Cochlear potentials in clinical audiology.

The recording of cochlear and auditory nerve potentials in humans via Electrocochleography (ECochG) has emerged as a valuable tool for a variety of clinical applications. This review consolidates current research on the use of cochlear potentials and ECochG in the clinical setting and identifies several areas in need of additional study. Methodological topics discussed include a review of ECochG recording approaches (i.e. transtympanic versus extratympanic) and issues related to choice of stimuli (clicks versus tonebursts). The review of current applications for cochlear potentials focuses pri

[Electrocochleography and the glycerol test in the diagnosis of Meniére's disease].

ECochG and glycerol test are two kinds of objective examinations for the diagnosis of Ménière's disease. Previous reports showed that the positive rates in both methods vary widely. In the present study, the incidence of abnormal SP enhancement was 63% (51 out of 81 ears in 70 patients). The SP enhancement correlated with the symptoms, especially aural fullness and high frequency hearing loss. The glycerol test was preformed in 25 patients (29 ears). The positive rate was 55.2%. There was a high positive rate of glycerol test in patients with low frequency and fluctuant hearing loss. Both exam

Sensitivity and specificity of audiological tests in patients with vertigo.

In patients with vertigo a comprehensive test battery has been used for localizing pathological processes responsible for the symptoms. Audiological tests have been an integral part of the otoneurological investigation. However, in many studies the diagnostic sharpness of psychoacoustic tests in the localization of specific disease in the cranial nerve system has been called in question. Other tests, such as stapedius reflex and auditory brainstem response, have been of increasing importance in the diagnostic set-up. The purpose of this paper is to present a survey of the literature regarding

Considerations in the differentiation between brainstem lesions and sensori-neural hearing loss using auditory nerve-brainstem evoked responses.

When auditory nerve-brainstem evoked responses are used in neurological diagnosis, an important response parameter to be evaluated is brainstem transmission time - the time interval between the auditory nerve response (wave 1) and the response from the rostal brainstem (wave P4). Occasionally the only response wave present is wave P4 and its latency is prolonged. This can be compatible with a conductive or a sensorineural hearing loss (SNHL), or a neurological (brain stem) lesion such as a retrocochlear hearing loss (RCHL). A conductive hearing loss can be excluded by otoscopic, audiometric an

Pattern of gentamicin-induced cochlear degeneration in the guinea pig. A morphological and electrophysiological study.

Gentamicin-induced cochlear degeneration in the guinea pig was studied by complete hair-cell counting (cytocochleograms) and phase-contrast and interference microscopical examination of the stria vascularis and Reissner's membrane. Gentamicin (100 mg/kg/day) was administered over a period of 7-17 days. The first loss of hair cells (OHC) occurred in a region 6-8 mm from the round window. From this 'degeneration point', the loss of haircells progressed towards the round window (fast) and the apex (slowly). The stria vascularis showed no signs of degeneration. Reissner's membrane, on the other ha

Peripheral hearing, brainstem auditory responses, and P300 potentials in Parkinson's disease: A cross-sectional study with one-year follow-up.

BACKGROUND: Auditory dysfunction and cognitive impairment are increasingly recognized as non-motor features of Parkinson's disease (PD), but longitudinal data integrating peripheral hearing, brainstem auditory responses, and cortical auditory-cognitive processing are limited. This study compared pure-tone audiometry, brainstem auditory evoked responses, and auditory P300 potentials between patients with PD and healthy controls at baseline and one year. METHODS: This cross-sectional study with one-year follow-up evaluated pure-tone audiometry, Brainstem Evoked Response Audiometry (BERA), and a

Investigation of cochlear microphonics recorded with different stimulus types.

BACKGROUND: Electrocochleography (ECochG), one of the first defined tests under auditory evoked potentials, is a total electrical response of inner and outer hair cells inside the cochlea and auditory nerve record technique to the presence of an acoustic stimulus. These records can be used in Meniere disease and auditory neuropathy spectrum disorder diagnosis, intraoperative monitoring. In addition, the presence of cochlear microphonics plays a crucial role in auditory neuropathy spectrum disorder diagnosis. In our study, healthy individuals were tested with extratimpanic electrocochleography

The influence of aging on auditory brainstem response and electrocochleography in the elderly.

Auditory brainstem responses and simultaneous transtympanic electrocochleographies were investigated in 92 subjects aged between 50 and 89 years with normal hearing or mild presbycusis. Subjects were classified into four age-related groups: 16 subjects in the sixth decade, 28 in the seventh decade, 32 in the eighth decade and 16 in the ninth decade. Thirty young subjects with normal hearing aged 20-29 years were chosen as controls. The result showed progressive delay of wave I (or action potential: AP), wave III and wave V and lengthening of the interpeak intervals (IPIs) of waves III-V and I-

Comparison between AP and SP parameters in trans- and extratympanic electrocochleography.

Results of the comparison of action potential (AP) and summating potential (SP) parameters in trans- (TT) and extratympanic (ET) methods in 2 normal-hearing ears and 20 hearing-impaired ears with various etiologies and audiogram shapes are reported. In normal and impaired ears the TT/ET ratio of N1 amplitude was intensity dependent and was greater at higher intensities than at lower intensities. N1 latency was identical in both methods. The AP waveform was almost identical in both methods in ears except in a noise-induced hearing loss: N1/N2 ratio was greater in the TT method than in the ET me

The vestibulo-collic reflex.

Until now there has no been no simple, reliable clinical test that assesses the function of the otolith organs. However, recent work has suggested the occurrence of an easily identifiable electromyographic (EMG) waveform in isometrically contracting sternomastoid muscles in response to loud clicks. This is thought to represent an otolith evoked disynaptic reflex. Equipment routinely used for evoked response audiometry (ERA) has been adapted to investigate this response. A short latency (8 ms) positive-negative EMG waveform has been recorded in 31 of 32 normal subjects. The latency of the posit

Optimizing non-invasive functional markers for cochlear deafferentation based on electrocochleography and auditory brainstem responses.

Accumulating evidence suggests that cochlear deafferentation may contribute to suprathreshold deficits observed with or without elevated hearing thresholds, and can lead to accelerated age-related hearing loss. Currently there are no clinical diagnostic tools to detect human cochlear deafferentation in vivo. Preclinical studies using a combination of electrophysiological and post-mortem histological methods clearly demonstrate cochlear deafferentation including myelination loss, mitochondrial damages in spiral ganglion neurons (SGNs), and synaptic loss between inner hair cells and SGNs. Since

SP/AP area ratio in the diagnosis of Ménière's disease.

Conventional analysis of the click-evoked electrocochleogram includes measurement of the amplitudes of the cochlear summating potential (SP) and auditory nerve action potential (AP) to derive the SP/AP amplitude ratio. It is now well documented that an amplitude ratio where SP is elevated relative to AP may be a positive finding for endolymphatic hydrops in patients suspected of having Ménière's disease (MD). However, the incidence of an elevated SP/AP amplitude ratio in the general MD population is only approximately 60%. This relatively low value necessitates efforts to improve the diagnosti

[Neonatal hearing screening].

Auditory deprivation in early life significantly effects speech, language, cognition and learning development. The importance of neonatal hearing screening in order to detect and manage children with hearing impairment during the critical period before three years of age can't be neglected. The neonatal hearing screening program at Veterans General Hospital, Taipei, under a cooperation of audiologists, otologists and pediatricians, identified 1885 neonates for hearing loss from September 1990 to August 1991. There were two study groups, a high risk group of hearing impairment and a non-high ri

The effect of furosemide on evoked otoacoustic emissions in guinea pigs.

After recording transiently evoked otoacoustic emissions (TEOAEs) to a click stimulus in guinea pigs by using the IL088 which was developed by Bray and Kemp (1987) for easy recording and analysis of TEOAE, the changes after intravenous administration of furosemide (30 mg/kg or 50 mg/kg) were examined. The wave of the TEOAE could be detected from 20 of 24 ears (83%). After the i.v. injection of furosemide (30 mg/kg), TEOAE powers (total echo power and highest peak power in FFT pictures) decreased quickly and showed minimum values after 5-10 min. Then they increased rapidly and recovered normall

Congenital conductive hearing loss in Apert syndrome.

Acrocephalosyndactyly (Apert syndrome) is a rare craniosynostotic syndrome characterized by acrocephaly, syndactyly of the hands and feet, and--occasionally--conductive hearing loss. We report three cases of conductive hearing loss in Apert syndrome. One patient was found to have bilateral stapes fixation. His daughter (the second case) had chronic bilateral otitis media with effusion. The third case involved a fixed incus and hypomobile stapes. The management of these patients and a review of the literature are presented.
