United States Investigational Device Exemption study of the Revolution Peripheral Atherectomy System.

OBJECTIVE: Atherectomy has become commonplace as an adjunct to interventional treatments for peripheral arterial disease, but the procedures have been complicated by risks including distal embolization and arterial perforation. This study aimed to examine the safety and effectiveness of a novel atherectomy system to treat femoropopliteal and below-knee peripheral arterial disease. METHODS: The Revolution Peripheral Atherectomy System (Rex Medical LP, Conshohocken, Pa) was studied in 121 patients with 148 femoropopliteal and below-knee lesions, enrolled at 17 United States institutions. Successful angioplasty of saphenous venous graft in-stent restenosis using rotational atherectomy.

An 81-year-old, functionally independent woman with a history of bypass surgery and a previous vein graft intervention presented to us with significant angina refractory to medical management. An urgent angiogram revealed severe in-stent restenosis (ISR) in the proximal stent of the right coronary artery graft. At the same time, multiple trials to predilate the tight ISR using various-sized non-compliant and cutting balloons were unsuccessful. During the second attempt the next day, rotational atherectomy was used for plaque modification as the lesion appeared fibrocalcific angiographically. A

Percutaneous angioplasty versus atherectomy for treatment of symptomatic infra-popliteal arterial disease.

BACKGROUND: Outcomes for debulking by atherectomy (ATH) for adjunctive treatment of below the knee (BTK) symptomatic arterial disease compared to percutaneous transluminal angioplasty alone (PTA) are unclear. METHODS: MEDLINE, EMBASE, PubMed and the Cochrane Central Register of Controlled Trials were queried from between 2000 and 2017 including studies comparing PTA alone to PTA-ATH. Random effect meta-analysis model was used to pool the data across the studies. Study endpoints included: vessel dissection, residual stenosis (<30%), mortality at 12months and amputation rates at 1 and 12months.

Coronary artery atherectomy reduces plaque shear strains: an endovascular elastography imaging study.

Mechanical response and properties of the arterial wall can be used to identify the biomechanical instability of plaques and predict their vulnerability to rupture. Shear strain elastography (SSE) is proposed to identify vulnerable plaque features attributed to mechanical structural heterogeneities. The aims of this study were: 1) to report on the potential of SSE to identify atherosclerotic plaques; and 2) to use SSE maps to highlight biomechanical changes in lesion characteristics after directional coronary atherectomy (DCA) interventions. For this purpose, SSE was imaged using in vivo intra

Lower restenosis rate with stenting following aggressive versus less aggressive rotational atherectomy.

The present study evaluated the acute and follow-up results of stenting following aggressive rotational atherectomy compared with stenting following less aggressive rotational atherectomy. Recent work has demonstrated that stenting following rotational atherectomy is a promising strategy for complex and calcified lesions. However, there is little information available regarding the optimal procedural technique of rotational atherectomy to be employed before stent implantation. Between May 1995 and February 1997, 162 lesions in 126 patients were stented following rotational atherectomy because

Femoropopliteal Artery Atherectomy for Symptomatic Peripheral Artery Disease in the XLPAD Registry.

Despite widespread use of atherectomy in endovascular treatment of peripheral artery disease (PAD), it remains unsupported by major international guidelines, due to insufficient evidence of efficacy. This study compared atherectomy and nonatherectomy-based endovascular revascularization for femoropopliteal (FP) artery PAD. We analyzed data from the multicenter XLPAD registry (27 U.S. sites, 2007-2024; NCT01904851) to compare atherectomy versus nonatherectomy interventions for treating FP PAD. The primary outcome was technical failure or need for stenting during the index procedure. Technical f

Stent-Free Excimer Laser Atherectomy in AMI With Aplastic Anemia and Thrombocytopenia.

BACKGROUND: Acute myocardial infarction (AMI) complicated by aplastic anemia with severe thrombocytopenia presents a therapeutic dilemma because of conflicting bleeding and thrombotic risks. CASE SUMMARY: A 60-year-old man with aplastic anemia (platelet count: 28-32 × 109/L) and ST-segment elevation myocardial infarction underwent excimer laser coronary atherectomy (ELCA) combined with a stent-free strategy. Coronary angiography revealed multivessel disease with total right coronary artery occlusion. ELCA reduced thrombus burden, restored TIMI flow grade 3, and avoided the need for dual antip

Combined atherectomy and intravascular lithotripsy in calcified coronary lesions: a meta-analysis.

OBJECTIVES: Studies exploring the feasibility and safety of combined atherectomy and intravascular lithotripsy (IVL) for treatment of severely calcified coronary lesions are limited. The authors aimed to study the pooled evidence for the safety and efficacy of this combined approach in severely calcified coronary lesions and evaluate its clinical and procedural outcomes. METHODS: Online databases were searched for studies using combined atherectomy (rotational, orbital, or excimer laser) and IVL during PCI for calcified coronary lesions. The outcomes of interest were acute procedural success,

Incidence and Mechanisms of Coronary Perforations during Rotational Atherectomy in Modern Practice.

OBJECTIVE: Heavy calcifications remain formidable challenges to PCI, even for well-experienced operators. However, rotational atherectomy (RA)-induced coronary perforations (CPs) still could not be obviated. This study was to explore incidence and mechanisms of RA-induced CP in real-world practice. Knowing why CPs occur in RA should help operators avert such mishaps. METHOD: Patients who received coronary RA from April 2010 to December 2019 with keywords related to perforations were retrieved from database. The procedure details, angiography, and clinical information were reviewed in detail.

Direct Stenting in Patients Treated with Orbital Atherectomy: An ORBIT II Subanalysis.

BACKGROUND: Direct stenting offers many potential advantages in appropriately selected lesions. Coronary artery calcification increases the complexity and risk of adverse events associated with percutaneous coronary intervention. This study aimed to examine the feasibility of direct stenting after treatment with orbital atherectomy (OA). METHODS: ORBIT II was a single-arm trial enrolling 443 subjects with de novo severely calcified coronary lesions treated with OA; direct stenting was utilized in 59.0% of cases. Procedural outcomes and 3-year major adverse cardiac event (MACE) rates were comp

Endovascular SilverHawk directional atherectomy for thromboangiitis obliterans with occlusion of the popliteal artery.

Thromboangiitis obliterans (TAO), also known as Buerger disease, is a nonatherosclerotic segmental inflammatory disease of small and medium-sized arteries of the distal extremities occurring predominantly in young men who are long-term tobacco smokers. We treated 2 patients who presented with TAO involving popliteal and tibial arteries. Directional atherectomy with a SilverHawk device was used to recanalize the arteries without major complications. During follow-up conducted using computed tomography or sonography, we observed that the treated vessels remained patent. These cases report illust

Midterm patency following atherectomy for infrainguinal occlusive disease: a word of caution.

There has been widespread initial enthusiasm for peripheral atherectomy using the SilverHawk device. We sought to evaluate our midterm patency following infrainguinal atherectomy. Nineteen consecutive patients underwent 23 separate atherectomy procedures on 20 limbs from March 2005 through June 2006 (11 males, age 66 +/- 14 years). The primary lesions were atherosclerotic (n = 18) and vein graft stenoses (n = 2). Three additional procedures were redo atherectomies for restenotic lesions. The TASC classification of the primary lesions was A in 3, B in 9, and C in 8. The median number of treated

Effect of Rotablator atherectomy and adjunctive balloon angioplasty on coronary blood flow.

BACKGROUND: The purpose of this study was to assess serial changes in coronary blood flow velocity before and after Rotablator atherectomy and after adjunctive percutaneous transluminal coronary angioplasty (PTCA). Since Rotablator atherectomy results in luminal enlargement by plaque pulverization and distal embolization, improvement in coronary blood flow could be attenuated despite luminal enlargement. METHODS AND RESULTS: Intracoronary Doppler blood flow velocity measurements were obtained with a Doppler Flowire. Basal average peak velocity (bAPV), hyperemic APV (hAPV), diastolic/systolic

Guidewire Traction Retrieval of a Dislodged Classic Crown During Coronary Orbital Atherectomy.

BACKGROUND Heavily calcified coronary lesions often lead to stent underexpansion and procedural complications. Treatment comprises plaque modification techniques, such as atherectomy. Orbital atherectomy (OA) is effective for preparing severely calcified lesions prior to stenting. Device-related complications, including crown detachment, are rare but potentially serious. CASE REPORT A 70-year-old man with stable angina and 90% proximal right coronary artery stenosis displayed concentric, heavy calcification on intravascular ultrasound. OA was performed using the Diamondback 360 Orbital Atherec

Novel Therapeutic Concepts for Complex Femoropopliteal Lesions Using the Jetstream Atherectomy System.

INTRODUCTION: The presence of severe arterial calcification is associated with less favorable outcomes in terms of procedural and clinical success as well as higher rates of major adverse limb events. Recent studies incorporating rotational atherectomy for effective preparation of severely calcified lesions demonstrate beneficial procedural outcomes by obtaining maximal luminal gain and improved long-term outcomes. METHODS: This prospective single-center, observational study includes patients with severely calcified femoropopliteal lesions with chronic limb ischemia Rutherford 1-5 between Jan
