Epstein-Barr virus-associated meningitis presenting with hearing impairment.

A 42-year-old man presented with fever, headache and liver dysfunction, and was diagnosed as having aseptic meningitis by lumbar puncture. The PCR detected Epstein-Barr virus (EBV)-DNA in the peripheral blood and cerebrospinal fluid. About 20 days after onset, the patient presented with hearing impairment in the right ear, which was confirmed by a pure tone audiogram. The hearing acuity improved after the initiation of hydrocortisone sodium phosphate. We presume that the hearing impairment was due to auditory nerve neuritis related to extension of inflammation of the meninges. This is the firs

[Comparison of NB Chirp ABR and 40 Hz AERP in Normal Hearing Adults].

OBJECTIVES: To investigate the relationship between the narrow band Chirp auditory brainstem response (NB Chirp ABR) response thresholds, 40 Hz auditory event related potential (40 Hz AERP) response thresholds and pure-tone audiometry (PTA) thresholds in normal hearing adults under sound stimulation of different frequencies. METHODS: A total of 100 ears of 50 normal-hearing adult subjects were selected as the experimental group. PTA, NB Chirp ABR and 40 Hz AERP tests were conducted at the frequencies of 0.5, 1, 2, and 4 kHz. The relationship between the NB Chirp ABR, 40 Hz AERP response thres

Diagnostic accuracy of tuning fork weber test and audiometric weber test in school-aged children with conductive hearing loss.

PURPOSE: While the application of tuning fork Weber (TFW) test in adult is well-documented, its accuracy in the paediatric population remains uncertain although conductive hearing loss is frequently identified in children. Similarly, the effectiveness of the audiometric Weber (AW) test is ambiguous. Given evidence suggesting that age influences lateralization accuracy and understanding of instructions, this research specifically targets school-aged children to evaluate these diagnostic tools effectively. This study aims to explore the lateralization accuracy of TFW and AW tests in detecting co

Effect of external auditory canal pressure upon the hearing threshold in patients with Menière's disease.

Fluctuation of hearing at low frequencies is one of the most characteristic findings in Meniere's disease and seems to be a phenomenon closely related to changes of endolymphatic pressure and volume. In the present study, pressure was applied to the external auditory canal of patients with Meniere's disease, and the effect on the bone conduction threshold was examined at times of depressed and improved hearing. In more than half of these cases, it was found that the pressure effect was marked at the time of improved hearing acuity, but not at the time of depressed hearing. This effect was also

The loose wire syndrome.

The loose wire syndrome occurs in patients who have had stapedectomy surgery and insertion of a prosthesis which attaches to the long process of the incus by means of a crimped wire. The syndrome consists of a triad of one or more symptoms which improve temporarily with middle ear inflation. These include improvements in auditory acuity, in distortion of sound, and in speech discrimination. Forty-three patients were studied for age, sex, and symptom distribution, and for results of corrective surgery. In all cases, a loose wire at its attachment to the incus was found and corrective surgery (t

Hearing loss in a whirlin mouse mutant is not reversed by activating the Whrn gene.

WHRN gene mutations underlie several forms of deafness in humans and mice. It is not known whether WHRN-associated hearing loss can be reversed after its onset and we aimed to address this using a genetic approach. We show that a knockdown mouse allele of Whrn, Whrntm1a, caused reduced transcription of Whrn and early onset recessive hearing loss. A tamoxifen-inducible Flp recombinase (Flpo) was used to remove the transcription disruption cassette of the Whrntm1a allele (conversion to Whrntm1c) to allow expression of the Whrn gene after the onset of hearing loss. Tamoxifen administration at pos

A non-invasive test for stria vascularis dysfunction.

Age-related hearing loss is common in the human population but it is highly heterogeneous in aetiology which has hampered efforts to develop ways of stopping its progression. Three major sites of the initial dysfunction are the sensory hair cells, their innervation, and the stria vascularis which generates the high potassium endolymph maintained at a high endocochlear potential bathing the apical surface of hair cells. Treatments aimed at the initial site-of-lesion may be useful, and diagnostic tools to distinguish the primary site would help stratify clinical trials and facilitate selection o

Simultaneous cochlear implantation during retrosigmoid resection of a large vestibular schwannoma under continuous electrically evoked auditory brainstem response monitoring.

Hearing preservation during surgery for large vestibular schwannomas (VS) remains challenging, and reports of simultaneous cochlear implantation (CI) via the retrosigmoid approach are exceedingly rare. We report, through close collaboration between neurosurgery and otolaryngology, what is, to our knowledge, the first case of simultaneous CI during retrosigmoid resection of a large sporadic VS under continuous intraoperative electrically evoked auditory brainstem response (EABR) monitoring. A 70-year-old woman with a 30-mm Koos grade 4 sporadic VS and contralateral profound hearing loss present

A preliminary study on the relationship between central auditory processing and childhood primary headaches in the intercritical phase.

BACKGROUND: Recently, an increasing number of articles have appeared on central auditory processing disorders, but in the literature there is only one study that evaluated the possible correlation between migraine in the critical phase and central auditory processing. The aim of our study was to assess the correlation between auditory processing information and childhood primary headaches in the intercritical phase. METHODS: This is an observational study. We enrolled 54 patients, 30 with primary headache (migraine and tension headache) and 24 normal controls, matched for sex and age. The mea

Performance of normal adults and children on central auditory diagnostic tests and their corresponding visual analogs.

BACKGROUND: It has been suggested that, in order to validate a diagnosis of (C)APD (central auditory processing disorder), testing using direct cross-modal analogs should be performed to demonstrate that deficits exist solely or primarily in the auditory modality (McFarland and Cacace, 1995; Cacace and McFarland, 2005). This modality-specific viewpoint is controversial and not universally accepted (American Speech-Language-Hearing Association [ASHA], 2005; Musiek et al, 2005). Further, no such analogs have been developed to date, and neither the feasibility of such testing in normally function

The Effects of Chronic Substance Misuse on the Auditory and Vestibular Systems: Preliminary Findings.

OBJECTIVES: Accumulating reports suggest that substance use disorders (SUDs) potentially lead to hearing and/or vestibular loss, particularly when overdose is involved. However, the existing literature is weak, consisting of case reports or small sample studies and that did not comprehensively evaluate both the auditory and vestibular systems. The objectives of this study were to determine the effect of chronic substance misuse on behavioral and physiological auditory and vestibular outcomes in adults with substance misuse histories (SUD group) as compared with an age- and sex-matched control

Functional diversity of brain networks supports consciousness and verbal intelligence.

How are the myriad stimuli arriving at our senses transformed into conscious thought? To address this question, in a series of studies, we asked whether a common mechanism underlies loss of information processing in unconscious states across different conditions, which could shed light on the brain mechanisms of conscious cognition. With a novel approach, we brought together for the first time, data from the same paradigm-a highly engaging auditory-only narrative-in three independent domains: anesthesia-induced unconsciousness, unconsciousness after brain injury, and individual differences in

Traumatic brain injury impact on normal hearing.

Traumatic brain injury (TBI) incidents bring drastic health adversities in multiple systems of the body. The immediate impact on the head and central nervous system is promptly accelerated to the autonomous and peripheral nervous system. It causes an instant variability in intracranial pressure, heart rate, blood pressure, normal function of chemo/baro receptors, and sudden failure in normal communication. These effects promote long-term secondary health adversity, such as nerve schwannoma, gliosis, Wallerian degeneration, mitochondrial dysfunction, change in blood-brain barrier pathology, and

[The role of TXNIP-mediated ferroptosis in the senescence of cochlear hair cells in age-related hearing loss].

Objective: To investigate the role and molecular mechanism of Thioredoxin-interacting protein (TXNIP) in the pathogenesis of age-related hearing loss (ARHL), focusing on whether it mediates cochlear hair cell senescence by regulating the ferroptosis pathway, thereby providing new theoretical insights and potential therapeutic targets for the prevention and treatment of ARHL. Methods: 1-month-old and 15-month-old male C57BL/6 mice were selected to detect changes in TXNIP levels in their serum and cochlea. 5-month-old male C57BL/6 mice were used to establish a senescence model via intraperitonea

Long-term Hearing Outcomes and Prognostic Factors in Patients with Meniere's Disease.

BACKGROUND: Meniere's disease (MD) presents with vertigo and fluctuating auditory symptoms, linked to endolymphatic edema. Increased antidiuretic hormone levels due to stress suggest that hydration therapy (HT) can suppress this hormone. This study examined hearing outcomes in patients with MD treated with HT and factors influencing hearing fluctuations. METHODS: The study included 188 participants (45 men and 143 women) with definite MD who were followed for over 4 years with HT (35 mL/kg water daily). Clinical characteristics, hearing thresholds, and hematological findings were analyzed. Th

Diffuse hepatic amebiasis detected by FDG PET/CT.

A 39-year-old man presented with sudden decreased visual acuity in his left eye. Orbital CT and MRI revealed a soft tissue lesion in his left orbital apex. FDG PET/CT showed increased FDG uptake by the left orbital lesion, abnormal focal FDG uptake in the soft tissues of the external ears, and abnormal heterogeneous FDG activity throughout the liver. Percutaneous liver biopsy, external auditory canal discharge, and stool specimens revealed amebiasis. The patient responded to antiamebic therapy, and his lesions improved. The case demonstrates that during its early stage, hepatic amebiasis may b

Ocular and auditory toxicity in hemodialyzed patients receiving desferrioxamine.

During an 18-month period of study 41 hemodialyzed patients receiving desferrioxamine (10-40 mg/kg BW/3 times weekly) for the first time were monitored for detection of audiovisual toxicity. 6 patients presented clinical symptoms of visual or auditory toxicity. Moreover, detailed ophthalmologic and audiologic studies disclosed abnormalities in 7 more asymptomatic patients. Visual toxicity was of retinal origin and was characterized by a tritan-type dyschromatopsy, sometimes associated with a loss of visual acuity and pigmentary retinal deposits. Auditory toxicity was characterized by a mid- to

Furosemide prevents noise-induced hearing loss and enhances the preventive effect of N-acetylcysteine.

Disruption of reactive oxygen species (ROS) homeostasis is a key mechanism underlying noise-induced sensory hair cell damage. Antioxidant treatments such as N-acetylcysteine (NAC) have been shown to attenuate noise-induced hearing loss (NIHL), supporting the role of ROS accumulation. However, no FDA-approved pharmaceutical therapy currently exists for the prevention or treatment of NIHL, likely due to the complexity of the damaging mechanisms and the presence of the blood-labyrinth barrier (BLB), which limits drug permeability and prevents therapeutic compounds from reaching effective concentr

Ototoxicity and pharmacokinetically determined dosages of amikacin in granulocytopenic cancer patients.

Pharmacokinetic principles were used to determine amikacin doses for granulocytopenic cancer patients on empirical antibiotic regimens, and audiometry was used to determine the effect of this treatment on auditory acuity. Patients received ticarcillin 300 mg/kg/day plus amikacin, or moxalactam 8 g/day plus amikacin, in a blinded study. Amikacin doses were calculated to achieve a peak serum concentration (one hour after infusion) of approximately 25 micrograms/ml and a trough concentration of approximately 8 micrograms/ml. Baseline audiometry was conducted, and follow-up audiometry was done on
