed in the oral cavity and external genitals, and HSV peritonitis is extremely rare. Herein, we report a case of type II HSV peritonitis successfully diagnosed by ascitic cytology. A 66-year-old Japanese man, who had been treated with steroid inhalation for 5 years due to chronic obstructive pulmonary disease, was suspected to have acute cholecystitis. Laparoscopic cholecystectomy and intraoperative cytological examination of ascitic fluid were performed. Cytological study of ascitic fluid revealed that abundant granular cell debris, degener

The physical properties of ascitic fluid enhance metastatic potential by inducing mitotic abnormalities and multinucleation in ovarian cancer.

The tumor microenvironment plays a critical role in cancer progression. Although its cellular and biochemical composition has been extensively studied, the impact of physical factors on tumor cell malignancy remains poorly understood. In ovarian cancer metastasis, tumor cells that spread to the coelomic cavity promote the secretion and accumulation of ascitic fluid (AF). Currently, the effect of the pH and the mechanical properties of this AF on metastatic progression are not well known. To advance our understanding of this problem, we employed both, biopsies from three patients (two with ovar

Role of ascitic and peritoneal fluid effusion cytology in predicting intraperitoneal metastasis of malignant ovarian germ cell tumors.

INTRODUCTION: Peritoneal cytology (PC) is a useful predictor of ovarian surface involvement and intraperitoneal metastasis in epithelial malignancies. Most malignancies involving serous cavity effusions are adenocarcinomas; however, nonepithelial tumors such as lymphomas and germ cell tumors (GCTs) rarely cause malignant effusion. Data on PC in malignant ovarian germ cell tumor (MOGCT) are limited to isolated case reports with lack of systematic cytohistologic correlation, hence the need for this study. MATERIALS AND METHODS: All PC samples of surgically resected MOGCTs were classified using

The prognostic significance of HDL-associated apolipoproteins in ascitic fluid from patients with cirrhosis and spontaneous bacterial peritonitis.

Apolipoproteins (Apo) A-I and A-II are major components of high-density lipoproteins (HDL) with immunomodulatory functions. Low serum apoA-I levels indicate complications of cirrhosis. We hypothesized that HDL-associated apolipoproteins in ascitic fluid (AF) are differentially abundant during spontaneous bacterial peritonitis (SBP). Serum and non-infected AF samples from 18 patients with cirrhosis (cohort 1) were analyzed using LC-MS/MS to identify differentially regulated apolipoproteins. In cohort 2, samples from 59 patients with SBP and 59 without were analyzed for AF apoA-II and apoA-I con

Serum and ascitic fluid interleukin-17 in spontaneous bacterial peritonitis in Egyptian patients with HCV-related liver cirrhosis.

BACKGROUND: Spontaneous bacterial peritonitis (SBP) is a potentially lethal complication of ascites. The inflammatory response is very intense in case of SBP despite low concentration of bacteria in the ascitic fluid with IL-17A overproduced by intestinal Paneth cells and may have role in host immune defense and inflammatory response. AIMS: To study the diagnostic performance of serum and ascitic fluid IL-17A as a marker of SBP and its correlation with renal function. METHODS: 120 cirrhotic patients including 80 patients with HCV-induced cirrhotic ascites but not with SBP and 40 patients wit

Routine analysis of ascitic fluid for evidence of infection in children with chronic liver disease: Is it mandatory?

Ascitic fluid infection is a major cause of morbidity and mortality in cirrhotic patients, requiring early diagnosis and therapy. We aimed to determine predictors of ascitic fluid infection in children with chronic liver disease. The study included 45 children with chronic liver disease and ascites who underwent 66 paracentesis procedures. Full history taking and clinical examination of all patients were obtained including fever, abdominal pain and tenderness and respiratory distress. Investigations included: complete blood count, C-reactive protein, full liver function tests, ascitic fluid bi

Cytological Findings of Ascitic Fluid with a Malignant Ovarian Steroid Cell Tumor: A Case Report and Literature Review.

BACKGROUND: Ovarian steroid cell tumors (SCTs) are rare and usually benign, although 25-43% are reportedly malignant. The cytologic findings of these rare ovarian tumors have almost never been reported. CASE: We report a rare case of a malignant ovarian SCT with peritoneal dissemination and malignant ascites in a 40-year-old woman. Her tumor was classified as stage IIB (pT2bNoM0) according to the FIGO (International Federation of Gynecology and Obstetrics) classification system, and she was treated with adjuvant chemotherapy following staging laparotomy. Cytology of the ascitic fluid revealed

Marked Increase in Zinc Concentration of Ascitic Fluid After Cell-free and Concentrated Ascites Reinfusion Therapy in Patients With Cirrhotic Refractory Ascites: A Retrospective Observational Study.

BACKGROUND/AIM: Zinc (Zn) deficiency is common among patients with cirrhosis and is associated with hepatic encephalopathy, immune dysfunction, and sarcopenia. In Japan, cell-free and concentrated ascites reinfusion therapy (CART) is widely employed for refractory ascites, effectively restoring albumin (ALB) and coagulation factors. However, its effect on trace elements has not been investigated. This retrospective study aimed to assess Zn concentration changes in ascitic fluid before and after CART in patients with cirrhotic refractory ascites. PATIENTS AND METHODS: Data from 24 patients wit
