We present two cases with chronic total occlusion of the saphenous vein graft in two patients with a history of previous bypass surgery with unfavorable anatomic features for recanalization of the native coronary artery. In the first case, two dedicated attempts for recanalization of chronic total occlusion of the native artery failed and in the second case there was not an adequate visualization of the native vessel beyond the occlusion point, not even by contralateral injection. Excimer laser atherectomy was used in both cases as an adjuvant therapy during recanalization of the saphenous vei

OBJECTIVES: Primary coronary atherosclerotic lesions(de novo lesions) are a type of inflammatory vascular disease. Restenotic lesions after percutaneous coronary intervention mainly consist of proliferative vascular smooth muscle cells. Recent studies have demonstrated that locally synthesized cytokines, including chemokines, are important in both these coronary lesions. Monocyte chemoattractant protein(MCP)-1 and macrophage colony-stimulating factor(M-CSF) are two of the associated chemokines, but their role in coronary artery disease has not been sufficiently clarified. This study investigat

BACKGROUND AND AIMS: Calcific coronary lesions pose significant challenges to percutaneous coronary intervention (PCI), limiting stent delivery and expansion. Intravascular lithotripsy (IVL) and rotational atherectomy (RA) are widely used plaque modification techniques; however, comparative data on their effectiveness remain limited. We aimed to compare clinical and procedural outcomes between IVL and RA in the management of calcific coronary lesions.
METHODS: PubMed, Embase, Scopus, and Cochrane Library were searched through January 2025 for randomized controlled trials (RCTs) and observatio

BACKGROUND: Suboptimal stent delivery and deployment in calcified coronary lesions are associated with a poor clinical outcome.
METHODS: Using the National Inpatient Sample database, we identified patients undergoing percutaneous coronary intervention (PCI). Comparison between procedural and hospital outcomes between patients who underwent atherectomy and those who did not.
RESULTS: A total of 2,035,039 patients underwent PCI, of which 50,095 (2.4%) underwent lesion modification using atherectomy. After adjustment for baseline differences, patients who underwent atherectomy were found to hav

BACKGROUND: The combined predictive value of type 2 diabetes mellitus (T2DM) and coronary microvascular dysfunction (CMD) in rotational atherectomy (RA) patients during the PCI remains unclear. The study examined whether the co-occurrence of DM and CMD, determined by angiography-derived index of microcirculatory resistance in RA patients influences clinical outcomes.
METHODS: This was a retrospective, multicenter, observational study involving 452 patients at 3 medical centers. The primary endpoint was the occurrence of major adverse cardiac events (MACEs) at 24 months after the procedure, en

Directional Coronary Atherectomy (DCA) is a potential treatment option for left main (LM) bifurcation lesions, as it may prevent side branch occlusion and reduce the need for complex stenting. Recent studies have suggested that combining drug-coated balloon (DCB) with DCA can lead to favorable cardiovascular outcomes. However, the comparative efficacy of DCB and current drug-eluting stents (DES) following DCA for LM bifurcation lesions remains unclear. This study aimed to compare the clinical outcomes of DCB and DES following DCA for LM bifurcation lesions. A retrospective analysis was conduct

BACKGROUND: Drug-coated balloon angioplasty after directional coronary atherectomy (DCA) allows for a stentless strategy providing good short-term outcomes; however, late-phase restenosis and its mechanism remain unclear. Moreover, histopathological evaluation for late-phase restenosis post-drug-coated balloon angioplasty after DCA has never been reported.
CASE SUMMARY: We report the first case of late-phase restenosis post-drug-coated balloon angioplasty after DCA, wherein tissue analysis using intravascular coronary imaging and histopathology suggested neovascularization in newly developed

A 40-year-old man was referred to our hospital for the treatment of severe calcified coronary lesions of right coronary artery due to Kawasaki disease. He had a past history of early stent thrombosis after drug-eluting stent implantation for acute myocardial infarction at the left anterior descending artery, regardless of continued dual antiplatelet therapy and no stent malapposition and underexpansion. We performed rotational atherectomy and plain balloon angioplasty with optical frequency domain imaging guidance, and obtained an excellent initial result without stent implantation, which was

OBJECTIVES: To assess long-term outcome after rotational atherectomy (RA) is followed by drug-eluting stent (DES) implantation in complex calcified coronary lesions.
BACKGROUND: RA can favorably modify heavily calcified coronary lesions, but long-term outcome is poor when it is used as a stand-alone therapy or combined with bare-metal stents. DES have reduced rates of restenosis in a wide range of patient and lesion subsets, but little information is available on long-term clinical outcome when RA is followed by DES implantation (Rota-DES) in complex calcified lesions.

BACKGROUND: Adjunctive coronary atherectomy (CA) can be utilized in treating severely calcified coronary lesions; however, the temporal trends, patient selection, and variation in use of CA have not been well described. We sought to assess the trends in usage, interhospital variability, and outcomes with CA among patients undergoing percutaneous coronary intervention (PCI).
METHODS: All patients undergoing PCI in the National Cardiovascular Data Registry CathPCI Registry from July 1, 2009 to December 31, 2016 (N=3 864 377) were analyzed based on utilization of either rotational or orbital CA.

Stent underexpansion is a risk factor for in-stent restenosis (ISR) and stent thrombosis. Although excimer laser coronary atherectomy (ELCA) with contrast medium was shown to be effective to improve stent underexpansion in undilatable lesions, precise mechanism of this technique was not well understood. We report a case of ISR with stent underoptimal implantation within a circumferential-calcified atherosclerotic plaque beneath the stent strut that could not be dilated by repeated high-pressure balloon inflations. After rotational atherectomy, the mechanism of the underexpanded lesion could be

INTRODUCTION: Severe coronary artery calcification (CAC) can complicate percutaneous coronary intervention (PCI) and is associated with less favourable procedural and clinical outcomes. Rotational atherectomy (RA) and orbital atherectomy (OA) are established calcium-modification strategies used to facilitate PCI in heavily calcified lesions. This systematic review and meta-analysis evaluated the comparative clinical and procedural outcomes of RA and OA in patients with severe CAC.
METHODS: This systematic review and meta-analysis was conducted in accordance with PRISMA 2020 guidance. PubMed,

BACKGROUND: This study aimed to investigate whether clinical syntax score (CSS) could predict in-hospital and long-term outcomes in patients with acute coronary syndrome (ACS) who underwent drug-eluting stent implantation following rotational atherectomy (RA). METHODS: A total of 179 patients with ACS who underwent drug-eluting stent implantation following RA were recruited. Angiographic parameters, age, creatinine, and left ventricular ejection fraction were used to calculate CSS in the enrolled patients. The patients were classified into low-CSS (n = 89) and high-CSS (n = 90) groups. Major a

BACKGROUND: Plaque modification is crucial for successful stent expansion in calcified coronary lesions. Although rotational atherectomy (RA) improves initial procedural success compared to balloon predilatation, data comparing elective versus bailout RA, particularly in chronic kidney disease (CKD) patients, are scarce.
AIMS: To perform a cost analysis of elective RA (E-RA) versus bailout RA (B-RA) to treat severely calcified lesions in patients with CKD, comparing procedural material cost and total healthcare cost.
METHODS: This prospective, multicentre, randomized study compared the costs

BACKGROUND: The presence of coronary artery calcification during percutaneous coronary intervention is associated with an increased risk of complications and poor outcomes. Comparison of the efficacy and safety of intravascular lithotripsy and rotational atherectomy in coronary artery calcification has always been a topic of debate in the literature.
AIM: To present the most recent comparative analysis of these two calcium debulking techniques.
METHODS: A search was conducted in online databases, including PubMed, Web of Science and the Cochrane Library. We performed a meta-analysis to find

BACKGROUND: Coronary artery disease (CAD) is a common finding among patients undergoing trans-catheter aortic valve implantation (TAVI), who often present severely calcified coronary lesions. Evidence is scarce about the use of rotational atherectomy (RA) in this setting, in particular regarding long-term outcome.
METHODS: RA was performed on severely calcified coronary lesions concomitant with TAVI in a consecutive series of patients treated between 2010 and 2020. Immediate and long-term clinical outcomes are reported.
RESULTS: A concomitant CAD (coronary stenosis visually > 50%) was observ

BACKGROUND: Balloon dilatation or debulking seems to be essential to allow successful stent implantation in calcified coronary lesions. Compared with standard balloon predilatation, debulking using high-speed rotational atherectomy (RA) is associated with higher initial procedural success albeit with higher in-stent late lumen loss at intermediate-term follow-up. Whether modified (scoring or cutting) balloons (MB) could achieve similar procedural success compared with RA is not known. In addition, whether new-generation drug-eluting stents could counterbalance the excessive neointimal prolifer
