Comparison of antimicrobial susceptibility testing methods for aztreonam-avibactam against NDM-producing Enterobacterales clinical isolates.

Aztreonam-avibactam (ATM-AVI) is a promising therapeutic for New Delhi metallo-β-lactamase-producing Enterobacterales (NDM-E) infections. We evaluated the performance of disk diffusion and gradient diffusion methods compared with reference broth microdilution (BMD) for ATM-AVI susceptibility testing. A total of 245 non-duplicate NDM-E isolates were included, collected from unique patients at hospitals in Chile and the United States between 2022 and 2024. Nine ATM-AVI non-susceptible isolates from a separate collection were added to enrich for resistant isolates, yielding a total of 254 isolate

Clinical Emergence and Genomic Characterization of Aztreonam-Avibactam-Resistant Escherichia coli ST410 Isolates in China.

BACKGROUND: Aztreonam-avibactam (ATM-AVI) is an important therapeutic option for infections caused by metallo-β-lactamase-producing Enterobacterales. However, the emergence of resistance may limit its clinical effectiveness. Here, we report two high-level ATM-AVI-resistant Escherichia coli isolates recovered from critically ill patients in a tertiary hospital in Guangzhou, both of whom had prior exposure to broad-spectrum antimicrobial agents. METHODS: Antimicrobial susceptibility testing was performed by broth microdilution. Whole-genome sequencing (WGS) was conducted using the Illumina Nova

Ceftazidime/avibactam and aztreonam combined with an aminoglycoside combat metallo-β-lactamase-producing Klebsiella pneumoniae.

There is an urgent need for additional therapeutic solutions to treat serious bacterial infections caused by metallo-β-lactamase (MBL)-producing Klebsiella pneumoniae. Aztreonam, when partnered with ceftazidime/avibactam, has in vitro activity against most MBL-producing K. pneumoniae, but clinical outcomes may be suboptimal. The purpose of this study was to determine if aminoglycosides can enhance the pharmacodynamic activity of ceftazidime/avibactam plus aztreonam against MBL-producing K. pneumoniae. K. pneumoniae isolates harboring MBL genes (blaNDM, blaVIM, and blaIMP) were evaluated in tim

Development of spray-dried phage-aztreonam microparticles for inhalation therapy of Pseudomonas aeruginosa lung infections.

Despite considerable efforts in antibiotic therapy, pulmonary infections caused by Pseudomonas aeruginosa remain a major challenge, particularly in diseases such as cystic fibrosis. Bacteriophage-antibiotic combinations have recently emerged as a potential alternative due to synergistic effects and the ability to overcome resistance. However, phage instability and sensitivity continue to hinder clinical translation. In this study, we developed an inhalable spray-dried microparticle formulation co-encapsulating bacteriophages against P. aeruginosa and the β-lactam antibiotic aztreonam using man

Turbitest: an innovative rapid test for determining in vitro susceptibility to ceftazidime-avibactam plus aztreonam in Enterobacterales.

Treating carbapenem-resistant Enterobacterales, mainly those producing metallo-β-lactamases (MBL) is a subject of major concern. The combination aztreonam-avibactam (AZA) was developed in an attempt to overcome this challenge, as avibactam inhibits KPC and other serine-β-lactamases, enabling aztreonam (ATM) to act against MBL. However, in scenarios where AZA is not licensed so far, the combination of ATM with ceftazidime-avibactam (CZA) is an alternative that has been proved in vivo efficacy and in vitro synergic activity. We propose the TurbiTest, an alternative method based on turbidity meas

Case Commentary: Overcoming intrinsic resistance-the successful use of aztreonam and avibactam for Stenotrophomonas maltophilia meningitis.

Wong-So and colleagues describe a case of post-neurosurgical Stenotrophomonas maltophilia meningitis successfully managed with ceftazidime-avibactam and aztreonam. While aztreonam is stable against L1 metallo-β-lactamase-mediated hydrolysis, it remains susceptible to L2 serine β-lactamases, which are effectively inhibited by avibactam, restoring aztreonam's bactericidal activity. Simultaneous peak and trough levels in cerebrospinal fluid (CSF) and plasma demonstrated favorable CSF-to-plasma ratios for both aztreonam and avibactam. This case highlights aztreonam-avibactam as a rational therapeu

[The influence of a monobactam antibiotic, aztreonam, and other antibiotics on blood coagulation in vitamin K-deficient rats].

The influence of aztreonam (AZT), a new monobactam antibiotic, on blood coagulation was compared with latamoxef (LMOX), cefoperazon (CPZ), cefotetan (CTT) and ampicillin (ABPC). These antibiotics were administered intraperitoneally once a day for 7 days to rats fed a vitamin K-deficient diet. On the 8th day, blood coagulation activity and bacteria in the caecum were observed. 1) One hundred mg/kg and 300 mg/kg of LMOX prolonged the blood coagulation time and decreased normal prothrombin in a dose-dependent manner.
